Responsibilities
As a Bereavement Coordinator You will:
* Coordinate individual, group, and follow-up grief counseling for families and caregivers
* Oversee the development and improvement of the Bereavement Program
* Provide emotional support to hospice staff coping with work-related grief
* Offer bereavement information and referrals to community members
* Assist in planning activities, including wellness retreats and weekly support groups
* Participate in staff education, IDT meetings, and relevant organizational committees
* Ensure compliance with all state and federal regulations
* Pursue ongoing training to enhance professional skills and knowledge in grief support
Qualifications
* Bachelor's degree or equivalent theological degree or Master's degree in Social Work with qualifications of experience clinical grief counseling
* Experience with death and dying.
* Understanding of grief response and experience with counseling individuals and/or groups in loss.
* If making home visits, must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
* Knowledge and commitment to the hospice philosophy of care.
* Ability to work as a team member.
* Demonstrates good verbal and written communication, and organization skills.

As a Pharmacist you will:

Summary:
Acts as the general manager for a Plasma Collection Center and has overall responsibility for the center's operation. Works under general occasional guidance and general oversight of regional management. Compensation and Benefits:
This position is eligible to participate in up to 30% of the company bonus pool. How you'll contribute
Therapist performs social services functions to include assisting patients in meeting their bio-psychosocial needs while enabling them access to therapeutic treatment skills to achieve their optimal level of emotional health.

TRAVEL:

Travel must be able to be completed in a timely manner.
The range displayed is specifically for those potential hires who will work or reside in the state of California, if selected for this role, and may vary based on various factors such as the candidate's geographical location, qualifications, skills, competencies, and proficiency for the role. Salary Pay Range: $117,000 - $155,000 and eligibility for a sales incentive target of $39,000. The Registered Nurse (RN) in Home Health provides and directs provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies.
Coordinates total plan of care with other health care professionals involved in care and helps to achieve and maintain continuity of patient care by planning and exchanging information with physician, agency personnel, patient, family, and community resources. All done within a Point of Care setting.
Makes the initial and/or comprehensive nursing evaluation visit, determines primary focus of care, develops the plan of care within state specific guidelines, and submits accurate, complete, and timely documentation in collaboration with physician orders.
Performs patient comprehensive assessments at designated time points and develops the appropriate plan of care.
Ensures patients meet home health eligibility and medical necessity guidelines as defined by payer source.
License Requirements
Current RN licensure in state of practice
Current CPR certification
Current Drivers License, vehicle insurance, and access to a dependable vehicle or public transportation
