Provides support for people and/or groups of people with disabilities both in residential and day settings by performing and leading by example in carrying out Direct Support job responsibilities. Serves a mentor, trainer and coach to Direct Support Employees. Responsible for carrying out mentorship responsibilities for new Direct Support Employees to ensure thorough training and abilities. Assists with ensuring staff coverage of all three shifts as scheduled by the Residential Team Leader or as necessary due to scheduled staff not being able to work. At certain residences, the employee may be required to reside (sleep over) if he/she works a special schedule and/or work week. Assists the person(s) receiving services in identifying and communicating his or her requests and needs for services and supports. Implements services and supports to address the person's goals and desired outcomes as identified through each person's Person Centered Plan. Relies on knowledge, experience, and judgment to complete tasks with supervision and direction from the Residential Team Leader. Requires a high school diploma or GED and at least one year of experience working with the population served.

Responsibilities Include:
Assists people supported to identify personal outcomes that best suit his or her personal interests, cultural background, and desires for the future.
Assists the Interdisciplinary Team in identifying supports needed for personal outcomes to be realized.
Understands and promotes the people supported's goals, as well as personal dignity, respect, individualism and quality of life.
Provides opportunities for choice, and encourages people supported to make choices, and to exercise control over themselves and their environment.
Reports all incidents of people supported abuse, neglect or exploitation immediately
Ensures and assists with the cleanliness of group home by completing Environmental Assessments, work orders for maintenance needs, monitoring cleanliness of company vehicles and keeping home and grounds clean.
Supports people supported in eating, resting, dressing, bathing, grooming, toileting, playing, and working, according to each person's abilities and interests.
Helps people supported develop healthy personal and social habits.
Prepares, assists and instructs people supported in food preparation and cleaning of residences and vocational program areas.
Supports people supported in identifying and acquiring valued social roles in integrated community settings, including, but not limited to opportunities for membership in local groups, clubs, and organizations.
Instructs and assists people supported using techniques and strategies designed to improve sensory motor and perceptual motor development, perception, memory, language, cognition, and social and emotional development.
Works with each person receiving services to increase motivation; provides consistent reinforcement to learning, continuous assessment of level of functioning, and continuous feedback to each individual for all learning activities.
Trains people supported in self-medication administration according to company policy.
Requirements:
Must have a High school diploma or (GED).
Prefer experience working with IDD individuals.
Must have a valid driver's license (this is a condition of employment) including personal vehicle insurance coverage.
Must maintain proficiency in company sponsored training and certifications and privileging as required.
Maintain CPR and First Aid certification , Med Tech and NCI certifications and other core competency training that may be specified and/or required by future company regulations or service definitions of service being provided
Depending on service being provided must be clinically supervised by a qualified professional according to supervision requirements as specified by licensure or certification requirements of appropriate discipline or service being provided.
Must be able to demonstrate proficiency in CPR from floor level. Practicing CPR skills will require you to work on your hands and knees, bending, standing and lifting.
Must meet conditions of employment by agreeing to criminal record check, pre-employment drug screening, healthcare registry check, and motor vehicle registry check.

The CAP Case Manager provides critical case management services to beneficiaries who are at risk of institutionalization, ensuring their health, safety, and well-being are maintained through person-centered care planning and coordination of essential services.
The Case Manager works closely with families, RNs, and other interdisciplinary team members to assess needs, coordinate care, and provide ongoing support to help beneficiaries achieve the best possible quality of life.
DUTIES AND RESPONSIBILITIES:
Assessment and Care Planning:
Conduct initial pre-screening and assessments of beneficiaries and their families to evaluate medical, psychological, behavioral, financial, social, cultural, environmental, legal, vocational, educational, and other areas.
Consultation with the CAP beneficiary and primary caregiver to educate about waiver services, other Medicaid, and community resources to meet the beneficiaries' needs.
Identify needs to prevent health and safety factors to assist in maintaining community placement.
Develop and maintain individualized, person-centered care plans (emergency and disaster planning) to ensure the health, safety, and well-being of beneficiaries.
Review and update care plans at least every 12 months or when the status of the beneficiary changes.
Assist beneficiaries and families in understanding the plan of care and making informed choices.
Coordination of Services:
Link beneficiaries and their families to necessary services, equipment, and supplies to support care in the home.
Collaborate with community resources, healthcare providers, and other agencies to ensure comprehensive care delivery.
Initiate appropriate referrals and utilize community resources for planning and service coordination.
Monitoring and Follow-Up:
Provide ongoing monitoring of services through monthly phone calls and home visits, documenting observations, and beneficiary progress.
Evaluate the effectiveness of care plans and services, recommending or implementing changes as needed to achieve desired outcomes.
Maintain accurate, up-to-date case management documentation within the system, ensuring compliance with state and agency guidelines.
Counseling and Support:
Provide emotional support and basic counseling to beneficiaries and their families to strengthen their support systems.
Assist families in navigating challenges, including long-term palliative care, behavioral issues, and medical needs.
Documentation and Compliance:
Assist in obtaining documentation from medical staff to confirm the need for specific CAP services.
Maintain medical records for each beneficiary, ensuring documentation of current status, service changes, and referrals.
Ensure compliance with 10A NCAC 27G.0202 and other regulatory guidelines.
Complete all required records per agency policy and the State CAP manual, including discharge summaries when CAP services are completed.
Review and ensure proper billing codes and compliance for case management, in-home aide documentation, paid live-in caregiver, re-certifications, and supply billing.
Participate in NC Medicaid-certified training programs and ensure program compliance within 90 days of employment.
Collaboration and Advocacy:
Work closely with RNs and interdisciplinary team members to ensure a comprehensive approach to beneficiary care.
Participate in case discussions and provide input to ensure quality care and service delivery.
Advocate for program participants and their families to secure necessary resources and services.
Serve as a liaison between beneficiaries, families, and external providers to address care needs effectively.
Provide training and support to families to empower them in managing their child's care.
Continuing Education and Professional Development:
Complete all state-mandated training and agency-required continuing education annually.
Stay current on CAP guidelines, best practices, and new developments to enhance service delivery.
Ensure timely updates to the CAP Business system and other documentation systems as required.

SUPERVISORY RESPONSIBILITIES:
This position has no supervisory responsibilities.
MINIMUM QUALIFICATIONS:
Strong understanding of Medicaid programs and compliance requirements.
Excellent communication and interpersonal skills to work effectively with families and multidisciplinary teams.
Ability to manage multiple priorities and maintain detailed records.
Experience working with medically fragile children or in pediatric healthcare settings and physically disabled adults with complex care needs.
Education and Experience:
Candidates must meet one of the following criteria:
Social Work Background:
Bachelor's degree in social work from an accredited school of social work.
Minimum of one year of directly related community experience, preferably case management, in the health or medical field (homecare, long-term care, or personal care).
Completion of an NC Medicaid-certified training program within 90 calendar days of employment.
Human Services Background:
Bachelor's degree in a human services or equivalent field from an accredited college or university.
Minimum of two years of community experience, preferably case management, in the health or medical field (homecare, long-term care, or personal care).
Completion of an NC Medicaid-certified training program within 90 calendar days of employment.
Non-Human Services Background:

Nursing Background:
Current North Carolina Registered Nurse (RN) license with a two-year or four-year degree.

Note: An individual with a bachelor's degree or who holds a nursing license as described above, without the number of years of experience, may be designated as an apprentice and shall be hired to act in the role of case manager. Job Responsibilities

DUTIES AND RESPONSIBILITIES:
Conducts periodic audits of all service delivery settings, service records, personnel, requirements, and review of services for each person supported to ensure compliance with state and federal regulations and company policies. In addition to reviewing the record, an audit may include an interview with the person supported, direct support staff, professional staff, and management staff.
Observation of the implementation of Individual Services Plans (ISP) will also occur, along with assessing the condition of the home or administrative unit, and company vehicle for cleanliness, repair, and safety. Completes an audit report of findings in a specified format to facilitate quality improvement.
Conducts follow-up audits to ensure needed corrective action has been taken for any identified issue.
Participates in the unit's monthly Quality Assurance and Performance Improvement (QAPI) meeting, providing guidance in data analysis for determining trends and assisting in the development of a Quality Improvement Plan (QIP) to ensure that corrective action is specified and tracked to resolution of the issue/trend. Provide ongoing training to the Clinical and Management teams in quality improvement, assisting with satisfaction surveys to determine strengths in service provision as well as areas that are opportunities for improvement.
Participates in state and regional meetings as needed to stay abreast of changing regulations. Attends company meetings as scheduled. Assists in the development of materials and information for dissemination at meetings. Coordinates Statewide meetings at the direction of the Director of Quality Assurance and/or CCO.
Assists with projects, additional audits, updating forms, staff training, policy development and any other task that may be requested by the Director of QA. Assists in investigations of abuse, neglect and exploitation as assigned. Completes special projects as directed by the Chief Compliance Officer.

MINIMUM QUALIFICATIONS:
Bachelor's Degree in a Human Service Field
Five or more years of experience with the population served.

Our Deaf and Hard of Hearing (DHH) Therapists provides comprehensive clinical assessments and evaluations, as well as medically necessary outpatient therapy services to deaf and hard of hearing adults and children with mental health, developmental disabilities and/or substance abuse challenges so they may attain personal outcomes and lead full, meaningful lives that they envision.
Our DHH Therapists rely on experience, knowledge and education with limited direction to provide quality services to the Deaf and Hard of Hearing populations.

Education/Experience/Certifications/Licenses
The minimum requirement for this position is either:
A Doctorate Degree in Psychology, Social Work, Education, Nursing, Sociology, or related field with at least two years of mental health, developmental disabilities, and/or substance abuse clinical services experience; or
A Masters Degree in Psychology, Social Work, Education, Nursing, Sociology, or related field with at least two years of mental health, developmental disabilities, and/or substance abuse clinical services experience; or
Licensure or provisional licensure in one or more of the following categories: Psychologist (PhD-level), Licensed Psychological Associated (LPA), Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), Licensed Professional Counselor (LPC), Nurse Practitioners approved to practice and certified by the American Nurses Credential Center as an advanced practice nurse practitioner in psychiatric nursing.

Job Responsibilities:
Evaluates the functioning level and needs of service users from an educational perspective using formal and informal diagnostic tools.
Participates as a member of the Interdisciplinary Team in the development of each service user's individual program. Works with and communicates with the interdisciplinary team to ensure provision of quality services.
Develops/implements educational training programs as established by the Interdisciplinary Team.
Conducts and monitors training and behavioral programs.
Designs curriculum and activities for the classroom.

Maintains harmony among co worker, management and service users. Serves as a role model for service users.

Demonstrates competence in State and Federal standards for Therapeutic/Foster Care Services, licensing rules
Assist with recruiting qualified foster parents and staff necessary to meet the needs of the youth served.
Assists with ensuring Foster Parents and assigned staff complete and maintain all training requirements.
Provides pre-service and ongoing training to foster parents to ensure parents possess skills necessary to provide quality services to the youth being served.
Serves as liaison to DSS, DMH and other community agencies regarding any service planning issues and needs of and services to youth being served in Therapeutic/Foster Care program.
Processes requests for services by conducting intakes, obtaining/completing necessary assessments, completing social histories, obtaining consents, and collecting documentation for the determination of appropriate placement and services for youth referred to program.
Writes, coordinates, implements, and monitors person-centered service plans and ensures quality care of youth being served.
Serves as advocate for the youth in care and participates in the interdisciplinary team process in the development of the Person Centered Service Plan
Ensures service authorizations are current and provides necessary updates to authorizing authorities in timely manner as requested and/or required for service approval.
Provides mediation between the youth and the foster parents, guardians, natural family members, and other agencies relative to the needs and desires identified by the team.

We are seeking an organized and positive Program Manager to coordinate and monitor the array of services and supports needed to address the goals and desired outcomes of the people we serve. As a Program Manager with RHA, you will supervise Group Home Managers and Direct Care Associates as you spend time at our residential or office locations to train staff on aspects of residential management and direct care.
Job Responsibilities
As a Program Manager with RHA, you will serve as the center of the interdisciplinary service team, which consists of nurses, Direct Care Associates, the Administrator and Group Home Manager, to assists in the management, development and oversight of the individual support plan. Ensures that the individual support plan is implemented as written and initiate monthly services reviews to record and determine progress. The Program Manger also assists in supervision and oversight of day, residential, and personal assistant services. Collaborates with ISC's, Therapist, and members of the clinical management team by performing the following duties.
Additional responsibilities of the Human Services / Support Coordinator or Program Manager include:
Supervises and monitors the implementation of the Individual Support Plan (ISP).
Analyzes and summarizes status of progress by completing monthly service reviews, determining appropriateness of programs and recommending changes as appropriate.
Communicates regularly with the Independent Support Coordinator and other members of the circle of support if amendments to the ISP are needed.
Ensures all aspects of the residential activity, including appearance of home, leisure materials, cleanliness and adequate staffing, documentation, etc. are operating smoothly.
Documents all communication by utilizing the Program Manager Contact Notes Form.
Ensures no deficiencies are noted from State and Federal Inspection & Survey teams.
Spends time on a regular basis at the location where services are being performed.
That means regular announced and unannounced visits in residential homes including each shift to ensure policy, procedures and program data are in place and being implemented appropriately.
Incumbents are expected to provide coverage for shortages of direct support personnel but while doing so continue to be responsible for their management duties.
May assist in on-call rotation as specified by the Administrator, including weekends and holidays.
Assumes duties and responsibilities in the absence of the Home Manager.
Trains and in-services direct care staff as needed.
