The SRSP Recovery Manager is responsible for working with individuals and their care team to develop a person-centered care plan to address their home and community based needs.
Essential Functions:
Document current list of medications and assist with medication adherence (linkage/resources).Conducts annual and event-based assessments of an individual's eligibility according to program specifications, taking into account the cultural and linguistic needs of each member.Engage with the member in a variety of settings to establish an effective, professional relationship. Education and Experience:

Position Summary:

Arizona Care Network (ACN) is a clinically integrated (CI) physician network and Accountable Care Organization (ACO) sponsored by Dignity Health and Abrazo Health. The Program Manager, Post-Acute Services is an employee of Dignity Health Managed Services Organization (DHMSO), a wholly owned subsidiary of Dignity Health.

The Post-Acute Services Program Manager is responsible for leading a variety of operational and collaborative relationships related to post-acute services offered by ACN, including post-acute network design, post-acute care coordination services, post-acute provider relationship management, waiver program implementation and maintenance, post-acute program metric selection and reporting to internal and external stakeholders, as well as hospital system case management program partnerships.

Responsibilities may include:
- In collaboration with the Director, Provider Network establishes/refines selection criteria and requirements for participating in ACN, and is the primary point of contact for conducting contracting activities with post-acute care provider organizations.
- Responsible for initiating, onboarding, developing relationships and serving as the primary interface with ACN preferred post-acute facilities and provider organizations, including Skilled Nursing Facilities, Home Health Agencies, Rehabilitation Hospitals, Hospice, etc.
- Serves as key partner with clinical and administrative leaders throughout the post-acute network to identify opportunities and drive improvement in key areas such as quality, cost of care, care coordination, network coordination, patient/consumer experience.
- Leads the implementation, training, maintenance and evaluation of ACO waiver options available to ACN.
- Prepares for and facilitates ongoing Post-Acute Care Committee meetings by working closely with ACN leadership and the Committee Chair.
- Engage and collaborate with Dignity Health and Abrazo Community Health Network case management and physician leadership to maximize ACN’s post-acute care program by serving as the primary external interface to develop relationships with hospital and SNF case management to facilitate care transitions to the appropriate setting.
- Works collaboratively with ACN’s Director of Analytics and with appropriate data/analytics vendors to design and build post-acute care program metrics and dashboard reports that allow for regular tracking of program performance, and the implementation of improvement opportunities throughout the network.
- Participates in the periodic evaluation of technologies and other solutions being considered by ACN for post-acute providers.
- Other duties as assigned.
Qualifications:
Minimum Qualifications:

- Minimum 3 years’ experience in clinical operations in a hospital or post-acute care setting, or as a Case Manager in a hospital, nursing home, or health plan setting.
- Bachelor’s Degree - BS/BA in Business/Healthcare or equivalent combination of education and experience required.
- Must have excellent verbal and written communication skills; Must be able to communicate and interface effectively with internal and external customers. Able to summarize data findings concisely for internal and external stakeholders; able to make inferences and offer suggestions for improvements. Proficiency with the Microsoft Office suite of applications.
- A solid knowledge of budgeting and financial practices.
- Special skills:
Strong customer service skills with willingness to take initiative with lite to no direction. Demonstrated ability to analyze problems and issues from a variety of perspectives and to understand legal, privacy, risk, and operational impact of decisions.
