A comprehensive assessment and periodic reassessment must be conducted according to the program operations manual to determine the client's need for medical, social, educational, or other services. Assist the client and families with organizing, documenting and scheduling appointments in areas identified in the treatment plan. Provide referral or related activities to help client obtain needed services with medical providers, social providers, educational providers, or other programs and services. Coordinate home visits, discharge planning, and provide monitoring or follow up activities necessary to ensure the client's care plan is implemented and needs are being met . To have contacts with non-recipients who are directly related that have useful input regarding the client's past, or current functioning, symptoms, adherence to treatment, or other information relevant to the client's behavioral health condition and needs for services. Assist with the medical needs of the client, securing guardian approval if needed for medication changes and additions, including phone calls and follow up letters as needed. Ensure enrollment of client in an educational program, attend school meetings and Parent/Teacher conferences and complete documentation accordingly. Ensure client has crisis numbers for case manager, mental health center, psychiatrist or other support that may be needed and information to call 911. Attends meetings and court hearings regarding client's assigned to caseload. Provide transportation for clients to various appointments, court, meetings and home visits, as needed. Position may require services to be rendered in the home of the client at times. TRAINING/KNOWLEDGE/SKILLS: 5% Knowledgeable of effective coping skills, crisis intervention strategies, motivational interviewing, stages of change, and community resources. Must have the knowledge and awareness of suicide prevention and have the ability to screen for suicide risk. Obtain case management training within 6 months of hire and maintain continuing education requirements pursuant to 908 KAR 2:260. To maintain professional boundaries with client, family members, legal guardians, community resources, referral sources, and community partners at all time. Attend required trainings as assigned.
