The Speech Therapist in Home Health is responsible for the assessment and evaluation of patient care needs related to treating speech and language disorders and functional training in communication, swallowing, and cognitive impairments.

Position Summary:
The Speech Language Pathologist (SLP) is responsible to provide quality clinical care activities for pediatric patients (birth to 21) within the scope of practice as defined by the state of Florida and the American Speech Language and Hearing Association.
Works as part of a multidisciplinary team at the hospital and/or outpatient care centers.

Minimum Qualifications:

Speech-Language Pathologists (SLP) evaluate and provide skilled speech-language therapy to individuals in a home setting, under the care of an attending physician. At Enhabit Home Health & Hospice, the home setting may include single or multi-family homes, assisted living, independent living, or memory care.
The SLP evaluates and treats patients with language impairments, motor speech disorders, dysphagia, cognitive communication difficulties, voice disorders, and those requiring alternative or augmentative communication. Design and implement Social Club programming for Autistic students using Social Thinking methodologies.
Assessment and Documentation
Conduct initial and ongoing evaluations, screenings, and assessments to identify communication disorders.
Develop, write, and update IEP goals and present levels of performance in collaboration with the IEP team.
Maintain clear, accurate, and confidential records including session notes, progress reports, and compliance documentation.
Ensure therapy and documentation adhere to IDEA regulations and school policies.
Collaboration and Consultation
Provide support to general education and special education staff with classroom strategies and interventions for at-risk or struggling students.
Participate actively in student support meetings, IEP meetings, and family consultations.
