7. Complete within three visits of admission a comprehensive biopsychosocial assessment of all clients using an assessment instrument which assesses, but not limited to (i.e. medical status, vocational/employment support, alcohol, tobacco and other drugs, legal status, family status, psychiatric status, and behavior risk factors for HIV and hepatitis. (N.J.A.C.-
10:161B-9.1 (a)).
8. If applicable, engages in full compliance with the Scope of Practice of an LSW, LAC, and LCADC but not limited to: Social Work Services, Clinical Social Work Services, Professional Counselor/ Associate Counselor, and Alcohol and Drug Counselor under the supervision of an LCSW, LPC or LCADC (N.J.A.C. 13:44G-3.2, N.J.A.C. 13:34-10.1 and N.J.AC. 13:34C 1.2-1.2).
9. If applicable, will complete DSM-5- TR diagnoses.
10. Initiate and complete the development of a measurable client treatment plan upon the client's admission and enter the client's treatment plan into the client record within 72 hours following the client's admission (N.J.A.C 8:111-9.2(a)1).
11. Revision and/or updates of client treatment plan(s) as assessments are completed, and/or new client information is obtained or on an as-needed basis (N.J.A.C. 8:11119.2(a)1i
12. Identify and document goals for discharge, which shall be incorporated in the client's treatment plan upon admission to the facility and shall address problems identified at admission and during treatment (N.J.A.C 8:111-18.1(a)1, 10:61B-17.1(a)1).
13. Initiate the development of the client's treatment plan upon admission and shall be entered into the client's record at least after three visits following the admission not to exceed 30 days (N.J.A.C.- 10:161B-9.2(a)1).
14. Initiate, implement, review and revise client's treatment plan to include DSM-5TR diagnosis, ASAM Level of Care Assessment, measurable goals, objectives and treatment outcomes (N.J.A.C.-10:161B-6.2 (a)11).
15. Shall document evidence of client participation in the development and implementation of the treatment plan, including, but not limited to, dated signatures of client as well as signatures of participating multidisciplinary team members (N.J.A.C.-10:161B-9.2(a)2vii).
16. Facilitates psycho-educational and/or process therapy group sessions on substance use and recovery-related topics in the Data, Assessment, Plan (DAP) documentation note taking format.
17. Provide detailed daily progress notes for all individual, group, and/or family counseling services in clients' electronic health record (EHR) at the close of the business day.
18. Provide case management duties (i.e., make referrals and coordinate scheduling of clients for evaluations and follow-up appointments with Eva's Village's psychiatrists and/or APN or other community psychiatrists/behavioral health providers).
19. Performs Level of Care (LOCI) extension requests utilizing the New Jersey Substance Monitoring System (NJSAMS) and American Society of Addiction Medicine Criteria (ASAM) 4th Edition.
20. Participates in a case conference with multidisciplinary treatment team to determine the best course of treatment for clients and to address all the client's needs. Shall review the treatment plan and client's progress at least every 30 days, with such review and revisions, if any documented in the client's electronic health record (N.J.A.C. 8:111-9.2d
21. Incorporates results of random drug and alcohol screening into client interventions and treatment planning process upon review with the client (N.J.A.C. 8:111-9.2(e), 10:161B-9.2(e)).
22. Shall document in the client's clinical record, the client and the client's family (if indicated and considered appropriate), participation in the development of the client treatment plan and continuum of care plan (N.J.A.C 8:111-9.2 (c) 1.
23. Maintains effective intra-office and inter-office communication within departments.
24. Participate in clinical staff meetings within the agency.

documentation for mental health services provided.

QUALIFICATIONS
EDUCATION:
Bachelor's degree in counseling or related counseling field required.
Master's degree in counseling or a related counseling field preferred.
Certified Alcohol and Drug Counselors (CADC), required.
Licensed Clinical Alcohol and Drug Counselors (LCADC), or other licensed mental health professionals (LAC/LSW/LMT, LPC/LCSW/LFMT), preferred.
EXPERIENCE: Minimum one to three years' experience in the Behavioral Health field with experience in assessment, crisis intervention, and facilitation of individual and group counseling sessions

Obtain appropriately signed consents when soliciting from or providing information to outside sources to protect client confidentiality and rights.
Provide Orientation to clients following the orientation manual, on an individual and/or group basis within the first week of treatment.
Initiate an initial treatment plan within 72 hours of admission, and the complete a master treatment plan within the first four weeks of treatment. Treatment plans must be completed in collaboration with the client and updated a minimum of once per month.
Treatment plans should be client-centered, recovery oriented, and measurable.
Assessing clients using the standardized or evidence-based assessment tools and diagnosing clients for substance use disorders using the DSM 5 under supervision.
Gather relevant history from client including by not limited to alcohol and other drug abuse using the appropriate assessment tools and interview techniques.
Evaluate psychological, social, and physiological signs and symptoms of substance related and mental health disorders using appropriate assessment tools.
Identify any existing conditions (medical, physical, psychiatric, etc.) that indicate need for additional professional assessment and/or services and make appropriate referrals.
Obtain previous records that are relevant to the current treatment episode upon securing proper consent to release information form
Develop an Interpretive Summary within the first week of treatment.
Identify the appropriate therapeutic approaches and resources to be utilized with each individual client.
Develop and review discharge plans at least once per month.
Individualize counseling in accordance with cultural, gender, and lifestyle differences.
Prepare and discuss clients' clinical cases with the clinical team and explain the rationale for all actions taken throughout the course of treatment including any case management activities done for the client.
Identifying the needs of a client that cannot be met by the counselor or agency, coordinate services for the client, and assisting the client to utilize the support systems and community resources available.
Seek a consult with supervisor whenever issues are identified that are beyond your base of knowledge and skills in accordance with the agency policies and procedures
Consult with supervisor to ensure the provision of effective treatment services.
Utilize clinical staff meetings, case conferences, direct supervision, and clinical supervision to ensure proper quality of care for client.
Implement an immediate course of action appropriate to a crisis under the direction of the supervisor and in accordance with the agency policies and procedures
Transport clients, as necessary.
Manage an assigned case load of twelve clients.
Keep all records in designated place to ensure safety and confidentiality in accordance with the agency policy and procedures.
Send documentation pertaining to client's admission, participation, progress and discharge status and other relative data to referral sources keeping in mind the time deadlines and authorization for release of information in accordance with the agency policy and procedures.
Conduct a peer-review of clinical records, review folders and provide a report of the findings to the Director.
Obtain random urine sample for drug screening as assigned.
Supervise clients while they self-administer their medication as assigned.
Procedures must be followed to ensure that any required reporting is done immediately to the Director of the Program or Delegate, by completing an incident report in Accreditation within 24 hours.
When driving an agency car inspect if for safety, fill the mileage log, ensure that it is kept clean and refrain from smoking while in it. Operation of the company vehicles should be conducted in a safe manner adhering to New Jersey vehicle operation laws.
Orientate, train and supervise the House Committee in accordance with the modified TC model.
Complete Relias trainings in accordance to the agency policies and procedures

Services include assessment, brief counseling and consultation, action planning, referral to resources (assuring linkage as appropriate), and follow-up as indicated.
CYB counselors will also have a focus on supporting the staff and personnel of DoDEA , community schools, summer camps (if available/assigned) as well as providing parent training and guidance.
Provides training and health and wellness presentations, participates in health fairs and other base/installation activities focused on children and youth.
Enters counselor activity data daily through smart phone, or web application, to assure that reporting is accurate from assigned installation while maintaining confidentiality and anonymity of service/family member.
Establishes and maintains working relationships with community resources focused on children and youth and provides appropriate linkages. Consistently visible within the school which allows MFLC services to be accessible when working with children, the counselor must abide by line of sight protocol (another adult with responsibility for the child must be able to see the counselor and child they are working with at all times).
Partners with POC to provide CYB services in a manner that addresses the needs of the installation/facility. Develops an effective and professional working relationship with the installation/facility POC.
Manages duty to warn and restricted reporting situations according to DoD protocol and staffs the cases with Regional Supervisor/Regional Director.
Communicates with Regional Supervisor and participates in regular individual and group supervision, sharing information regarding trends and issues on the installations and in facilities to which they are assigned. Responds to critical incidents and special requests as directed by the POC and approved by the Office of Secretary of Defense (OSD) program manager. The CYB will flex schedule to respond to urgent requests as needed or at the request of the Regional Supervisor/Regional Director.
Participates in regular in-services/training, Quality Improvement committees, initiatives, studies, pilot programs other contract activities as assigned/appropriate as directed by the customer and/or Magellan. This includes participation in pilot program certification and training processes, completion of activity documentation, integration of pilot activities in non-medical counseling work, and the utilization of applicable technology to complete required activities.

Summary Description:
The Functional Family Therapy Child Welfare (FFT-CW) Clinician is responsible for providing FFT services to children and families involved in the child welfare system. This evidence-based practice will be conducted in the family's home setting. The goals of this short-term intervention are to improve family functioning, improve communication, reduce conflict, decrease problem behaviors, reduce hospitalization, reduce out of home placements and delinquency, improve parenting skills, and increase effective problem-solving skills. The Clinician must have the ability to internalize, work within the FFT philosophy, and model as well as being able to adapt and individualize program intervention to utilize family strengths and skills. The Clinician must be eligible and maintain a certification as a Functional Family Therapist Clinician. Training and certification will be provided during the first year of employment. The Clinician will support recovery by providing services that reflect our deeply held values that include the recognition of individual rights of self-determination, choice, shared decision-making and collaboration. The Clinician will utilize this evidence-based practice; always embracing a trauma-informed and person-centered approach.
Key Responsibilities:
Provide direct clinical services to approximately 8-9 families each month which may include, but are not limited to, outreach, assessment and treatment planning, weekly family therapy, crisis counseling and intervention, and community consultation and education.
Coordinate treatment with other community agencies and service providers and participate in meetings as needed.
Participate in program development, training programs and/or meetings as designated by Program Coordinator.
Maintain up-to-date and complete case records, including documentation, and provide written reports and statistics as required.
Complete the required number of client contacts per month.
Follow the established agency policies and procedures and meet all guidelines set forth by the Department of Social Services contract.
Provide 24-hour telephonic crisis intervention services on a rotating basis
Due to the nature of the program, must have the ability to be flexible with working hours.

Oversees the provision of clinical services to select CLS residences and day program services. Develops or oversees development of behavioral goals for individuals based on functional analyses and behavioral observations. Establishes specific program goals and carries out follow up activities and monitoring of individuals' progress. Reviews and compiles recorded data to develop progress notes and to discuss with treatment team and individuals any needed revision of planning.
Performs as-needed direct counseling designed to assist individuals identify and alleviate problem areas, relieve stress, improve socialization and interpersonal relationships, and develop coping skills and attitudes. Oversees individuals' groups to facilitate community integration and independence within these facilities.
Prepares Informed Consent and Human Rights Committee information for individuals who have behavior support plans and psychotropic medications or restrictive plans on annual basis or as needed when changes occur.
Works with psychiatrists (or PNPs) to review progress with individuals in order for the physician to have current behavioral reports and other information regarding the individuals' status. Advises prescriber of changes in individuals progress, impact of medications and behavioral observations.
Carries out a variety of in-service and education programs for direct care staff or professionals, and at team meetings regarding individual's needs, progress made, and the reasoning behind behavioral plans and methodology for implementing the plans. Provides training to department staff regarding individuals psychiatric diagnoses, as applicable and other instruction to assist them in interaction with individuals.
Carries out training to assist staff and individuals to perform their functions more effectively, including use of role playing, practicing tasks, and discussions of other activities which will be undertaken. May provide education and training outside the agency.
Prepares reports and data regarding activities with the individuals, including progress, analysis of strengths or deficits, and other characteristics required to prepare a comprehensive psychological profile for use by other professionals.
Participates in direct crisis intervention as may be necessary when conflicts or problems arise; provide direct guidance, counseling, and intervention using appropriate techniques and skills to address a particular event.
