Timely files any governmental claims via certified and return receipt mail and requests a conformed copy of each claim filed. Calendars the “45 day from date the claim was mailed out” in the case management system. Assesses if additional treatment is needed and/or approved on each case. -Reviews the police reports and medical records obtained by the Records Clerk. On Serious Injury Cases (SI), forwards these reports to the Team Attorney. For Serious Injury (SI) Cases: Follows up with Records Clerk in order to obtain the police report, establishes liability, requests the policy limits of the defendant, obtains the client declarations page, obtains any important evidence (i.e. PD photos, injury photos, videos, witness statements, etc.) - For Pre-Procedure Evaluations: Drafts the PPE questionnaire, obtains the reports, bills and estimated cost or each procedure, obtains the defendant's policy limits, runs and ISO on the client, and provides all information to the Lead Attorney for their review. For surgery evaluations runs a Criminal Background Check (CBC). -Contacts client to over the ISO and/or CBC with client to confirm the information and/or obtain any pre-ret treatment regarding any previous accidents. Adds a note regarding the ISO/CBC review. -CM will obtain medical provider full contact information, prior attorney information and provide this to their respective RC for cases where there are pre-existing accidents as they will request the records. Job Description The Substance Use Counselor (SUC) is responsible for providing adult client supervision in a shelter or recuperative care facility, and assisting in the maintenance of a safe, secure environment that enhances behavioral health and substance use counseling while utilizing the harm reduction and recovery model to all clients being served. The Male Community Reentry Program (MCRP) is a residential facility with a capacity of 200 beds, designed for adult males who have less than two years remaining on their prison sentences. Instead of being incarcerated in a state prison, eligible individuals can complete the remainder of their sentences in the community through MCRP. The program offers a variety of rehabilitative services focused on substance use disorder (SUD), mental health care, primary care, employment, education, housing, family reunification, and social support. Treatment for substance use disorders and mental health issues provided by MCRP typically lasts from 6 to 12 months, ensuring comprehensive support for participants. The Intake Coordinator I conducts the initial evaluation of potential clients to assess their requirements for Substance Use Disorder (SUD) services, completes admission paperwork, and creates the initial Treatment Plan. They ensure a smooth transition for clients to their treatment team, ensuring that new clients are stabilized, and immediate needs are met. The Intake Coordinator I collaborates closely with counselors, case managers, therapists, and other team members to organize and oversee client concerns, make necessary referrals, and offer educational support. This is a union position. KEY RESPONSIBILITIES Assessment and Client Care Welcome clients to the program and ensure high-quality customer service is provided. Facilitate admissions and discharges. Coordinate client referrals and verify services as necessary. Conduct thorough initial assessments to determine candidates' needs. Ensure immediate client needs are addressed, and appropriate referrals are made. Assist clients in applying for Medi-Cal and/or County funding if necessary. Develop and implement Initial Treatment Plans as required. Facilitate smooth transitions by providing warm handoffs to residential treatment staff. Process billing and verification activities accurately and reconcile discrepancies. Write and finalize progress notes within a specified timeframe. Ensure completion of all necessary documentation related to admissions processes. Maintain compliance with relevant registration or certification standards. Uphold internal and external regulatory requirements such as HIPAA, agency policies, and other applicable standards.
