Complete within three visits of admission a comprehensive biopsychosocial assessment of all clients using an assessment instrument which assesses, but not limited to (i.e. medical status, vocational/employment support, alcohol, tobacco and other drugs, legal status, family status, psychiatric status, and behavior risk factors for HIV and hepatitis. (N.J.A.C.- 10:161B-9.1 (a)). 8. If applicable, engages in full compliance with the Scope of Practice of an LSW, LAC, and LCADC but not limited to: Social Work Services, Clinical Social Work Services, Professional Counselor/ Associate Counselor, and Alcohol and Drug Counselor under the supervision of an LCSW, LPC or LCADC (N.J.A.C. 13:44G-3.2, N.J.A.C. 13:34-10.1 and N.J.AC. 13:34C 1.2-1.2). 9. If applicable, will complete DSM-5- TR diagnoses. 10. Initiate and complete the development of a measurable client treatment plan upon the client's admission and enter the client's treatment plan into the client record within 72 hours following the client's admission (N.J.A.C 8:111-9.2(a)1). 11. Revision and/or updates of client treatment plan(s) as assessments are completed, and/or new client information is obtained or on an as-needed basis (N.J.A.C. 8:11119.2(a)1i 12. Identify and document goals for discharge, which shall be incorporated in the client's treatment plan upon admission to the facility and shall address problems identified at admission and during treatment (N.J.A.C 8:111-18.1(a)1, 10:61B-17.1(a)1). 13. Initiate the development of the client's treatment plan upon admission and shall be entered into the client's record at least after three visits following the admission not to exceed 30 days (N.J.A.C.- 10:161B-9.2(a)1). 14. Initiate, implement, review and revise client's treatment plan to include DSM-5TR diagnosis, ASAM Level of Care Assessment, measurable goals, objectives and treatment outcomes (N.J.A.C.-10:161B-6.2 (a)11). 15. Shall document evidence of client participation in the development and implementation of the treatment plan, including, but not limited to, dated signatures of client as well as signatures of participating multidisciplinary team members (N.J.A.C.-10:161B-9.2(a)2vii). 16. Facilitates psycho-educational and/or process therapy group sessions on substance use and recovery-related topics in the Data, Assessment, Plan (DAP) documentation note taking format. 17. Provide detailed daily progress notes for all individual, group, and/or family counseling services in clients' electronic health record (EHR) at the close of the business day. 18. Provide case management duties (i.e., make referrals and coordinate scheduling of clients for evaluations and follow-up appointments with Eva's Village's psychiatrists and/or APN or other community psychiatrists/behavioral health providers). 19. Performs Level of Care (LOCI) extension requests utilizing the New Jersey Substance Monitoring System (NJSAMS) and American Society of Addiction Medicine Criteria (ASAM) 4th Edition. 20. Participates in a case conference with multidisciplinary treatment team to determine the best course of treatment for clients and to address all the client's needs. Shall review the treatment plan and client's progress at least every 30 days, with such review and revisions, if any documented in the client's electronic health record (N.J.A.C. 8:111-9.2d 21. Incorporates results of random drug and alcohol screening into client interventions and treatment planning process upon review with the client (N.J.A.C. 8:111-9.2(e), 10:161B-9.2(e)). 22. Shall document in the client's clinical record, the client and the client's family (if indicated and considered appropriate), participation in the development of the client treatment plan and continuum of care plan (N.J.A.C 8:111-9.2 (c) 1. 23. Maintains effective intra-office and inter-office communication within departments. 24. Participate in clinical staff meetings within the agency. 25. The director of substance abuse counseling must maintain an active client caseload if the director of substance abuse counseling is to be counted in the above ratios. Each substance abuse counselor shall be either an LCADC or CADC or another licensed health professional doing work of an alcohol and drug counseling nature within their scope of practice. A CADC must work under the supervision of an LCADC or another New Jersey licensed clinical professional designated as a qualified clinical supervisor per N.J.A.C. 13:34C-6.2. A CADC cannot diagnose substance abuse. Substance abuse counseling staff without an LCADC or CADC status or who do not possess another New Jersey clinical professional license that includes work of an alcohol or drug counseling nature within their scope of practice shall function as alcohol and drug counselor-interns or credential-interns and shall: Be enrolled in a course of study leading to a CADC or LCADC, or another New Jersey clinical professional license that includes work of an alcohol and drug counseling nature within its scope of practice, without regard to changes in employment, with progress towards certification or licensing on file, reviewed by the facility at least semi-annually and documented; and Be trained, evaluated and receive continuing formal clinical supervision by the director of substance abuse counseling or designee, pursuant to the clinical supervision rules of the State health professional licensing board for the course of study in which they are enrolled: (1) The State Board of Marriage and Family Therapy Examiners' for licensed marriage and family therapists; (2) The State Board of Marriage and Family Therapy Examiners' Professional Counselor Examiners Committee for licensed professional counselors and licensed associate counselors; (3) The State Board of Marriage and Family Therapy Examiners Alcohol and Drug Counselor Committee for LCADCs and CADCs; (4) The State Board of Psychological Examiners for licensed psychologists; and (5) The State Board of Social Work Examiners for licensed clinical social workers and licensed social workers. (b) Counseling staff employed in a residential substance use disorders treatment facility subsequent to July 15, 2013 shall have three years from the date of employment to become certified as an LCADC or CADC, or another clinical licensed professional that includes the work of an alcohol and drug counseling nature within its scope of practice. (c) Only staff possessing the appropriate clinical background and educational qualifications from the appropriate clinical discipline may provide the diagnosis, assessment and treatment of clients with co-occurring disorders. (d) Each substance abuse counselor shall be responsible for the following: Assessing the counseling needs of the clients; Assessing clients using the ASI or using other standardized assessment tool and diagnosing clients for substance disorders using the DSM; Determining the appropriate level of care based on ASAM PPC; Obtaining previous records that are relevant to the current treatment episode; Collaborating with the client in order to develop a written treatment plan that is client-centered and recovery oriented and includes goals and measurable objectives. The director of substance abuse counseling shall develop, monitor and provide clients and the treatment team with written schedules that will be used to update each client treatment plan(s) in order to ensure that each client's treatment needs are addressed; Providing the substance abuse counseling services specified in the client treatment plan; Reviewing clients throughout the treatment episode according to ASAM PPC, to determine the need for continued services or discharge/transfer; Reviewing and, where necessary, revising the substance abuse counseling portion of the client treatment plan to address emerging problems; Developing the client discharge/transfer plans to ensure movement to the appropriate levels of care; Contacting referral sources, providing case consultation, and coordination with referral sources (for example: mental health treatment providers, criminal justice agencies, schools, employers, the Department of Children and Families); Participating as a member of a multidisciplinary team for assigned clients; Providing active case consultation; and Documenting all counseling and education services, assessments, reassessments, referrals and follow-up in the client's clinical record, providing appropriate signatures and dating of such entries, including those made in electronic records.
