Assists in case management/discharge planning, with appropriate referrals based on client needs. The CSAC assures that an Individualized Treatment Plan is established for each participant and that a Treatment Plan Review is completed weekly for those who remain incarcerated for greater than 16 weeks. Daily tasks consist of attending and leading group sessions and individual sessions.
The CSAC is a strategic member of the Treatment Team and makes decisions and recommendations in the area of social services required placement in other social structures and counseling with recommendations for other wraparound services, including re-entry.
Work is performed in accordance with established departmental policies with application of professional knowledge and skills in completion of case assignments, working with a degree of professional independence. The counselor will work closely with a multidisciplinary team consisting of jail administrators and staff, medical providers, program staff (counselors, re-entry specialists), and community partners such as CSB and similar agencies. Performance of duties are under the supervision of the Executive Director of Community Outreach and Programs.
Essential Functions
Essential functions include, but are not limited to:
Demonstrate the ability to engage individuals in a hopeful, empathetic manner regardless of the phase of recovery.
Provide a welcoming environment while conducting daily classes for individuals enrolled in substanceabuse treatment and counseling.

The York-Poquoson Department of Social Services is hiring a Family Services Specialist III the child welfare team. Title Description- The Family Services Specialist III with the Child Protective Services unit, is a professional position under the supervision of a Family Services Supervisor that performs in program areas and assumes more complex duties such as: interviewing/investigating in cases involving suspected child abuse, neglect, out-of-home placements, guardianships, emergency removal and protective orders, testifying in legal proceedings; and placing children in foster/adoptive homes. The EHS Manager will be a key player supporting the implementation and administration of the ITAC EHS Program. As a QMHP you will provide intensive in-home counseling services in a one-to-one, family and group setting for youth and their families with sessions being conducted in the family home. Intensive In-Home Counseling services place an emphasis on offering the individual behavioral, social and/or recreational skills coaching as well as parenting skills training. You will provide 24 hour crisis intervention support, implement positive behavior support plans, and coordinate care with other agency's involved in your client's care. The ideal candidate is compassionate, works well within a team environment, and has strong communication and interpersonal skills.
Responsibilities
Provide services in the homes and communities of clients in various Hampton Roads cities in the state of Virginia
Document all service delivery as required by the Department of Behavioral Health and Developmental Services

MENTAL HEALTH PROFESSIONAL MINIMUM REQUIREMENTS
Must be a Licensed, Master's Level Behavioral Health Clinician who can administer assessments, make clinical decisions, provide Mental Health treatment, and hold any of the following licenses: CMHC, LCPC, LPC, LCSW, LMFT, ACMHC, ALMFT, CSW or equivalent. The Behavioral Health Professional co-leads group therapy, one on one therapy, and crisis therapy.
MENTAL HEALTH PROFESSIONAL ESSENTIAL FUNCTIONS
Professionals will provide crisis stabilization, complete intakes and on-going evaluations, and provide consultation to in-house and community services.
Participate in monthly behavioral health calls, work collaborative with the medical team to best determine overall treatment for each individual.
The Behavioral Health Professional performs risk assessment of inmates; coordination of care with interdisciplinary team including medical staff, community providers, psychiatric hospitals and corrections; and discharge planning.
The Behavioral Health Professional assists in planning and implementing the goals and objectives of programs and projects.
The Behavioral Health Professional provides the necessary preparation of documentation, necessary records and reports.
The Behavioral Health Professional utilizes clinical skills such as prioritizing treatment needs; crisis management; brief clinical interventions including CBT and Motivational Interviewing.
Excellent and timely documentation skills required.
The ideal candidate is able to make independent clinical decisions, while utilizing assistance and supervisory intervention, as needed.

Graduation from a Master's level program in a behavioral health related field.
