Serve as a member of a Multi-Disciplinary Clinical Team.

Minimum CASAC credential as Advanced CASAC preferred/ Maintain CASAC credentials, as required by New York State.

Implements and maintains an effective Trainee Employee Assistance Program (TEAP), in compliance with the Department of Labor (DOL) and management directives, emphasizing substance abuse awareness, prevention, staff training, and networking with community resources.
Schedule:
We are seeking (1) Full-Time TEAP Specialist with a Full CASAC (Full-Time = 44 hrs./week for this position)
Duties and Responsibilities:
Makes assessments of all students to determine those who might require interventions due to substance use
Conduct individual and group counseling for students who may require interventions
Participate in the orientation of new students during the Career Preparation Period
Ensure all students who test positive for drugs are retested within the 45-day Probationary Period
Provide follow-up counseling to students who have completed TEAP to encourage and prevent relapses
Provide prevention education training to student employees during all phases of the Job Corps program
Conduct in-service training sessions with both staff and students in all areas related to substance use and abuse
Collaborate with the Recreational department in developing leisure time activities and incentives to reinforce an alcohol and drug-free lifestyle
Recommend medical separations with reinstatement for students who require alcohol and other drug treatments
Act as a liaison between center and agency resources and referral contacts

Skills & Competencies:

Demonstrated ability to assess student's need for inpatient/outpatient substance abuse treatment and, when appropriate, coordinate access to these services

This is a Full-Time position, working 40 hours per week (Monday-Friday) between the Montgomery County Jail and the Fulton County Correctional Facility.

Job Responsibilities:
Responsible for the clinical monitoring, coordination, and implementation of the behavioral health program in accordance with Company policies and procedures.

Duties Description The Addiction Counselor 2 will serve as a senior member of a multidisciplinary treatment team performing assessment, treatment planning, case management, and clinical services with a marked degree of independence and limited need for supervision. Purpose: Provide counseling and support to Veterans, caregivers, and Authorized Representatives in the Veteran Directed Care (VDC) Program, while connecting Medicaid-eligible individuals to essential health and social care services that address Health-Related Social Needs (HRSNs). Job Duties:
The successful candidate will be expected to:
Meet with Veteran to develop a comprehensive person-centered spending plan.
Maintain monthly contact with Veteran to monitor program satisfaction.
Monitor monthly spending reports to ensure spending does not exceed spending plan in VDC.
Serves as the primary contact for all matters associated with VDC program.
Communicate effectively with Veteran and/or Authorized Representative and provide counseling if not adhering to spending plan.
Serve as the first point of contact for referrals, conducting screenings and eligibility assessments using standardized tools (e.g., Unite US Platform) for Healthy Alliance Program (HAP).
Identify and address social needs such as housing, food security, transportation, and emotional well-being for HAP.
Facilitate access to appropriate community and health resources, ensuring timely outreach, referral, and follow-up. (HAP)
Maintain accurate documentation of all outreach, screenings, and referrals; report trends to support continuous improvement. (HAP)
Assess, identify, coordinate, and monitor supports and services on an ongoing basis.
Have a valid NYS driver license, have own/reliable transportation, be able to travel, and have a flexible schedule to accommodate Veteran's need.
Education:
A degree in Social Work, Human Services, Health, or a related field is preferred; relevant experience will be considered. At minimum, a High School Diploma or equivalent is required.
Knowledge, Skills & Abilities:
Must be proficient in Microsoft Office suite applications for preparation of documents, spreadsheets, and data management.
Sensitivity and awareness of issues related to Veterans.
Knowledge of human services, Independent Living philosophy and case management is preferred.
Strong organizational skills, strong verbal and written communication and time management skills.
Proficient in Microsoft Word and Excel, ability to work independently and with diverse populations.
