POSITION SUMMARY
This position independently performs prescribed diagnostic sleep monitoring studies, which may include home sleep tests, to patients. This position analyzes the results to aide clinicians with diagnosis, treatment, monitoring and care of patients. This position acts as a resource to the department.
CORE FUNCTIONS
1. Obtains artifact free sleep tracings that accurately reflect physiologic processes occurring during sleep of patients utilizing the latest technology.
2. Analyzes all types of sleep records of patients. Identifies and documents sleep stages and quantify physiologic processes during the data acquisition and summarizes this information as outlined by departmental policies and procedures.
3. Ensures an accurate and readable recording using deductive reasoning to trouble shoot minor problems and make adjustments as needed during the testing process.
4. Collects and/or reviews pertinent medical, historical, and medication information in order to perform the appropriate study prescribed by the physician as outlined by departmental policies and procedures.
5. Assists with the development of department policies, procedures and processes, facilitate testing and maintain quality to achieve optimal patient results.
6. Assists leadership with operations, scheduling, orientation and/or training of new technical staff. May conduct or lead educational in-services and case study presentations.
7. Ensures quality assurance standards as outlined by departmental policies and procedures in accordance with regulatory agencies.
8. Independently performs prescribed diagnostic sleep monitoring, which may include home sleep test, to patients and acts as a resource to the department. Internal customers: All levels of nursing management and staff, medical staff, and all other members of the interdisciplinary health care team. External Customers: regulatory and health agencies, patients and family members.
MINIMUM QUALIFICATIONS
High school diploma/GED or equivalent working knowledge. Basic Life Support (BLS) certification required.
Requires Registry by the Board of Polysomnographic Technologists or proof of having passed the National Board for Respiratory Care (NBRC) as a Registered Respiratory Therapist (RRT).
The position requires clinical knowledge typically achieved with 3 years of experience in all aspects of sleep disorders testing. Must be proficient in the use and theory of all equipment related to sleep testing and a thorough knowledge of the sleep/wake cycle and its pathologies, as well as, EEG theory appropriate to sleep monitoring and cardiopulmonary physiology during sleep. Must demonstrate critical thinking skills, problem-solving abilities and effective communication skills.

**PRINCIPAL** **DUTIES** **AND** **RESPONSIBILITIES:**

Education:
+ Assist other health care members in providing patient specific detailed education regarding adequacy measures where appropriate - Online Clearance Monitoring (OLC), Adequacy Monitoring Program (AMP), Urea Kinetic Modeling (UKM), and regarding disease process/access.
Treatment:
+ Welcome assigned patients and inquire as to their wellbeing since their last treatment.
+ Evaluate vascular access for patency, perform vascular access cannulation, and perform administration of heparin as delegated or as allowed by state law.
+ Obtain necessary pre and post treatment vital signs and weight and perform vascular access evaluation pre- treatment
+ Initiate dialysis treatment according to prescribed orders including blood flow (QB) and dialysate flow (QD).
+ Evaluate intradialytic problems and provide intervention as prescribed by physician order or as directed.
+ Monitor patients' response to dialysis therapy.
+ Obtain vital signs prior to reinfusion; perform all relevant functions necessary for the discontinuation of treatment - document.
+ Discontinue dialysis treatment according to established procedures and evaluate patient prior to termination of venous access - standing & sitting blood pressure
+ Obtain Hemostasis and apply appropriate dressings.
+ Evaluate the patient prior to discharge.
+ Perform and record Pre and Post dialysis evaluation, weight, and vital signs with initial identification
+ Responsible for calculating and entry of individual patients' dialysis machine programming for Ultrafiltration (UF) goal; treatment time; and UF modeling as prescribed.
+ Responsible for ensuring appropriate safety alarms are enabled, including Narrow Venous Limits, (NVL) 160.
+ Record accurate and timely information regarding vital signs and treatment parameters onto the Hemodialysis Treatment Sheet with initial identification
+ Responsible for accurate documentation of information related to patient treatment including completion of Hemodialysis Treatment Sheet and, if applicable, progress notes.
+ Report any significant information and/or change in patient condition directly to the registered nurse or supervisor.
+ Observe patient and conduct machine safety checks according to facility policy. Report any change or unusual findings to the registered nurse or supervisor.
+ Perform and document any intervention for unusual patient status and document patients' response to intervention.
+ Evaluate patients' vascular access during treatment including arterial and venous monitoring pressures, provide appropriate intervention as needed, document and report any unusual findings to the registered nurse or supervisor.
+ Obtain all prescribed laboratory testing and prepare specimens for collection.
+ Ensure that all blood spills are immediately cleaned with appropriate disinfectant according to facility policy.
+ Prepare, organize, and efficiently utilize supplies and equipment to prevent wastage.

**Staff Related**
+ Assists in training of applicable direct patient care staff on appropriate programs under the supervision of a nurse.
Education/Communication:

+ Assists in assuring patient understanding or home dialysis products, benefits of home and how home dialysis can work for them.
+ Sets and preforms a treatment on all machines used in the transitional care unit.
+ Assist in assuring patient understanding of treatment options including demonstration of the peritoneal dialysis set-up.
+ Assists with coordination of transition to patient's modality choice.

The essential functions of the SurgicalTechnician include, but are not limited to the following:
Prepares room in a timely manner with all anticipated instruments, sutures, supplies and equipment for each individual surgical procedure.
Demonstrates competence in care of specimens and accurately handles specimens at all times.
Assists in positioning, prepping and draping of patients for surgical procedures.
Sets up, scrubs and prepares procedures at least 30 minutes prior to scheduled time, unless otherwise directed by Circulating RN or Team Leader.
Performs services requiring technical or manual skills under the direction of Circulation RN.
Always maintains and orderly and sterile instrument table for the purposes of efficiency; follows department standard set up.
Accurately completes operating room counts (instruments, sponges, needles); follows proper procedure in the event of incorrect count; reports all discrepancies to Circulation RN and surgeon and/ or corrects immediately.
Carries out medical/ surgical asepsis during treatment/ procedures.
Follows as aseptic technique, when opening sterile supplies and setting up procedures.
Maintains safe environment for patient, co-workers and self, per ASC policies and procedures.
Checks all equipment/ instruments needed, prior to each procedure to ensure proper functioning.
Protects patients sensitivities and right to privacy; closely observes patient with minimal disturbance.
Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/ health care errors to include proper patient identification and surgical site verification.
Anticipates needs of surgeon during intraoperative phase.
Cleans and prepares rooms between procedures and after last case; restocks supplies, when necessary and organizes effectively for end of case clean up.
Is fully responsible for care of instruments/ equipment during intra/postoperative phase, including cleaning and processing of instruments.
Follows written procedures for steam sterilization; knows responsibilities and the necessity for chemical and biological monitoring.
Consults with team when in question or unfamiliar with equipment or procedure.
Regularly collaborates with other member of the nursing team to maintain equipment and supplies for surgical procedures.
Interacts professionally with patient and family.
Provides patient with explanations and verbal reassurances, as necessary.
Notifies appropriate licensed professional when patient complains of pain.
Adapts procedures to meet individual needs of adult patients for treatment.
Treats patients and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic, and religious/ spiritual needs of patients and their families.
Able to interpret data about the patients status to identify patients age specific needs and provide care needed by appropriate age demographic patient group.
Establishes and maintains effective communication and good working relationship with coworkers for the patient benefit.
Makes decisions reflecting knowledge of facts and good judgment within the operating room technicians scope of practice.
Makes decisions reflecting knowledge of assigned case and instruments and/ or trays to be pulled for the procedure.
Qualifications

**PRINCIPAL** **DUTIES** **AND** **RESPONSIBILITIES:**

Education:
+ Assist other health care members in providing patient specific detailed education regarding adequacy measures where appropriate - Online Clearance Monitoring (OLC), Adequacy Monitoring Program (AMP), Urea Kinetic Modeling (UKM), and regarding disease process/access.

+ Record accurate and timely information regarding vital signs and treatment parameters onto the Hemodialysis Treatment Sheet with initial identification
+ Responsible for accurate documentation of information related to patient treatment including completion of Hemodialysis Treatment Sheet and, if applicable, progress notes.
+ Report any significant information and/or change in patient condition directly to the registered nurse or supervisor.
