Initiates and maintains written (including clinical and progress notes) and verbal communications with team members and management to assure optimal patient/client care. Provides therapeutic care in accordance with VNS Health policies, practices, procedures, and Standards of Rehabilitative care, which may require standing, stooping, sitting, crouching, bending, and stretching to deliver patient/client care. Transports and utilizes required medical equipment and other supplies using VNS Health designated/supplied carrying case weighing approximately 25 - 30 lbs. to and from patient/client homes/care facilities. Follows work procedures in relation to established workflows. Maintains productivity sufficient to meet VNS Health goals. Travels to patient/clients' homes and/or other facilities with varying environments (e.g., elevated buildings, walk-ups, care facilities, single/multiple family homes, presence of pets, etc.) using approved transportation options to deliver direct care to the patient/client. Assumes responsibility for personal professional growth through attendance at in-services, training, continued formal education and select journal readership. Complete assessment and evaluation of patient's impairments and deficits. Educates patient and caregivers on recommendations for continues therapy services, equipment for home use, and home exercise. 7. Completes documentation accurately and in a timely manner for evaluation, discharges, daily notes and re-evaluations according to department and state policies. 8. Assures continuation of therapeutic plan following discharge by designing home exercise programs. 9. Identify and report all real or potential problems regarding the development or administration of patient care. 10. Records, reports, and interprets patient responses to treatment and care to appropriate members of the inter-disciplinary team. 11. Demonstrate team approach to problem solving through frequent communication and collaboration on complex issues to provide appropriate care. 12. Ensures operation of equipment by using equipment consistent with equipment specifications; completing preventive maintenance requirements; following manufacturer's instructions; troubleshooting malfunctions; and calling for repairs. 13. Participates and represents Harborage and PT staff on committees, as requested by supervisors. 14. Demonstrates appropriate knowledge in assessing the need for durable medical equipment for patients. Follows through with appropriate professionals to ensure proper fitting and educates patient on proper usage. 15. Demonstrates appropriate judgment in ordering and measuring for prosthetic/orthotic device. Assesses the fit and function for the device and the need for adjustments. 16. Maintains professional competence and keeps current with Physical Therapy trends through participation in continuing education, in-service programs, professional staff sessions, reviewing professional publications; establishing personal networks; and participating in professional societies. 17. Complies with federal, state, and local legal and professional requirements by studying existing and new legislation; anticipating future legislation; enforcing adherence to requirements; advising management on needed actions. 18. Fosters good relationships and communications with all Team Members, physicians, patients, family/significant others and visitors. 19. Assists with new Team Member orientation and provide key experiences to prepare Team Member for independent performance of essential job functions. POSITION OVERVIEW: CFTSS Clinical services are provided to children/youth and are intended to help identify and prevent the progression of behavioral health needs through early identification and intervention. Services may be provided to children/youth in need of assessment for whom behavioral conditions have not yet been diagnosed or to youth with a diagnosis for whom flexible community-based treatment is needed to correct or ameliorate a developmental delay or problem in functioning or capacity for healthy relationships. The clinician conducts a comprehensive assessment to determine if children/youth are eligible for CFTSS, makes recommendations regarding which CFTSS services are to be provided, and completes a treatment plan, which serves as a rehabilitative service package to support a child and family whose needs may be complex and/or require flexible nontraditional approaches.
