Be the Best Clinical Territory Patient Admissions Coordinator/LVN You Can Be
If you meet these qualifications, we want to meet you!
LVN License required
3-7 years of experience in facility/physician relationships with a deep understanding of facility discharge processes, preferred.
Minimum of one year experience in home care, hospice, or personal care services, preferred
Prior use of Electronic Medical Records (EMR), preferred
Experience in dealing with a variety of payors in healthcare, e.g., Medicare, Medicaid, and commercial payers, preferred.
Responsibilities:
Collaborates and maintains effective communication with the Referral Source, Sales teams, Clients and/or local agency operations to collect needed information to process a quality referral.
Obtains & Documents Verbal Orders for patient evaluation, as required in state in which the Territory Patient Admission Coordinator is licensed.
Documents all referral related Information, actions taken, and follow-up required in the medical record in a timely and accurate manner. As applicable utilizes the agency's software solution to facilitate the referral process by completing required workflow task within the Company's established time frames.
Ensures timely attention to referrals received from a variety of platforms, e.g., eFax, telephone, or referral portal/Hospital EMR portal.
Where applicable, reviews Eligibility Alerts and Responses for Medicare Clients.
Requests, H&P, Discharge Summary, and other documents needed for ICD 10 coding by the CPOCC team, if not received with the referral information.

Receives requests for home care services and schedules/reschedules Care Partners as necessary. Gathers all necessary case information from caller. Checks Care Partner availability in ContinuLink
Schedule/reschedules Care Partners based on case requirements and availability. Confirms and verifies schedules with clients and Care Partners and verifies that all visits are made.
Calls client to confirm care schedule. Confirms Care Partner availability and understanding of the case. Communicates results to office staff, if necessary
Responsible for entering and maintaining schedules in ContinueLink system.
Confirms/updates Care Partner availability/history daily
Maintains all client information/changes daily. Performs regular information audits to verify accuracy
