Provides counseling and coaching to individuals and families to help them understand and improve credit, establish a budget, reduce debt and create an action plan that will help existing and potential homebuyers meet the requirements necessary for stable and sustainable homeownership.
Counsels and provides coaching to older adults and their families/caregivers to help them understand their housing options; creates action plans that will help older adults transition to safe, affordable and sustainable homeownership.
Mediates workout options between homeowners, the servicer or lender of the mortgage, and real estate professionals to help homeowners keep their homes, execute a deed in lieu of foreclosure or enable clients to short sell their homes.
Facilitates and presents virtual and in-person homeownership workshops.
Provides regular and timely updates to clients on program requirements and goal progress.
Participates on existing and regularly scheduled internal and external housing collaborations and committees;
Inputs client data points into Client Resource Management (CRM) systems; Creates and provides monthly reports on client outputs and outcomes to the Homeownership Manager.
Speaks and presents at community outreach events, seminars, and meetings to promote Benjamin Rose housing programs and services.
Maintains all necessary certifications and program training requirements.
Assists Homeownership Manager on special projects, data reporting and process and program implementation.

They assist to carry out Care Solutions Center operational strategies by conducting needs assessments, performance reviews, capacity planning, and cost-benefit analysis: defining user requirements, establishing productivity, quality, customer service standards, contributing information and analysis to organizational strategic plans and reviews.
Builds relationships with staff in hospitals they serve, being a trusted partner.
Trained and qualified nurse who can help guide teams to provide support for families and patients with empathy and confidence
They implement the Care Solutions systems by assisting the Director, and Chief Clinical Officer with patients, families, referral providers, doctors, and Hospice of the Western Reserve field staff.
Collaborate with trainers to implement a comprehensive training program for staff in the field.
Determines training needs by observing agent and service encounters, studying all phases of the Care Solutions services, technical, service and customer experience results reports, conferring with management.

You are relied upon to take care of patients and families who need help after hours in typically urgent times. WHEN DO ON-CALL NURSES MAKE A DIFFERENCE?
Your schedule is typically 8p - 8:30a, with an expectation to work every other weekend and every other holiday. This position includes a $3,000 sign-on bonus for FT.
WHAT YOU WILL DO:
Assess physical, psychosocial and spiritual needs of the patient and family (RN only).
Gather data related to patient and family's physical, psychosocial and spiritual needs (LPN).
Provide intermittent skilled nursing services to the patient under the patient's physician's direction.
Monitor and evaluate pain and symptoms; provide information and assistance to the patient and family to manage those problems.
Serve as a liaison to the patient's physician(s) when needed; provide information to the physician based on knowledge relating to the patient's needs.
Provide emotional support to the patient and family.
Refer patients and/or family members to other services when appropriate.
Provide instruction (health care teaching) to the patient and family as related to care in a residence.
Make visit to patient's residence at the time of death.
Maintain, promptly complete, and return all documentation as required.
Communicates and acts as liaison with patients, family, and health professionals in a manner that respects confidentiality and dignity.
Relay pertinent information to appropriate team members.
Participate in meetings as initiated by team leader.
Participates in mandatory or elective HWR University events, trainings, or in-services as necessary.
RN's shall participate in triage coverage as needed
Perform other duties as assigned.
SUCCESS CAPABILITIES:
High School Diploma or equivalent required.
Graduate of an accredited school of nursing, with a Bachelors degree preferred.
Licensed RN/LPN in state of Ohio.
Experience preferred in pain and symptom management, oncology, or mental health.
Ability to communicate effectively, orally and in writing
Ability to work independently and collaboratively with a team
Must have demonstrated knowledge and skills to provide appropriate care to all age groups.
Ability to handle stressful and constantly changing conditions; must be able to make good professional decisions quickly despite stressful atmosphere.
Ability to access patients and families in any residential and/or public setting in any weather condition (i.e., climbing stairs or snow filled walkways, etc.).
Proficiency in Microsoft Office suite of products, particularly proficient in Word and Excel, use of shared team files, a donor database, grant portals and virtual meeting technology
Must have and maintain a valid driver's license, good driving record and auto accident insurance at a level acceptable to HWR and/or HWR's insurer.
PHYSICAL REQUIREMENTS
Ability to stand and walk for extensive periods of time.
Manual and finger dexterity and eye-hand coordination to provide nursing care and operating equipment.
This is heavy work requiring the exertion of up to 75 lbs. of force occasionally as well as 50 lbs. frequently utilizing appropriate safe transfer techniques and body mechanics when working with patients.
Ability to stoop, squat, or bend frequently.
Corrected vision and hearing within normal range to observe and communicate with patients and professional staff.
Possibility of exposure to environmental elements, internal and external, such as disease, bloodborne pathogens, and chemicals and/or chemical fumes, (pet dander and/or exposure to allergens) and adverse effects of inclement weather that may occur as well as potentially difficult patient and family situations.

Experience preferred in pain and symptom management, oncology, public health, mental health
Experience in pediatrics and knowledge regarding normal growth and development, pediatric assessment skills, and pharmacology
Must work a minimum of 40 hours, four days/week
May at times work 4 days, 10-hour shifts based on department needs and supervisor approval
1 evening per week for call coverage and 1 weekend worked per month.
Must have valid driver's license, car available, and insurance coverage as required in Ohio
Working coverage area may include: Lorain, Cuyahoga, Lake, Ashtabula, Medina, Summit, Geauga, and Portage counties
All attempts will be made to place PED Nurse in the service areas closer to living residence

Provide personal care under the supervision of a nurse according to the patient's plan of care. Collaborate with transdisciplinary team in a variety of settings. This position has a $1,000 sign-on bonus!
WHAT YOU WILL DO:
Provide personal care and assist with ADL's according to the POC.
Provide safe environment for patients.
Safely ambulate/transfer patient as appropriate.
Assist with passive and active ROM exercises as directed by the nurse.
Prepare nutritional snacks as appropriate for the patient.
Under the supervision of the nurse, assist patient with pre-poured medications that are ordinarily self-administered and that are prescribed by a physician under the supervision nurse (Home Care only).
Under the direction of the nurse, perform simple wound care, skin care, and bowel care protocols according to the plan of care where appropriate.
Recognize and promptly report any changes in the mental or physical status of the patient to the nurse.
Record in clinical record, on appropriate documents, the activities provided and observations of patient at the time of visit.
Complete all charting, timesheets and mileage logs as defined by the agency policy and procedures.
Attend and actively contribute during patient care conference.
Participate in HNA discipline meetings, team meetings, staff meetings, retreats and in-services as appropriate.
Attend in-services to obtain 3 education hours per quarter.
Participate in the on-call rotation as scheduled (Home Care only).
Assist the Team Leader with other designated activities as necessary (scheduling, supply organization, answering office telephone, etc.)
Demonstrate positive customer-service responsiveness according to agency standards.
Flexible in meeting patient/family needs that require variable shifts and weekends.
Participate in chart review quality improvement activities and other meetings as assigned.
Participate in coordination of and communication with volunteer staff.
Provide basic care-giving education to caregiver/family members, such as turning and positioning, transfer techniques and changing linens
Maintain Transmission Based Precaution.
Complete annual competency requirements.
SUCCESS CAPABILITIES:
Must be able to read and write English proficiently.
Must have 12 months experience as a nursing assistant.
Must be state tested nursing assistant or certified nursing assistant in good standing
Equivalent education/experience may substitute for minimum qualifications except when there are legal requirements, such as a license/certification/registration.

The Clinical Admissions Nurse is responsible for assuring for assuring that HWR programs are effectively and responsibly represented to patients, families, and others who have interest in hospice services through assessment, information sharing and/or admissions in a patient's home, nursing facility, or in a health care facility. This position works closely with the Referral Department and other referral sources to promote the hospice philosophy of care.
WHAT YOU WILL DO:
Performs all duties and responsibilities in accordance with the Nurse Practice Act and in accordance with basic principles and guidelines of professional nursing.
Provides information assessment, and admission services to patients who have been referred to HWR: conducting assessment visits in the home nursing facility or health care facility, as necessary.
Communicates with Referral Department, the referring Physician, and Hospice MDs to assure appropriateness of each referral and those necessary certifications are obtained.
Keeps the Referral Department informed about the progress of the referral/admission.
Applies nursing principles and techniques in the treatment of patients. Provides nursing care, emotional support and education to patients and families.
Utilizes computer skills to access medical information and documents in electronic medical records as required.
Promotes teamwork by participating actively and appropriately as a member of an interdisciplinary team, shares essential patient information with all members to coordinate care.
Completes and submits documentation within agency timelines policies that is complete, legible, and clearly reflects care given.
Utilizes appropriate resources to maintain knowledge and skills for the hospice care in all settings including regulatory/ reimbursement guidelines.
Collaborates with other members of the IDT in coordinating care to be efficient and financially responsible.
Makes appropriate referrals to other discipline services and medical equipment/ supply companies to meet patient care needs in a timely manner.
Works with diverse social and economic situations without imposing own values on patients or family, maintaining appropriate professional boundaries.
Meets standards for continuing education and maintain licensure.
Demonstrates understanding of HIPPA privacy Standards and related Agency policies.
SUCCESS CAPABILITIES:
Graduate of an accredited nursing program: BSN and/ or CHPN Experience with volunteer management and HR practices preferred
Required Nurse Licensure by the state of Ohio, valid driver's license with an automobile that is insured in accordance with the state and/or agency requirements and is in good working order.
