FLOAT HOME HEALTH PROVIDER (PAS ATTENDANT)
Responsible for providing Personal Assistance Services to participants in their place of residence; examples include, but are not limited to, assistance with removing and putting on clothing, bathing, eating, using the restroom, transferring, light housecleaning, and meal preparation. Personal Assistance Services do not include performing medical procedures (e.g., administering shots) or medical monitoring (e.g., takes blood pressure). Follows established individualized service plans for participants under the instruction and supervision of the registered nurse. Assists family members in maximizing participant comfort through the provision of effective home health care and observes and reports any changes in the participant's status to the assigned supervisor.
The Float PAS Attendant is not assigned to one location. Instead, the Float PAS Attendant will be assigned work in a location or department, to fill vacancies created by leaves of absence, vacation, vacant positions, or anyone who misses work.
RESPONSIBILITIES:
Assumes responsibility for providing effective personal assistance services.
Effectively follows company policy and procedures.
Assumes responsibility for knowledge and demonstration of proper safety techniques.
Assumes responsibility for establishing and maintaining effective working relationships with participants, family members, staff members, supervisors, and management.

APPOINTMENT SCHEDULER I
Under the direct supervision of the Dispatch Supervisor, the Appointment Scheduler I is responsible for the complex daily scheduling of diagnostic and subspecialty appointments in a manner that is fully integrated with day center operations.
RESPONSIBILITIES:
1. Schedules all routine and non-routine appointments, diagnostic, imaging, procedures, and surgeries consistent with the participants' diagnosis and procedure request/physician's order, appropriate provider, time, and location utilizing automated scheduling system and enters notes/special instructions needed for scheduling in the electronic medical record.
2. Provides accurate, detailed information to the clinic staff regarding test preparations and any other directional information needs.
3. Communicates with clinic the need for prior authorization as needed. Obtains proper medical clearance and pre-surgical testing as needed.
4. Ensures that appointments are scheduled based on urgency, and that all physician orders are scheduled prior to the due date. Ensures appointments are set up two weeks in advance and participates in weekly meetings with designated staff to discuss and coordinate transportation and escorts for upcoming scheduled appointments.
5. Maintains current automated scheduling system(s) to create participant appointment matrix, list of unscheduled orders, scheduled appointments, and completed appointments.
6. Responsible for participating in weekly meetings with designated staff to discuss and coordinate transportation and escorts for the following week's appointments.
7. Responsible for double checking the physician order to ensure the Specialist's name and the specialty is the correct one.
8. Adheres to regulations regarding participant confidential information and HIPAA.
9. Maintains constant communication with the IDT related to appointment
10. Ensures that accurate & timely entries are done daily using appropriate automated systems.
11. Prepares a variety of periodic and special reports that may require gathering of information from different sources, compiling data and arranging it into a proper format.
12. Provides coverage, as necessary, for other members of the department due to vacancies or absences to avoid backlogs and delays.
13. Completes projects and perform other duties as assigned by immediate supervisor, department manager or division chief.
QUALIFICATIONS / REQUIREMENTS:
1. High School Diploma or equivalent.
2. Associate Degree and/or certificate program preferred.

DRIVER - PRN
Under the supervision of the Transportation Supervisor, the Driver is to safely transport and physically assist Bienvivir/PACE participants to and from the center to home and to medical appointments as dictated by the Interdisciplinary Team (IDT)
RESPONSIBILITIES:
1. Safely transport participants and provide door to door service to/and from the center, home, and medical appointments.
2. Sign-in/register participants at Specialist Office for medical appointments.
3. Physically assist and escort frail participants from their residence to the van and back.
4. Deliver medications and medical supplies to the participant's home and/or other facilities as required.
5. Driver will verify the name of the participant, the address and the center of attendance prior to delivery of any medication to participants.
6. Driver will request a signature from the person receiving the medication.
7. Driver will return undelivered medication to the pharmacy and will request a signature from the person receiving the medication.
8. Ensure that all straps are firmly and securely locked down when participants in wheelchairs are being transported. There should not be any slack in straps.
9. Drivers are required to carry oxygen tanks in the vehicle and set up a participant with a tank or exchange the empty tank with a full one, then set the liter flow on the regulator.
10. Take appropriate actions (contact clinic staff) in the event of an incident/accident with a participant or vehicle.
11. Provide the Transportation Supervisor with daily delivery logs. Observe and report participants' changes, or participants who refuse to attend the center or are not ready.
12.To ensure participants have all their required Durable Medical Equipment (DME) when boarding/off boarding the vehicle.
13. Reads Mobile Data Terminal (MDT) notes to see how the participant should board/off board the van, - via main door or via lift.
14. Conducts daily vehicle inspections to ensure assigned vehicle is safe to operate.
15. Carry and maintain assigned cellular phone ON at all times during working hours or as required. Cellular phone will not be used while driving. All calls must be answered as promptly as possible without compromising safety.
16. Must work outside normal working hours in case of a medical emergency and/or late scheduled appointments to include medication deliveries.
17. Provide on-call coverage after hours, on weekends and holidays as assigned to provide transportation to participants, deliver medications, medical supplies, etc…
18. Performs other duties as assigned by immediate supervisor, department manager or division chief.
QUALIFICATIONS / REQUIREMENTS:

2. Must be at least twenty-one (21) years of age with five years' experience of continuous driving experience.

DAYCARE ESCORT
The Daycare Escort is responsible for accompanying participants to medical specialty appointments, overseeing the safety of the participant during appointments, and for assisting with ADLs as needed while at the appointment. When not assigned to Escort duty, the position will assist in the center adult day care as assigned by the Floor Manager.
RESPONSIBILITIES:
1. Be available to attend scheduled appointments or other unscheduled (STAT) appointments designated by the Floor Manager.
2. Provide personal care needed by participants attending appointments (transfers, toileting, ambulating, etc.).
3. Communicate all necessary appointment-related information to the Floor Manager and/or Clinic Participant Manager involved with the coordination of participant care.
4. Maintain a folder that includes a personal list of the employee's weekly participantscheduled appointments that may be modified as required.
5. Clock in/out on time at assigned centers. Will immediately report any time-keeping issues to Floor Manager.
6. Ensure participants have their snack/lunches identified by the Floor Manager and verified on the Appointment Matrix no later than the day of the appointment.
7. Attend monthly Center Staff Meetings, annual Skills Fair and other mandatory meetings/trainings as scheduled.
8. Maintain all necessary equipment and supplies (gloves, gait belt, diapers, CPR mask, appointment paperwork, etc.) needed to carry out appointment Escort duties.
9. Report to Floor Manager daily for assignments and drive from center to the participant's appointment(s) in own vehicle.
10. Contact Clinic Participant Manager (at the center where the participant attends) and Floor Manager and if necessary activate EMS, regarding participant emergencies encountered during appointments.
11. When not assigned to Escort duties, will assist center floor operations, within their training and scope, as assigned by Floor Manager. Examples of duties may include, but not limited to, ambulating participants requiring assistance, supply inventory, assisting participants with ADLs, providing assistance to Recreation Department, handing out food trays, etc.
12. Other duties as assigned by supervisor.
QUALIFICATIONS / REQUIREMENTS:
1. Must be at least 18 years of age or older.

3. Certified Nursing Assistant in the State of Texas preferred.

MEDICAL RECORDS REVIEWER
The Medical Records Reviewer works under the general supervision of the Medical Records (MR) Supervisor, is responsible for complying with and enforcing organization and department policies and procedures, the National PACE Association (NPA) standards, Center for Medicare & Medicaid Services (CMS) regulations, HIPAA requirements, and Electronic Medical Record (EMR) guidelines. Responsible for data entry and audits to ensure medical records are accurately documented, securely stored, and accessible to authorized users in compliance with healthcare regulations. Duties include scanning and organizing documents into the EMR, properly categorizing and labeling records, processing record requests, preparing and releasing medical records to nursing home facilities, and maintaining confidentiality and security. Additional duties include covering MR staff responsibilities, assisting with training, and performing administrative, clerical, and project-based tasks.
RESPONSIBILITIES:
1. Responsible for data entry into the EMR. Effectively scans, uploads, and audits all relevant paper and electronic documents into the EMR.
a. Audits documents for accuracy, completeness, and relevance; perform data entry in the EMR as required.
b. Follows EMR and scanning program guidelines in accordance with Compliance and MR policies and procedures.
c. Reviews and prepares paper documents for scanning and/or uploading. Review electronic documents to ensure completeness, accuracy, and correct categorization before uploading them to the EMR. Performs audits as needed.
d. Identifies and resolves discrepancies by coordinating with appropriate MR staff before uploading documents to the EMR.
e. Serves as backup for uploading electronic progress notes to the EMR.
f. Assists in auditing electronic progress notes uploads.
Manages the electronic lab process by processing results from contracted laboratories for all centers and promptly entering lab data into the EMR. Maintains tracking logs and ensures timely documentation.
2. Responsible for collecting information related to participants' inpatient stays, emergency room visits, and completes the Service Utilization Report (SUR).
a. Collects and tracks hospitalization data from interdepartmental meeting minutes.
b. Audits, reviews, and processes hospital medical records, ensuring accuracy, completeness, and relevance before scanning and/or uploading them to the EMR.
c. Maintains updated logs to reflect any changes affecting data entry or reporting.
d. Prepares and submits weekly hospitalization data to the MR Coordinators for entry into the EMR, providing the information required for SUR processing.
e. Collaborates with MR Coordinators to resolve discrepancies in received medical records when information does not match what was reported.
3. Responsible for processing medical records for Visitors.
a. Completes the entry of both electronic and paper Visitors documents into the EMR. Processes and pre-audits visitor medical records for storage in the network file share and upload them into the EMR.
b. Maintains an up-to-date visitor tracking log.
c. Ensures all visitor records are saved in the network file share and accurately labeled and categorized in their appropriate folders.
d. Maintains and organizes the visitors purge folder in accordance with record retention policies.
e. Contacts external facilities to cancel medical record requests based on visitor disposition and complete the record disposition process as required.
4. Responsible for initiating, verifying, and tracking medical record requests for participants' hospitalization stays, emergency room visits, and visitors' previous medical history in compliance with departmental and regulatory standards.
a. Contacts the Billing Departments of external facilities to verify admission and discharge dates, emergency room visits, and facility names.
b. Initiates medical record requests by preparing and faxing the request form, tracks, follows up on pending records, and keeps the ER/Hospital tracking log up to date.
c. Contacts external facilities to verify and request visitor medical records, using the information provided in the visitor's authorization for release of information form.
d. Tracks and follows up weekly with external facilities on pending record releases to obtain visitor records prior to their scheduled appointments with Bienvivir Primary Care Providers. Maintains an up-to-date visitor tracking log.
5. Responsible for preparing and auditing medical records to be sent to Nursing Home Facilities upon admission and monthly thereafter for the duration of the participant's stay at the facility in accordance with MR standard operating procedures and Interdisciplinary team (IDT) policy.
a. Retrieves records from the EMR, prepares and audits medical record packets for participants placement admissions, respite stays, and monthly medical records updates to be sent to nursing home facilities and maintains records of the packets sent as a backup documentation.
6. Provides backup support and coverage for MR staff during absences, including the following responsibilities:
a. MR Reviewer: Performs all duties associated with this role as needed.
b. MR Progress Notes Specialist: Completes electronic progress note processes and manages the mail-out process for nursing home facilities.
c. MR Coordinator: Attends daily morning meetings and documents meeting minutes and center updates.
7. Meetings:
a. Attends the organization and department staff meetings, training, and in-services (for missed meetings staff are responsible for checking with the MR Supervisor to obtain information).
8. Additional Duties as Assigned:
a. Prepares and submits monthly labels requests to ensure adequate inventory.
b. Must actively participate in assigned cross-training within the MR Department.
c. Performs various projects and clerical duties as assigned.
QUALIFICATIONS / REQUIREMENTS:
High school diploma or GED equivalent required.
Certified from an accredited technical/trade school in the medical field preferred.
Two (2) years of work experience in the medical field preferred.
Experience with electronic health records programs, scanning procedures, and familiarity with HIPAA, CMS, TMHP and other regulatory requirements preferred.

APPOINTMENT SCHEDULER II
Under the direct supervision of the Dispatch Supervisor, the Appointment Scheduler II is responsible for calling Specialists' offices and confirming appointment information, to include date, time, and address. The Appointment Scheduler II reschedules and reminds participants and/or family members of upcoming appointments.
RESPONSIBILITIES:
1. Calls Specialists' office two days prior to the participant's appointment to confirm appointment date, time, and location. Consecutively, the scheduler reminds participants/family members of appointment.
2. Reschedules all routine and non-routine appointments and procedures, e.g., diagnostic, imaging surgeries, follow-ups, etc. consistent with the participants' diagnosis and procedure request/physician's order, based on, urgency, provider, time/location while utilizing automated scheduling system.
3. Enters appointment notes/special instructions requested by the specialist in the electronic medical record.
4. Provides accurate, detailed information to the clinic staff regarding test preparations and any other directional information needs as requested by the specialist.
5. Responsible for participating in weekly meetings with designated staff to discuss and coordinate transportation and escorts for the following week's appointments.
6. Communicates daily with specialists' offices to inform them of any late and/or cancelled appointment(s) and enters information in an automated scheduling system to create participant appointment matrix.
7. Responsible for preparing and sending all necessary paperwork to specialist office, e.g., Protected Health Information (PHI), such as X-Ray films, CDs, Medication List, Biographical Information, etc.
8. Maintains current automated scheduling system(s) to create participant appointment matrix of rescheduled appointments.
9. Maintains an established filing system, retrieves documents, and creates new files as required.
10. Adheres to regulations regarding participant confidential information and HIPAA.
11. Prepares a variety of periodic and special reports that may require gathering of information from different sources, compiling data and arranging it into a proper format.

Summary
