The Safe Haven PSB program is a specialized residential treatment facility treating
boys between the ages of 13 - 17 who exhibit problem sexual behaviors to the extent
that they require a staff secure treatment facility to address their sexual acting out.
II. POSITION SUMMARY:
The Case Manager provides service linkages, coordinates, follow-ups, and advocacy.
Case Manger utilize strength-based, family driven, youth guided, culturally responsive
principles in developing the care management plan that builds therapeutic
relationships with families.
II. ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:
Assist clinicians in managing up to 12 families.
Maintain bi-weekly telephone calls with families served at Safe Haven during treatment and after youth's discharged from treatment.
Meet with families in their home as needed to continue and reinforce treatment services.
Perform a Pre and a Post DCF- Family Need Assessment Form.
Utilize knowledge of existing community resources and educate families.
Prepare a packet and refer families to Natural Supports within their community.
Coordinate prosocial activities for the youth(s) one month prior to discharge and at discharge.
Assist with the coordination of Discharge Planning with the Safe Haven clinical team and other providers, such as DCF, CSSD, and Beacon.
Participate in (PSB) consultation calls on a weekly basis.
Help families to manage crises post discharge through the development and implementation of a proactive safety plan.
Coordinate with the LEA regarding the youths' educational needs prior to and after discharge.
Attend school-related meetings (PPTs, 504, IEP) with families, as needed providing agency advocacy and support for the family at those meetings.
Work in collaboration with DCF, Beacon Health Provider, and the Safe Haven clinical team to ensure collaborative care management.
Regularly attend staff meetings, clinical rounds, and LIST meetings
Provide documentation of treatment efforts that promote successful transition.
Perform additional duties as assigned to deliver seamless services throughout the agency.
Documents all services in adherence with state statures, regulatory, billing, COA standards and Boys & Girls Village Standards.
Job duties and or expectations may change contingent upon program needs and population being served.
III. ESSENTIAL QUALCIFICATIONS/SKILLS:
A college degree (BS, BA or BSW) in Human Services or related field.
Experience or comfort level working with families in their homes.
Valid driver's license, good driving record, and use of a reliable automobile.
Demonstrates culturally competent work history and training.

The Charles F. Hayden School is committed to serving children through a psycho-educational, clinically informed therapeutic milieu. Collaborative educational planning and placement teams including families and district personnel design Individualized Educational Programs (IEP) for every student.
POSITION SUMMARY:
The Paraeducator provides direct care, behavior management, and supports educational and clinical services for the clients in the Charles F. Hayden School. The Para transitions the shelter children at 3pm from the Charles F. Hayden School to the shelters and assist with the after school routine. During school vacations/holidays and other school closings or delays (except for when school staff development is scheduled) the paraeducator has the opportunity to work in the shelter if needed.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:
Provides required assistance and supports to facilitate client's behavior, emotional, educational, and social development and learning in accordance with an individualized service/treatment plan in a structured, intensive therapeutic milieu setting by working collaboratively with other members of the team
Participate in the planning and implementation of programming specifically designed to encourage development growth for a wide range of ages
Participate in weekly therapeutic groups as a co-facilitator, under guidance of clinical staff
Supports classroom activities and lessons with the educational staff
Provides crisis intervention utilizing Cornell Therapeutic Crisis Intervention techniques
Maintains appropriate ratios and motivation as indicated on point sheets
Provide a safe, orderly and nurturing living experience for children in care
Ensure that goals/objectives of each client's service/treatment plan are implemented as part of the daily activities
Documentation of each client's response to care and clinical interventions
Immediate notification to Shift Leads, Coordinator, or Manager regarding crisis situations, including elopements, major aggressions and sexual acting out.
When scheduled in the shelter, you are expected to work in inclement weather and may be mandated to stay beyond the end of the shift if coverage is required.
Be familiar with and implement all program policies and procedures
Complete Incident Reports as needed
Completes documentation in accordance with state, federal, and Boys & Girls Village policies
When 1:1 with a child must remain within arm's length of the client until relieved by other staff
Attends supervision and staff meetings weekly
Discuss observations of client behaviors with Shift Lead Daily, Coordinator or Manager as requested
Create and maintain a clean work environment
Adhere to the staffing schedule with the understanding that shift assignments are determined by program needs
Carries out all additional assignments as directed
ESSENTIAL QUALIFICATIONS/SKILLS:
3 years' experience working with children in a milieu based program.
Previous experience working with children in a recreational, educational, residential or day program
Valid driver's license and clean driving record
Specialized license (Endorsement A) to drive students in agency vehicles

Essential Duties & Responsibilities
Under general supervision the Tumor Registrar will;
Review medical records to identify and abstract cancer cases and related information.
Collect detailed data on patient demographics, tumor characteristics, staging, treatment, and outcomes.
Maintain an accurate, up-to-date cancer registry, ensuring comprehensive and timely data entry
Perform data validation and quality checks to ensure completeness and consistency of the cancer registry.
Monitor data trends and resolve discrepancies in the dataset through communication with medical staff or external organizations.
Ensure that all data complies with industry standards and regulations, such as the American College of Surgeons (ACoS) and the Commission on Cancer (CoC) guidelines
Prepare and submit cancer registry reports to state, national, and regulatory agencies as required (e.g., SEER, National Cancer Database).

Assist in preparing data for quality assurance, audits, and accreditation reviews.

Support research initiatives by providing tumor registry data for studies and clinical trials.
Assist in tracking patient outcomes and treatment patterns to guide cancer care planning.
Stay current with changes in tumor registry standards, coding systems (e.g., ICD-10, AJCC staging), and cancer care practices.
Participate in professional development and certification programs to maintain and enhance expertise in tumor registry operations.

Accountabilities:
The ideal candidate for the Specialty Nutrition Systems, Long-Term Feeding Account Consultant will utilize personal skills, product knowledge, and clinical expertise to maintain and build existing and new revenue within a set sales territory. The Specialty Nutrition Systems, Long-Term Feeding Account Consultant will be responsible for a continued demonstration of sales expertise of appropriate skills and functions that include but are not limited to product presentations, clinical presentations (including bedside coaching and case support functions), product pricing, sales territory management, internal and external communication, accurate sales forecasting/reporting. These activities are to result in net sales and profit growth by achieving product and sales category objectives within an assigned sales territory.
Key Responsibilities:
Sales positioning, analysis, and in-service implementation of all AVANOS Enteral Feeding product categories leading to year-on-year sales increases as per the AVANOS DH sales plan
The development and demonstration of business value selling skills and overall AVANOS sales acumen
Developing and maintaining expertise across the entire range of AVANOS EF products and service platforms
Tactical implementation of selling and business activities developed by region, National Sales Director, and VP of Sales, to meet sales objectives
Create and nurture high gain sales relationships with multiple decision makers and influencers within assigned accounts or markets (e.g., clinician, surgeons, physicians, department decision makers and/or administrators)
Development and enforcement of contract and price negotiations/contract management with desired outcomes of obtaining committed business at the highest margins possible
Active collaboration with sales and marketing leadership, SOC Specialist(s), NICU Specialist(s), Clinical Education Specialist(s), internal partners/stakeholders, and customers
Effective utilization of AVANOS EF sales and marketing resources and tools necessary to meet objectives

Follow compliance and credentialing requirements for uninterrupted access to customer facilities

The Senior Clinical Advisor (SCA), under the general supervision of the Nurse Manager collaborates to provide continuity of care by ensuring adequate resources are available on a shift to shift basis.
The SCA collaborates with the healthcare team to plan and coordinate timely admission, transfer and discharge of patients.
Creates an environment that ensures the delivery of safe and high quality patient care , high patient satisfaction, positive patient outcomes.

PAY RATE: $14.50 HRLY
Must have full valid Driver's License that you have held for 12 months and a reliable vehicle with proof of auto insurance and registration used to transport clients. We also have locations in Danville, Junction City, and Perryville.
HOURS: PART-TIME (up to 31hrs per week)
We do NOT offer traditional Monday-Friday 8 AM - 5 PM shifts. Work schedules are based on client needs and include mostly nights and weekends such as 12-hour shifts (8 AM - 8 PM/ 8 PM - 8 AM) and 16-hour shifts (4 PM - 8 AM).
REQUIREMENTS:
Must be 18 years or older.
