Collaborates with patients, families, physicians and the IDT for Diet Liberalization: Aims for long term care patients to have self-directed nutritional care by utilizing Regular diets, providing real foods first and personal choice. Works with physicians/physician extenders to communicate nutritional recommendations, weight changes and other areas as needed. Communicates nutrition issues to other disciplines and the facility administrator as they arise to effectively solve problems. Effectively informs resident and legal representatives of new orders, diet changes and weight loss notifications as warranted.
