Job Description
Provide quality incident investigations for all allegations within the agency. Ensures that appropriate recommendations are made to safeguard individuals and provide the highest quality of services. Provides education and training to all staff in the areas of abuse prevention, incident reporting, positive approaches and other classes as identified. Ensures compliance with all applicable standards.
Essential Job Functions:
Maintains current and thorough knowledge of incident reporting regulations (14NYCRR Part 624 and Part 625) as well as NYS Justice Center/OPWDD directives and guidance regarding reporting, investigation and monitoring of incidents.
Completes OPWDD 147 Form and Immediate Response Manager Report as assigned.
Completes thorough and quality investigations (and all required forms) as assigned within regulatory and agency guidelines and timeframes.
Monitors and tracks incident follow-up for assigned investigations ensuring all documentation and corrective actions are completed by required due date.
Uploads assigned incident investigations into IRMA
Maintains investigation electronic folders.
Meets with supervisor as scheduled to review status of incident investigations, follow-up and performance.
Informs supervisor of any significant investigation concerns as they arise.
Completes Notable Event investigations as assigned. Ensures all follow up and corrective actions are addressed.
Reviews Event (notable/internal) program investigations as assigned.
Completes investigation review tool for all assigned investigations prior to submission.
Completes monthly incident site review as assigned.
Provides training to employees in the areas of abuse prevention, incident reporting, and reporting of Notable/Internal Events, as assigned.
Performs other duties relevant to the position as requested by the supervisor.
Advises direct supervisor of any obstacles to achieving responsibilities.
Knowledge, Skills, and Abilities
Associates Degree, Bachelor's Degree preferred.
Knowledge of OPWDD Part 624 Regulations, preferred.
Previous experience in Quality Assurance/Quality Improvement within OPWDD, OMH, DOH, OASAS, or OCFS preferred.
Successful completion of OPWDD/NYS Justice Center for the Protection of Vulnerable Persons Incident Investigation Training, upon hire.

Position Overview: Responsible for the safe, economic operation of company vehicles, and the safe and timely transportation of passengers. A Day in the Life of a Human Resources Generalist:

The Asset Manager is responsible for maximizing value and ensuring efficiency of operations of all CDS Housing properties. Ensure properties in operation are effectively managed and maintained by third-party management agents.
Monitor operations of the portfolio, analyzing relevant data to identify anomalies or inefficiencies. When issues are identified, work with property management to take corrective action and notify senior staff as appropriate.
Review annual, quarterly, and monthly reports (financial, agency, and investors) and provide guidance to property management to improve financial performance.
Analyze and evaluate monthly and/or quarterly financial statements. Write clear and concise owner's reports summarizing findings for management.
Analyze operational data to identify trends and recommend appropriate strategies and adjustments to promote strong financial returns and long-term viability for each property.
Visit property sites and participate in site audits as requested.
Ensure escrow accounts are fully funded and all tax payments are correctly calculated and paid timely. Monitor reserve balances for appropriate funding levels.

Review management plans and agreements and amend/update as appropriate.
Act as the lead representative for all lease-up activities (AFMHP, rent-up meetings, etc.).
Prepare summary reports for property dispositions.

Receive incident reports and keep track of claims in process.
Maintain electronic copies of policies and insurance certificates; ensure management agent receives updated certifications annually.
Track all fixed assets at all locations.

Summary:
To provide supervision and direction to all staff and participants in their assigned module or group. To provide coordination for assigned participants, assuring implementation of the participant's IPP, integration of all services and progress made by the participant. Essential Job Functions:
Participates in/facilitates and writes up the Intake meetings, reviews and special ITT's for assigned participants.
Coordinates with members of the Team, participants' families, and residences to facilitate program planning and to maximize optimum functioning of the participant in his/her program.
Assesses habilitation skills of participants assigned to the group. Develops, implements, and documents outcomes for all assigned participants. Provides input to the Interdisciplinary team, ensures training is consistent with the IPP. Monitors participant progresses toward the IPP outcomes with the ITT and adjusts as needed.
Ensures documentation is completed for services provided - data sheets, outcome performance records, progress notes, travel logs, etc.
Develops a curriculum for all activities which is outcome oriented and applicable to the individual participant needs.
Assumes all record keeping requirements are met for assigned participants - ensuring that charts are maintained accurately with current information at all times.
Screens and recommends for employment and termination of employees under the specific chain of command assigned to the position. Secures references.
Provides training to staff as needed.
Supervises the module's assigned staff, providing ongoing continuous supervision throughout the program day. Gives praise and discipline, when necessary (for assigned staff) and completes their performance appraisals.
Approves time off requests of subordinates and secures coverage for duties when absent.
Attends management meetings for module development.
Maintains program area and equipment, completing requests for new supplies and/or work orders.
Performs any other duties as required by assigned supervisor.

A Day in the Life of a School Psychologist:
Serves as a member of a multidisciplinary evaluation team with responsibility for completing evaluations of children including social histories, record reviews, and observations of students in preschool/natural environments.
May conduct Functional Behavioral Assessments (FBA) as necessary. Collaborates to support completion of Behavior Intervention Plans (BIP) if required based on the results of an FBA. Monitors the BIP and assists the classroom teacher in completing regular progress updates.

Summary:
Under supervision of the Manager of Nursing Services, the I/DD RN provides nursing services to assigned sites, consultation, and any required clinical supervision related to the provision of nursing services for individuals with intellectual and developmental disabilities. The RN will function as an Interdisciplinary Treatment Team (ITT) member and assure compliance with all applicable OPWDD and other regulatory standards.
Essential Job Functions:
Provide initial and annual nursing assessments on all individuals, following the format established by the Program
Oversee and deliver plans of nursing service for individuals at assigned sites. Create, revise, implement, and train policies and procedures, including plans of care and physician orders
Provide information and referrals to medical or mental health providers as needed.
Responsible for appointment scheduling, monitoring and tracking
Provide direct clinical supervision to assigned staff.
Participate in clinical meetings and serve on assigned committees.
Fulfill all record keeping and documentation requirements of the individual's MAR, tracking, and medical chart as per Agency requirements
Provide or assure required education and training to staff including medication certification, new employee orientation, and specific nursing needs as required at assigned locations
Provide phone triage as needed during regular business hours
Act as point of contact and liaison for medical providers and families in regard to individual's nursing needs
Collaborate with Residence Managers on a regular basis to ensure appropriate medical coverage and communication of individual's needs at sites as needed
Participate in annual audits by Agency and/or regulatory bodies
Perform all other necessary duties relevant to the position as requested by the supervisor.

Education and Experience:
Must be licensed and currently registered as a Registered Nurse by the State of New York State Education Department.
Specialized training or one year of professional experience in treating the developmentally disabled.

Summary:
Provide direct care services to the individuals who reside at the house. Assist Residence Manager with day to day operations of the program.
Essential Job Functions:
Carry out direct care duties as assigned. This includes, but is not limited to:
Encouraging independence in all areas of the resident's lives; treating them with dignity and respect.
Housekeeping, meal preparation, recreational activities, and meeting dietary needs.
Adhering to the resident's Individual Program Plans (IPP's).

Ensure the use of communication systems ensuring information is disseminated to appropriate persons/programs as needed. Keep the Manager and/or designee informed of events that occur, including incidents and serious reportable events.
Ensures all paperwork and documentation is complete.
Attend meetings as required and provide in-service training to staff regarding goal implementation, documentation, and other related areas. Ensure assigned direct care staff completes all required training.
Participates in the on call system for the house.
Ensure home is maintained in a clean, safe, and well-organized manner (according to monthly home inspection checklists); monitor supplies/equipment/physical plant, and inform manager of needs.
Adhere to all Agency policies and procedures, NYCRR 624, 633, 635, and other relevant regulations.
Performs any other related duties as directed by the supervisor.
