Provides patient care through the provision of direct operational and/or clinical patient services. Pharmacists will collaborate within the department to enhance departmental operations and clinical programs. Staff will participate in departmental and/or institutional committees, actively participate in the provision of education, and promotion of the profession. This position will require employees to work weekends, any required evenings, and holidays. The amount of time allocated to operations and clinical services will depend upon patients requirements, staffing, and overall pharmacy workload. This position also acts as a mentor to facilitate learning among pharmacy students and/or residents.
The ClinicalPharmacist collaborates with the multidisciplinary team to improve care, achieve optimal outcomes, and provide continuity of care to patients through the provision of pharmacy patient care services to include therapy care plan development, medication management, education, monitoring and follow up. The pharmacist provides evidence-based and cost-effective comprehensive medication management for patients with chronic conditions.
In order to enhance collaborative patient care, pharmacists will be delegated the authority to obtain a patient medical history, order and evaluate laboratory results, and make medication decisions including modifying or discontinuing treatment when appropriate with their scope of practice.

Provides accurate medication order-entry, drug distribution, and patient care. Confirms the appropriateness of drug therapy as it relates to the care of neonates, pediatrics, adolescents, adults and geriatrics. Seeks to minimize turnaround time for products and services.
Provides concise, applicable, timely responses to requests for drug information.
Oversees pharmacy support personnel; checks and verifies the accuracy of all tasks performed. Staff may also function as a lead pharmacist within the department to monitor workload and workflow.
Shares clinical and scholarly activity that promote the evidence-based use of medications and advance the profession of pharmacy to a variety of audiences (represents Prisma Health within professional organizations through service, presentations or publications; provides education within didactic and experiential training programs and to postgraduate professionals, nursing and medical staff). Provides lab review, clinical services, patient education, and corresponding documentation via the departmental clinical initiatives.
Provides experiential education, evaluation, and training for technicians, students, pharmacy residents, medical students/residents and new employees.
Represents the department in its collaborative effort to integrate pharmaceutical care throughout the patients continuum of care.
Provides leadership and pharmaceutical excellence through direct patient care activities as a member of a multidisciplinary healthcare team.
Acts as liaison between the department of pharmacy and service line.
Performs other duties as assigned.
Supervisory/Management Responsibilities
This is a non-management job that will report to a supervisor, manager, director or executive.
Minimum Requirements
Education - Bachelor's degree from an ACPE-approved college of Pharmacy. PharmD preferred.
Experience - One (1) year of Pharmacy Resident experience or three (3) years clinical pharmacy experience.
In Lieu Of
In lieu of an active Pharmacist license, may be a graduate of an ACPE accredited US college of Pharmacy with an intern certificate pending initial licensure or have completed requirements for reciprocity pending board of pharmacy interview for licensure.

The incumbent is responsible for the provision of comprehensive and age appropriate patient care. The incumbent is considered a source of pharmaceutical clinical information for direct patient healthcare and for the professional staff.
Job Description
Clinical Functions
Conducts medical staff authorized treatment of patients. Takes an appropriate health history, adjusts the developed treatment plan, and monitors the patient's outcome to the prescribed treatment plan.
Provides ambulatory care services for complex patients. Accepts referrals for drug therapy consults, develops treatment plans, and manages drug-drug, drug-disease, and drug-food interactions.
Interviews patients to ascertain significant past medical history and medication history. Evaluates medication regimens for appropriateness, drug interactions, therapeutic duplications, non-compliance and adverse drug reactions.
Performs medication reconciliations via chart review and/or patient interview.
Analyzes medication regimens to ensure effective, safe, and economical treatment plans.
Initiates, monitors, titrates, discontinues, and/or alters medications per CPP agreements. Prescriptions will be written by the CPP as appropriate.
Provides recommendations to other providers for therapeutic adjustments when not part of the CPP agreement.
Orders and interprets laboratory tests as approved by medical staff.
Schedules appropriate follow up for the patient.
Counsels patients and families on drug use, therapeutic effects, potential adverse effects, precautions, appropriate storage of pharmaceuticals, and expected outcomes of drug therapy. Verifies the level of patient understanding concerning prescribed medications.
Transmits appropriate medical and pharmaceutical information to providers, nursing, and patients.
Documents clinical activities, updates medication allergy information, and verifies the accuracy of the medication list within the medical record.
Serves as therapeutic consultant and drug information resource to the medical staff as well as other Primary Care staff.
Conducts non-formulary medication research and provides recommendations for approval or denial.
Identifies and intervenes with patients that would benefit from pharmacist consultation.
Utilizes population health tools to improve the care of their assigned patients.
Collaborates with the healthcare team, providing drug information and staff education.
General Functions
Manages panels of consulted patients.
Provides health promotion and disease prevention activities related to drug use and preventative therapy.
Directs and completes clinic and/or pharmacy based quality improvement projects.
Participates in peer review processes.
Completes CPP meetings as required by the NC Board of Pharmacy

Education/Experience
The incumbent must be a graduate of a college of pharmacy accredited by the American Council of Pharmaceutical Education.
The incumbent must have earned a Bachelor of Science in Pharmacy or a Doctor of Pharmacy (Pharm D) degree.
The incumbent must have obtained a Specialty Board (BPS) Certification (BCPS/BCACP), is a Certified Geriatric Pharmacist as certified by the Commission for Certification in Geriatric Pharmacy, or completed an ASHP accredited residency program and have two total years of clinical experience. (residency year counts as one year of experience)
The incumbent must have an active unrestricted pharmacist license, and must be eligible for unrestricted pharmacist licensure in North Carolina. Pharmacist licensure in a state different than North Carolina is acceptable, however, if a Tribal Direct Employee, the incumbent must be eligible to obtain a North Carolina pharmacist license within the next 3 months following employment with CIHA. The incumbent must be able to apply for a NC ClinicalPharmacist Practitioner license within 12 months of employment at CIHA. Federal Employees, including commissioned officers, may hold and maintain an unrestricted pharmacist license from any State, and aren't required to maintain licensure with the NC Board of Pharmacy. However, commissioned officers, are highly encouraged to obtain their NC pharmacist and CPP license in order to practice at the top of their licensure for this position.
Job Knowledge
Professional knowledge of the theories, principles, practices, and techniques of clinical and basic pharmacy. Ability to apply such knowledge to the provision of quality direct clinical-pharmaceutical patient services.
Knowledge of the physical and chemical properties and characteristics of substances, therapeutic actions of drugs, and the principles of chemical, biological, pharmacological, and biopharmaceutical science.
Knowledge of current diagnostic, preventative, guidelines and therapeutic approaches related to the various disease states encountered in Primary Care.
Documented credentials and privileges by the Medical Staff are required, prior to conducting pharmacy-based clinic services.
Basic Life Support (BLS) certification is required
Skill in planning, monitoring, and evaluation of drug therapy in a clinical setting. Ability to manage pharmaceutical patients with exceedingly complex medical pharmacy histories, including patients on multiple drugs, managing severe side effects, and adverse drug reactions.
Ability to communicate with patients regarding disease states, desired outcomes, laboratory tests, proper administration, use, actions, precautions, and storage of medications.
Ability to communicate with fellow health professionals regarding patient care and pharmaceutical information and intervention.
Requires knowledge of cultural and age specific competencies.
Position requires maintenance of active and valid driver's license.
Complexity of Duties
The work often requires many different and unrelated processes. Decisions regarding what needs to be done includes the assessment of unusual circumstances, variations in approach and incomplete or conflicting data. The work requires originating, establishing, and monitoring protocol criteria and developing new clinical-pharmaceutical techniques and methods not previously utilized. The work includes managing comprehensive, clinical-pharmaceutical patient treatment plans of which many, are made extremely complex by: implementing novel techniques, new methodologies, and application of state of the art pharmacy knowledge, skills, and abilities. The work requires independent judgment wherein error could result in serious injury or death to a patient. Liability insurance is recommended.
Supervision Received
The position is under the direction of the Advanced ClinicalPharmacist Supervisor
The Incumbent operates relatively independently in the performance of usual, customary, and clinical pharmaceutical services. The incumbent will have the authority to make final decisions about technical aspects related to the provision of pharmaceutical services.
Unusual and/or non-routine situations are to be referred to the Advanced ClinicalPharmacist Supervisor or designated Supervisor.
The incumbent aids in the maintenance of the overall integrity of the program.
Additional policy and procedure guidance is received through the higher grade supervisor, the Cherokee Indian Hospital Pharmacy Policy and Procedure Manual, Service Unit policies and procedures, Indian Health Service policies, and Joint Commission guidelines and directives.
Responsibility for Accuracy
Errors in this position would be difficult to detect and can have adverse effects on the health of individuals, groups, and families. Errors can place the pharmacy at risk for substantial legal and financial liability. Undetected and unresolved errors can result in a range from minimal to life threatening patient consequence.
Contact with Others
The position requires frequent and substantial contact with others, which include health care professionals, support and clerical staff, patients, community members, business representatives, and representatives of outside organizations of all types. Contacts are numerous and, at times, can be under difficult circumstances or in difficult settings. Professionalism and tact are imperative for this position. Failure to act professionally can reflect poorly on the facility and department and contribute to unforeseen adverse effects on the organization. Utmost sensitivity is required when interacting with patients and their families.
Confidential Data
Much of the information processed in this position is considered highly sensitive and confidential. It must not be disclosed to other parties except within the performance of one's duties. All information will be handled in accordance with the Cherokee Hospital's guidelines and Federal Law. Improper handling of information can be detrimental to the operation of the organization and department and damage Pharmacy relations with its clients. Failure to maintain proper confidentiality of all records and improper disclosure of sensitive patient information can result in civil and criminal penalties.
Mental/Visual/Physical
Job duties can be mentally, emotionally, and physically stressful at times. The work requires balancing multiple priorities in a busy, rapid paced, work environment while attending to patient safety. The job is subject to frequent interruptions, which can cause distractions and potential for error. Physical activities require the ability to walk, sit, reach, bend, and lift. The job requires visual acuity, manual dexterity, and the ability to speak and hear clearly. Repetitive activities will occur with the use of a computer keyboards and preparation of pharmaceutical prescriptions. May occasionally lift more than 15 pounds.
Environment
The work is performed in a health care environment within a clinic, hospital, emergency room, field clinic, and/or pharmacy setting. Potential exposure to infectious and contagious disease can occur. Lifting or moving of pharmaceutical supplies, equipment, and patients that weigh over 50 pounds is possible. It may be necessary to work when Administrative leave is granted if patient care functions are required and/or if such care would be compromised.
Extent of Supervision
There is no official supervisory responsibility over permanent staff for this position. However, the incumbent may be called upon to direct work activity of support personnel and fellow colleagues during immediate, routine, and daily pharmacy patient and prescription operations. The incumbent does provide supervisory control and direction over pharmacy residents and student externs.
Scope of Supervision

Initiates, monitors, titrates, discontinues, and/or alters medications per CPP agreements. Prescriptions will be written by the CPP as appropriate.

Education/Experience
The incumbent must be a graduate of a college of pharmacy accredited by the American Council of Pharmaceutical Education.
The incumbent must have earned a Bachelor of Science in Pharmacy or a Doctor of Pharmacy (Pharm D) degree.
The incumbent must have obtained a Specialty Board (BPS) Certification (BCPS/BCACP), is a Certified Geriatric Pharmacist as certified by the Commission for Certification in Geriatric Pharmacy, or completed an ASHP accredited residency program and have two total years of clinical experience. (residency year counts as one year of experience)
The incumbent must have an active unrestricted pharmacist license, and must be eligible for unrestricted pharmacist licensure in North Carolina. Job Knowledge

The work often requires many different and unrelated processes. Supervision Received
The position is under the direction of the Advanced ClinicalPharmacist Supervisor
The Incumbent operates relatively independently in the performance of usual, customary, and clinical pharmaceutical services. The incumbent will have the authority to make final decisions about technical aspects related to the provision of pharmaceutical services.
Unusual and/or non-routine situations are to be referred to the Advanced ClinicalPharmacist Supervisor or designated Supervisor.

The position requires frequent and substantial contact with others, which include health care professionals, support and clerical staff, patients, community members, business representatives, and representatives of outside organizations of all types.
