Reporting to the Pharmacist in Charge, the Clinical Intervention Center Pharmacist (CIC RPh) plays a critical role in the healthcare industry by ensuring the safe and effective use of medications. The primary responsibilities of the CIC RPh involve monitoring clinical clarifications, change in prescriptions, backorders and anything else that would hinder the dispensing of medication to a patient. They are also responsible for communication with nursing centers and collaborating with healthcare professionals to optimize patient care. ESSENTIAL FUNCTIONS: To perform this job successfully, an individual must be able to perform each essential function satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Predictable, reliable, and punctual attendance is an essential function of the job role. The CIC RPh should be able to perform all functions of a staff pharmacist as well as those functions listed below. Duties are assigned for multiple pharmacies. Responsible for monitoring and following up on clinical clarifications utilizing eMAR system, dispensing history, and direct communication with the center. Should clear old pending clarifications from queue based on new orders received or non-responsiveness by profiling or deleting orders. Review and approve Change in Prescription suggestions based on payer preference utilizing the ECS message and test claims. Review and Approve therapeutic interchange suggestions for non-auto TI states. Communicate national backorder issues with center. Determine if alternative orders are required or for short term out of stock issues, utilizing backup pharmacies. Respond to email and other electronic communication (internal and external) including requests for RX Verify control withdrawals (if appropriately licensed). Review emergency CII prescriptions and contact the center or MD to ensure timely receipt of hardcopies. All other duties as assigned. Computer Skills: Intermediate proficiency with Pharmacy and medication distribution software (Pyxis Medbank, Frameworks,and experience with long term care pharmacy software preferred. Language Skills: Ability to read and interpret documents such as medication documentation, business correspondence and procedure manuals; ability to write routine reports and business correspondence; ability to speak effectively, both in person and by telephone, to customers or employees of the organization. Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals; ability to compute rate, ratio, and percent and to draw and interpret bar graphs; ability to understand and interpret statistical reports, data charts and graphs. Clinical Pharmacist, IV Services/IV Specialist * Oversees the preparation and storage of sterile preparations for inpatients and outpatients in the Outpatient Infusion Unit. Ensures compounded sterile preparations are prepared accurately and safely in accordance with USP 797 requirements, professional standards of practice, and laws, rules and regulations. Manages the preparation, storage and handling of hazardous materials, including spills. * Provides optimization of parenteral nutrition in conjunction with nutrition services by daily review of parenteral nutrition orders for appropriateness and safety by monitoring pertinent lab values, osmolarity, and other pertinent parameters. Makes recommendations to prescibers in order to optimize therapy. * Optimizes chemotherapy regimens by reviewing the regimen for indication, dose, route, preparation, and adjuvant medications including hydration. Monitors concomitant medications for drug interactions with the chemotherapy regimen. Monitors laboratory values as needed to evaluation for toxicity. Oversees accurate and safe preparation of chemotherapy. * Oversees the dispensing of sterile products from IV services, and directly supervise support staff. Works with the Inventory Manager to ensure proper acquisition/stock points of high-cost items. Generates proper electronic charges to facilitate billing/financial services. Oversees proper cleaning and maintenance of the clean room, anti-room, and laminar flow hoods. * Maintains up-to-date knowledge in pharmacotherapeutics, with a focus on oncology, nutrition, and biologicals. Maintains appropriate references pertaining to preparation of CSPs and chemotherapy, including related policies and procedures. Leads quality improvement processes in IV Services. * Performs all other related duties as assigned. Job Qualifications and Specifications Minimum of a Bachelor Degree in Pharmacy from an ACPE accredited School. Valid RI Pharmacist license. * Clinical Specialist: as above, plus preferred residency or 5 years of clinical experience. BLS/ACLS/PALS preferred. Board Certification in Pharmacotherapy encouraged. Additional training and competency assessment is required for these duties. * IV Specialist: as above, plus expertise in USP 797 protocols and procedures, chemotherapy protocols, hazardous drug handling, and parenteral nutrition.
