About the Family Centered Treatment Team
The FCT team is a fully implemented and homebased evidenced based trauma treatment model used to stabilize or reunify families. Our goal is to help families identify practical solutions to solve problems to keep them together in the community and to treat inter-generational trauma. Our core belief is that recipients are great people with tremendous internal strengths and resources.
As a Therapist with FCT, you will:
Work a flexible weekly schedule that includes some evening hours for client appointments, bases on client/family availability
Learn the evidenced-based framework of FCT and the four phases of treatment-including completing Level 1 Certification within 12 months of hire, supported by the supervisor and team trainer.
Provides direct clinical treatment using methods compatible with FCT principles and practices for assigned cases and meets fidelity requirements for treatment intensity (5 hours per week per family). Caseloads vary from 3-5 families, depending on travel.
Maintain clear, concise, and timely documentation records in an Electronic Health Record system to meet state, county and EBP standards.
Collaborate with all relevant systems and key participants to ensure buy-in and cooperation throughout FCT treatment
Participate in weekly individual and team supervision and provide kind, direct, and honest feedback to team members, including participation in peer reviews.
Participate in a rotating 24/7 caregiver coaching support system that has been established by the team (1-2 weekends in a quarter).
Essential Skills and Qualifications
Background Check Required
Education: Master's degree in Social Work with Mental Health concentration, Counseling, Psychology, Marriage and Family Therapy, or related program is required.
Experience: Practicum experience, through your degree program, related to services LSS provides is required.
Valid driver's license and reliable transportation are required.
Eligibility for training or full license by meeting criteria set for by WI Department of Safety and Professional Services as an Advanced Practice SocialWorker, Licensed ClinicalSocialWorker, Licensed Practical Counselor, Licensed Marriage and Family Therapist.

CLINICIAN Youth Crisis Stabilization Facility - WCS Jefferson County Responsible for providing individual therapy and supervising milieu group therapy to the youth residing at Youth Crisis Stabilization Facility utilizing the Integrated Treatment Model including Dialectical Behavior Therapy (DBT). Provide service coordination in collaboration with the Youth Treatment Specialist team.
The Youth Crisis Stabilization Facility is licensed under the Department of Health Services Chapters 50 and 34 to provide a short-term stabilization service for up to 8 youth (ages 10-17).
Essential Functions:
1. Interview, assess, screen, and approve admissions of youth for services at YCSF.
2. Assist with admissions process on site, assisting families with paperwork completion and orientation of youth to the program.
3. Supervise the youth coordinator, providing weekly individual supervision, review and sign off on youth care plans and service recommendations/resources.
4. Facilitate treatment and value-added groups for the youth including but not limited to DBT skills, sex education, healthy relationships, cultural awareness, substance use, independent living and life skills for 8 youth across the gender spectrum, ages 10-17 years old.
5. Provide daily (more frequently as needed) 1:1 individual therapy session following treatment plans utilizing a variety of therapeutic approaches in the spirit of DBT.
6. Complete assessments to develop youth care plans including stabilization and safety plans.
7. Provide DBT training and coaching to the direct care staff by engaging in activities/programming to role model and teach DBT skills to both youth and staff.
8. Provide weekly clinical supervision to Youth Treatment Specialists in the YCSF program per DHS 34 requirements.
9. Engage in monthly peer review with clinical supervisor per DHS 50.
10. Participate as a member of the medical peer review committee per DHS 50.
11. Participate in consultation teams both as a team member and as a facilitator for program staff to assist in monitoring the fidelity of the milieu group therapy.
12. Participate in weekly consultation, training and supervision on the integrated treatment model.
13. Conduct daily risk assessments to ensure the youth are safe and not a risk to themselves or others.
14. Coordinate with Jefferson County Crisis Services as needed.
15. Work collaboratively with child and family team to implement a comprehensive treatment plan outlining each youth's identified target behaviors.
16. Attend court hearings as needed to provide updates and advocate for youth.
17. Work on-call to ensure clinical needs of youth are being met and provide clinical guidance to staff during crisis after hours.
18. Document all interactions in the electronic medical record SecurManage.
19. Engage with youth's support network and participate in family engagement initiatives.
20. Attend psychiatric medical appointments and assist in medication administration.
21. Keep appropriate documentation for all funding sources.
22. Maintaining resident record files in accordance with YCSF certification, Department and Health Services (DHS) 50 and 34 to ensure all records are updated and complete.
23. Uphold and promote a positive and supportive culture for youth and staff.
24. Maintain confidentiality within all written and verbal communications
Other Duties and Responsibilities: Provide coverage and direct service to youth as needed. Other job-related duties as may be necessary to carry out the responsibilities of the position.
Knowledge, Skills, and Abilities:
Working knowledge of positive child/youth development; patience and understanding of difficult life situations; the ability to interact with escalating youth and/or parents in a calm and professional manner; ability to follow oral and written instructions; ability to respond appropriately in crisis situations; proficient with Microsoft Word and Excel; perform basic computer functions; uses time efficiently and effectively; ability to multi-task; sensitivity towards cultural, ethnic and disability needs.
Minimal Qualifications:
A master's degree in a social work, psychology, or a behavioral science field. In collaboration with Insurance Coordinators, provides information and education to patients about payment to dialysis (federal, state, commercial insurance, state renal programs, AKF HIPP, and entitlement programs).
Collaborates with the Insurance Coordinator of any changes to patient state that impacts insurance i.e. transplantation, discharged, loss of coverage, or extended travel.
Refers patients to patient billing solutions (PBS) department for questions/concerns in regard to treatment related bills

DEFINITION

EXAMPLES OF DUTIES
Receive, analyze and document reports of alleged maltreatment and unborn child abuse. Perform a gate-keeping function by determining which children and families receive further attention from child welfare. Meet with families to identify and prioritize needs and goals and to consider appropriate available services. Establish service agreements with families that ensure child safety, address identified needs within the family, and are consistent with juvenile court orders. Make referrals to appropriate community services and resources. Maintain regular contacts with schools, treatment providers and other involved professionals to ensure service provision, effectiveness, and family's progress. Maintain regular contact with the families to monitor progress and to assist them in following the case plan. Prepare factual, accurate and comprehensive petitions and court reports according to agency policy and file them in court within legal time limits. Attend court hearings according to agency policy and effectively/ proactively participate in the process. Contact families promptly after case assignment to schedule meetings. Prepare for assessments by reviewing the case record and other available information prior to family meetings. Conduct assessment interviews with family members according to Department protocols and State mandates. Gather additional information from collateral sources to complete assessments. Within required time limits, complete comprehensive and accurate family assessments, taking into consideration family and individual strengths and needs. Conduct initial assessments by reviewing all relevant information, interviewing all persons according to state standards and within mandated response times. Analyze information regarding risk level, child safety, and maltreatment substantiation.
Education, Training and Experience:
Applicants with a Bachelor's or Master's Degree in Social Work, or who are currently in their final academic semester towards obtaining a Bachelor's or Master's Degree in Social Work will be screened through. Applicants with any other major must possess a training certificate, temporary or permanent Social Work Certification from the Wisconsin Department of Safety and Professional Services (DSPS) prior to applying.
SocialWorker II:
All employees hired with a Master's degree and six (6) months of post-graduate t experience relevant to the position or a Bachelor's degree and two-and-a-half (2.5) years of post-graduate experience relevant to the position will be hired as a SocialWorker II, range 20. Those who do not meet these criteria upon hire will be hired as SocialWorker I and will advance to SocialWorker II after obtaining one of these criteria.
Certifications/Licenses:
All individuals must possess a degree and a training certificate, temporary or permanent Social Work Certification from the Wisconsin DSPS prior to the first day of employment. If any applicant does not possess a training certificate, temporary or permanent certification on the first day of employment the job offer will be rescinded and they will be terminated. Candidates with a training or temporary certificate must obtain a permanent certification within one (1) year in order to pass probation.
Special Requirements:
