City/State Williamsburg, VA Work Shift First (Days) Sentara Regional Resource Pool located in Hampton Roads is hiring an Inpatient Case Manager, MSW -Days/PRN for the Resource Pool. Required to work 4 hospitals in the Southside or Western Tidewater regions. Location and unit to be assigned based on greatest need.There are three region options.Must be within 75 miles and 90 minutes of all four hospitals in assigned region.
• Operational hours = 8:00 AM - 4:30 PM 7 days per week
• Flexi - Hours: The team member must enter at least 48 hours of prescheduled time per calendarmonth.
• Facility supported: Members of the team are required to work assigned shifts in all units within the case management specialty. Required to work in 4 hospitals in the Southside or Western Tidewater regions.
• Operational hours = 8:00 AM - 4:30 PM 7 days per week
• Flexi - Hours: The team member must self-schedule at least 48 hours of prescheduled time per calendar month in API. Of these prescheduled hours, 16 hours are to include availability to be scheduled on a weekend.

Education:
*Master's degree in Social Work
Certification/Licensure:
*Certification in Case Management (CCM, CMAC or ACM-SW) required within one year of eligibility.

As an Inpatient Care Manager, responsible for the provision of medical social work services to patients receiving care in the hospital setting. Assesses the social, psychological, cultural, environmental, and financial situations, as well as disposition needs of each referred patient. Collaborates with patients, families, healthcare team members, and community organizations, and uses strong advocacy skills to identify needs and resources in the hospital and community.
Develop and evaluate effective transition plans taking into consideration the client's diagnosis, prognosis course of treatment, past and present services, short-term and long-term goals, provider options, and/or available health care benefits as appropriate to the continuum of care. Central State Hospital (CSH) is a 277 bed Joint Commission accredited psychiatric inpatient facility located in Petersburg, Virginia. CSH has proudly been serving citizens of the Commonwealth since 1870, when we opened our doors as the first psychiatric hospital in the country dedicated to the treatment of African Americans. A new and modern CSH is currently under construction on the same property, with a scheduled completion date of 2027.
CSH maintains the only maximum-security treatment facility in the state (111 beds). We also have 166 beds available for individuals in the community who need inpatient care, as well as individuals in the criminal justice system. We believe that treatment should be holistic and focused on the person's individual needs, recognizing that recovery is not linear or the same for every person. Treatment modalities may include medication, evidence-based group and individual therapy, peer support, psychosocial rehabilitation, vocational training, preparation for court, and treatment for substance use.
Central State Hospital is recruiting for a Clinical Psychologist to join our dynamic inpatient treatment team. The Clinical Psychologist completes initial, annual, and suicide risk assessments; conducts NGRI Acquittee Privilege Requests, Submission Reports, Commissioner-Appointed Evaluations, and Initial Analysis of Risk Reports by assigned deadlines; and provides comprehensive psychological testing and high-quality clinical documentation. This role actively participates in interdisciplinary Treatment Team meetings, offering consultation on client progress, assessment findings, and treatment needs, while assisting with the development and review of individualized treatment plans and ensuring timely completion of TPR notes. What Youll Do:
Performs the psychosocial assessment of patients and families/caregivers in the hospice program in a timely manner and ensures that identified needs are addressed in the patients plan of care. Provides an assessment in the patients identified residence and assistance when this is not safe, and another plan is required
Demonstrates exceptional service to patients, families, referral sources, co-workers, and other customers. Assist team members as needed to ensure that this level of service is consistently provided
Counsels patient and family/ caregivers and staff as needed in relationship to stress, and other identified coping difficulties
Provides accurate and timely documentation of patient services to reflect the plan of care
Provides assistance with applications for Supplemental Security Income, Hospice, Medicaid, and Medicaid Waiver. Obtains resources from community social, health, pharmaceutical and welfare agencies to assist in meeting patient/family needs
Serves as liaison between patients and families/caregivers and community agencies
Qualifications:
Social Work Degree
Virginia: Bachelors degree in Social Work; Masters degree preferred.
Current State Licensure in Social Work
Virginia: Licensed not required.
Minimum of one years experience in health care or hospice setting
Valid drivers license and proof of insurance is required

PRIMARY FUNCTION: Provide individual, group and family therapy as a Residential Therapist for designated youth in Elk Hills residential programs
RESPONSIBILITIES:
Provide and document weekly individual therapy for residents; Provide and document weekly group psychotherapy for residents
Provide and document family therapy sessions, as required
Conduct initial assessments and re-assessments of clients as directed/assigned
Help facilitate and participate in service planning meetings for each residential youth. Conduct and construct service plans, in collaboration with the case manager, for each youth in residential services
Receive documented routine clinical and administrative supervision regarding services from the Residential Clinical Director (LMHP)
Help to obtain and maintain all applicable documentation required by licensing and/or Medicaid in the provision of services
Provide documentation for the Residential Director, Program Director, or Residential Case Manager at agency FAPT and FPM meetings and/or be present virtually when a clients case is being reviewed
Demonstrate sensitivity to the needs of clients, flexibility, in a non-threatening manner, respect for clients autonomy, confidentiality, socio-cultural values, personal goals, lifestyle choices, and family interactions, and appropriate professional boundaries and ethics
Develop and maintain positive relationships with referring agencies, social workers, court service personnel, parents, other licensed mental health professionals and all other persons involved in the residents services
Provide trainings to our staff, as needed, to help serve our youth, families, and community
Attend staff meetings on a weekly basis to help staff each youth but also to help staff understand various intervention strategies to promote youths progress
Work cooperatively with all Elk Hill staff to improve the quality of the whole organization
Serve on-call for those on their caseload that may need additional support and/or in crisis and requesting to speak to their residential therapist or provide additional support to the current staff mediating a situation
For LMHP-E clinicians: Provide all documentation required by Department of Health Professionals to clinical supervisor(s) in a timely fashion for completion. Submit all required supervision paperwork to Department of Health Professionals as required. Receive routine, as outlined within the parameters for eligibility of licensure, clinical and administrative supervision
If a licensed clinician, can provide monthly clinical and administrative supervision to all residential staff after two years of having their license and took supervisory course
If a licensed clinician, help support the Residential Clinical Director when on vacation, by signing off on treatment plans and approving them.
Other duties as assigned
ORGANIZATIONAL RELATIONSHIPS:
Responsible to the Residential Clinical Director
As a Clinical Therapist for assigned youth, work cooperatively with the Residential Program Directors, Case Managers, House Managers, and day/public school staff
If a licensed clinician providing supervision, responsible for providing appropriate and comprehensive supervision and documentation
CORE COMPETENCIES AND EXPERIENCE:
Experience with providing therapeutic services to at-risk youth preferred
Strong written and verbal communication skills
Ability to work independently and without direct supervision
Willingness to travel and work non-traditional business hours
Exhibit consistent personal integrity in dealings with residents, families, co-workers, and community agencies
The ability to support constructive family contact and involvement in community activities as appropriate
Ability to engage, support, and work effectively with others, including but not limited to the treatment team and other internal and external stakeholders
The temperament to work with, and care for, children and families with special needs
Must demonstrate sensitivity to Elk Hills service populations cultural and socioeconomic characteristics
CREDENTIALS AND/OR EDUCATION:
A masters degree in Social Work, Psychology or Counseling and a combination of two years professional experience with children, preferred.
Must be a Licensed Mental Health Professional; If providing clinical supervision to LMHP-E, must meet minimum board qualifications to provide supervision which consists of two years being licensed and having completed the two-day supervision seminar requirement; or
A masters degree in Social Work, Psychology or Counseling and a combination of one-year professional experience with children required. Must be board approved as license eligible and receive supervision from an LMHP
Ability to maintain professional license, or, continue working toward obtaining professional license
Must have up-to-date certification in the facilitys Behavior Management Program and have the willingness and ability to apply such skills as needed and/or required in the day-to-day interactions with youth
Must have up-to-date certifications in approved courses for CPR/First Aid
Maintain valid driver's license and good driving record.
PHYSICAL REQUIREMENTS:
Ability and willingness to physically perform all functions of the facilitys Behavior Management Program
Good physical health and remain current with annual TB screening and other certifications.
Ability to traverse inside and outside of the property including uneven terrain
Periods of sitting at a desk and working on a computer
Occasionally lift up to 25 pounds
TRAVEL REQUIREMENTS:
Periodic transportation of a client as needed or required
Frequent travel to other locations as needed or required
