Responsibilities

Primary Accountabilities:
Consult and collaborate on a multidisciplinary team to provide integrated on‐site care, recommendations, and feedback to physicians, physician residents, and advanced practice providers. Practice in a population‐based practice management strategy with flexibility for warm‐handoffs and the ability to respond to urgent situations. Engage in and advocate for CommUnityCare's primary care‐based behavioral health care model to leverage innovation and strategic roles and responsibilities to promote behavior change for optimal patient whole person health. Support and promote care team capacity to recognize and treat mental disorders and psychosocial problems. In expert consultative capacity, assess the clinical status of patients referred for behavioral health support. Use population health data and techniques to support behavioral and mental health needs of a primary care panel. Collaborate with care teams to effectively and efficiently treat and manage patients with chronic emotional and/or health problems. Collaborate with the care team to refer cases to mental health specialists as appropriate. Identify "at risk" patients and develop care plans to promote psychological or physical well‐being. Build care plans that address relapse and morbidity prevention in conditions that tend to recur over time. Collaborate on patient care plans with the care team. Teach patients, families, and staff care, prevention, and treatment enhancement techniques. Attend and participate in meetings and Quality Improvement activities as appropriate. Respond to patient or co‐worker complaints and work toward a positive resolution of any dispute. Work effectively on a team and communicate respectfully with patients and all staff based on shared goals and mutual respect. This includes active and positive participation in continuous process and quality improvement efforts to improve patient care within the organization's care model framework. Monitor the site's behavioral health program, identifying problems related to patient services and making recommendations for improvement. Participate in evaluation of peers and support staff. Attend patient care huddles, meetings, patient conferences, or planning sessions. These may be related to quality assurance, patient care and other related topics within the clinic. The Specialist offers on-site counseling and interventions for youth experiencing emotional, behavioral, or relational challenges impacting learning, development, or stability, while collaborating with program teams to create supportive environments where youth can thrive. In addition to client services, the Specialist trains and supports youth program staff in trauma-informed engagement.
This position is funded for three years, with potential for extension based on program outcomes and funding.
Essential Job Functions
· Provide case management and referral services for children and families experiencing behavioral, social-emotional, or mental health challenges.
· Conduct child- and family-centered assessments to identify strengths, needs, and potential risks.
· Develop and monitor action plans that promote stability, academic success, and positive family functioning.
· Implement crisis intervention, safety planning, and de-escalation strategies to support children and families in distress.
· Facilitate family engagement and caregiver coaching to strengthen communication and supportive home environments.
· Maintain accurate, timely documentation of services, referrals, and outcomes in compliance with agency standards.
· Track service utilization and prepare progress and outcome reports for internal stakeholders.
· Collaborate with Learning Center staff to support children displaying behavioral, emotional, or trauma-related challenges.
· Provide on-site consultation, modeling, and coaching for Learning Center staff in trauma-responsive, developmentally appropriate practices.
· Deliver trainings and workshops for staff on child development, social-emotional learning, and behavior management.
· Partner with Learning Center teams to create safe, supportive, and inclusive environments that promote resilience, academic engagement, and well-being.
· Serve as a liaison between families, schools, CPS, and community providers to coordinate wraparound services.
· Participate in multidisciplinary team meetings to align care plans and ensure continuity of support.
· Advocate for children's access to educational, mental health, and community resources.
· Participate in outreach efforts, including resident events, health fairs, and community activities, to build trust and connect residents with available supports.
· Prepare annual reports and updates summarizing program interventions, service trends, and effectiveness measures
· Participate in required staff meetings, trainings, and workshops.
· Home visits are necessary when assessing safety and needs.
· Uphold Foundation Communities' commitment to equity, inclusion, and trauma-informed care in all aspects of service delivery.
· Perform additional related duties assigned to support program and agency goals.
Minimum Qualifications
· Master's Degree in Social Work.
· Must be a licensed socialworker in the state of Texas, either LMSW or LCSW.
· Two (2)+ years experience providing therapy or behavioral health services to youth and families.
· Experience supporting child and adolescent development, family systems, and social-emotional learning.
· Strong skills in child/family assessment, crisis intervention, and care coordination.
· Ability to collaborate effectively with multidisciplinary teams.
· Must be willing to work a flexible schedule, including occasional evenings or weekends, to meet program or community needs.
· Must be able to travel or commute regularly to various Foundation Communities properties and locations as needed.
Preferred Qualifications
· Bilingual (Spanish/English or other language).
· Experience in school-based services, afterschool programming, or family case management.
· Familiarity with social-emotional learning frameworks and evidence-based youth interventions.

Responsibilities

* Plays an active role in daily unit-based patient care huddles and periodic clinic-wide informational meetings, patient conferences, and planning sessions. These engagements support quality assurance initiatives, care coordination, and continuous improvement in patient services.
* Participates in ongoing professional development through seminars and educational sessions, ensuring compliance with all licensure and certification requirements while staying current with best practices in the field.
* Participates in the implementation and evaluation of quality improvement strategies to ensure compliance with evidence-based guidelines and standards, while identifying opportunities to enhance patient outcomes and care delivery.
* Ensures all tasks provided and associated with patient care, patient administrative processes, and related duties comply with all regulatory and accreditation standards including The Joint Commission and CommUnityCare Policies and Procedures.

Qualifications
Education:

Work Experience:
