Administers medications, implements primary care orders, and evaluates participant needs physically and/or by telephone. Provides education to Senior CommUnity Care participant and caregiver in regard to plan of care and medications.
QUALIFICATIONS:
Graduate of a school of professional nursing required.
Current Colorado Practical Nurses License (L.P.N.).
Cardiopulmonary Resuscitation (CPR) certification required.
A minimum of one year's experience in working with the frail or elderly population required.
Intravenous Venous (IV) Certification preferred.
Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
Must have a valid driver's license, proof of insurance and have means of transportation.
Skills/Knowledge:
Broad knowledge base of health care problems of the frail elderly.
Knowledge of medical equipment and instruments.

ESSENTIAL FUNCTIONS:
Responsible for processing of medication orders for all participants through direct input or processing of medication orders input by staff provider, ensuring that a complete, accurate, and legal prescription is received by the dispensing pharmacy.
Reviews notes from outside providers and identifies and processes medication orders with approval of PACE provider.
Responsible for coordination to maintain participant medication schedules (maintenance of cycle system) through implementation of new medication orders, mid-cycle medication changes, discontinuations, and reconciliations through transition process as prescribed by policy.
Responsible for medication reconciliation through care setting transitions (LTC/Hospital/ALF discharge) for all participants in accordance with organization procedures and facilitates any changes required.
Responsible for communication with and between PACE providers, community-based providers, and the pharmacy in regard to medication orders, clarifications, and pharmacy recommendations.
Responsible for coordinating with local pharmacies to obtain urgent medications that are not available through on-site dispensing. Coordination involves transmission of medication order, demographics, and insurance billing information.
Responsible for coordinating and/or facilitating on-site medication dispenses through the system provided through the pharmacy. Also responsible for periodic inventory and maintenance of on-site medication stock.
Responsible for assessment and entry of all medication orders for new enrollments, setting up pharmacy cycle, delivery system, assistive systems if needed, linking diagnosis information to medications (through H&P), and verifying all information in writing with participant's primary care provider. Also responsible for coordinating medications for disenrolling participants to provide needed medications for a period of twenty-eight (28) days after leaving program.
In coordination with medication technicians, responsible for filling and/or quality control of alternative medication delivery systems (MRB, Med-E-Lert, etc.) in accordance with medication orders and organizational policies/procedures.
Responsible for coordinating delivery of medications from pharmacy to participant either through direct carrier shipping or delivery through or from PACE center.
Adheres to organization policies in maintenance of documentation of delivery.
Responsible for making medication changes to currently filled delivery systems in accordance with medication orders and organizational policies/procedures.
Ensures that organization policies regarding the storage and handling of controlled substances are strictly adhered to. Conducts daily controlled substance inventories with another qualified staff member.
Communicates with participants and family members on a regular basis, addressing any questions or concerns related to participant medications.
In coordination with nursing staff, administers medications and vaccinations to participants in both the home and center setting in accordance with medication orders and organizational policies and procedures.
In coordination withmedication technicians, orders floor stock and OTC medications in accordance with organizational policies and procedures.
Effectively communicates in Care Team/Interdisciplinary meetings, clinic meetings, or other meetings as assigned.
Participates in nursing on-call rotation.
Actively participates and engages in the implementation of quality improvement initiatives.
Assists in the development and monitoring of quality metrics and indicators. Recommends and implements measures to support ongoing improvement of the quality, consistency, and continuity of participant care and PACE services.
Establishes and maintains accountability for high customer service standards and monitors, evaluates, and responds to opportunities for improved participant and family/caregiver satisfaction.
Manages and reviews participant's records, charts, and other pertinent information. Documents as needed in medical records according to Senior CommUnity Care standards.
Maintains the stability and reputation of SCC by ensuring all activities and operations are performed in compliance with local, State, and Federal laws, regulations, and contractual requirements.

Offer telephone and in-person patient education on therapy protocols, drug side effects, and home infusion procedures.

During initial and ongoing consultations, serve as a patient liaison between the patient, their family, and the pharmacy to ensure a seamless therapy process.
Perform comprehensive, initial, and periodic head-to-toe assessments for patients and initiate preventative and rehabilitative care.
Use health assessment data, and input from the agency team, the physician, the patient, and the family to determine patient needs.
Manage patient and family expectations regarding services, outcomes, and discharge goals.
Establish primary and secondary diagnoses based on patient assessment and home health care focus.

Qualifications
Graduate of an accredited nursing school with a current RN state license.
5+ years of experience in home infusion and infusion centers.
3+ Experience with IV Immunoglobulins and IV Biologics. CRNI certification(Certified Registered Nurse Infusion) is preferred.
Experience with peripheral IV starts, infusion therapy, and phlebotomy for both adult and pediatric patients is preferred
Effective interpersonal, time management, and organizational skills.
Strong written and verbal communication skills.

Proficient in computer skills, including word processing, and efficient use of the internet and email.

Act as a clinical resource, working collaboratively with other pharmacy departments.
Maintain knowledge of drug side effects and interactions.
Offer telephone and in-person patient education on therapy protocols, drug side effects, and home infusion procedures.
Ensure patients understand their therapy plans, medications, and the importance of adherence.
During initial and ongoing consultations, serve as a patient liaison between the patient, their family, and the pharmacy to ensure a seamless therapy process.
Perform comprehensive, initial, and periodic head-to-toe assessments for patients and initiate preventative and rehabilitative care.
Use health assessment data, and input from the agency team, the physician, the patient, and the family to determine patient needs.
Manage patient and family expectations regarding services, outcomes, and discharge goals.
Establish primary and secondary diagnoses based on patient assessment and home health care focus.
Develop care plans with skilled interventions, medical supplies, and necessary services to meet goals.
Regularly evaluate patient progress, in collaboration with team members, and revise care plans as needed.
Perform other duties as assigned.
Qualifications
Graduate of an accredited nursing school with a current RN state license.
5+ years of experience in home infusion and infusion centers.
3+ Experience with IV Immunoglobulins and IV Biologics. CRNI certification(Certified Registered Nurse Infusion) is preferred.
Experience with peripheral IV starts, infusion therapy, and phlebotomy for both adult and pediatric patients is preferred
Effective interpersonal, time management, and organizational skills.
Strong written and verbal communication skills.
Organizational ability to maintain consistently accurate records.
Ability to direct information to the appropriate healthcare provider.
Proficient in computer skills, including word processing, and efficient use of the internet and email.
Current CPR and BLS (Red Cross and AHA) certification, valid driver's license, reliable transportation, and automobile insurance.
