POSITION SUMMARY:

ESSENTIAL JOB FUNCTIONS:
Essential functions of the job include, but are not limited to, traveling, driving or having reliable transportation; written communication; relationship building with clients and community partners; utilizing a computer for typing and conducting research; attending meetings; and presenting to individuals, small and large groups.
MAJOR AREAS OF RESPONSIBILITIES:
Provide direct medical services to patients, including but not limited to comprehensive medical history, medical assessment, primary care, specialty care for transgender and gender nonconforming people, specialty HIV care, laboratory surveillance, and physical exam.
Prescribe or recommend pharmacologic and non-pharmacologic therapies including controlled substances and hormone therapy.
Facilitate direct patient admissions into hospitals when needed.
Play key role on multidisciplinary team to develop coordinated treatment plans.
Use primary care clinic protocols and procedures in providing services.
Consult with Collaborating/Supervising Physician regularly.
Educate patients on Transgender Health, HIV disease, primary care issues, and adherence strategies to medications prescribed.
Assess patient risk behaviors and educate patients in prevention of HIV transmission, provide HIV prevention therapy as needed.
Assist with the development and implementation of clinic policies and procedures, clinical protocols, continuous quality improvement standards and program monitoring.
Develop strong collaborative relationships with the trans and gender nonconforming community to enhance patient access to services including specialists and consultant care.
Continually update own knowledge of trans health and HIV/AIDS through reading, conference attendance, lectures and other educational opportunities.
Participate in quality efforts to increase health outcomes for patients.
Demonstrate unconditional positive regard to clients and conduct all aspects of job responsibilities with a focus on exceptional customer service.
Continuously grow and develop cultural competency, exhibiting an understanding, awareness, and respect for diversity. Practice cultural humility in daily interactions with staff, patients and community.

EDUCATION/LICENSURE:
Minimum of a Master's Degree in Nursing with board certification as a FNP; or Minimum of a Master's Degree in Physician Assistant Studies with board certification as a PA.
Active Ohio licensure as a Nurse Practitioner or Physician Assistant with prescriptive authority.
Federal DEA license
Certification in CPR/BLS
Knowledge, Skills, Abilities and other Qualifications:
Experience in Epic preferred.
2 years or more experience in providing primary care desired
2 years or more experience in providing HIV care desired.
Ability to work Saturdays and extended hours on an intermittent and rotating basis.
Exemplary customer service as well as written and verbal communication skills.
Knowledge of health and social service needs of persons with HIV/AIDS.
Must have sensitivity to, interest in and competence in cultural differences, transgender and gender non-conforming patients, HIV/AIDS, minority health, sexual practices, chemical dependency and a demonstrated competence in working with persons of color, and gay/lesbian/bisexual/transgender community.
Proficiency in all Microsoft Office applications and other computer applications required.

This includes collecting all pertinent patient information, providing thorough and accurate insurance verification, and processing prescription orders with detail, efficiency and accuracy. Make sure that all insurances are billed such that the patient has the lowest out of pocket cost. · Fill prescription orders accurately and efficiently. * Under the direction of the Program Manager for Mozaic, assist with grassroots advocacy work and related activities on issues relating to LGBTQ+ rights, healthcare policy, and other areas of the agency's public policy priorities
* Support and assist with coordinating grassroots mobilization opportunities - both in-person and online and under the agency and Mozaic brands - for members of the community and in coordination with the work being conducted by the legislative affairs team
* Draft - with support from departmental leadership and in coordination with the Development and Marketing teams, when applicable - program-specific updates (i.e., memos, donor communications, website content, social media posts, etc.) for Mozaic to engage community members, internal audiences, external stakeholders, donors, government officials, and others
* Coordinate and support events and projects, under both the agency and Mozaic brands, related to grassroots advocacy and other priorities - including access to care and related outreach - as assigned

* Direct attendees to Equitas Health services and/or the coalition partner resources, as interest is expressed
* Identify appropriate venue, communicate with venue partners, and determine optimal time and date for each clinic during the annual calendar
* Assist with drafting - with support from supervisor and in collaboration with staff from external partner organizations - the clinic's volunteer schedule by communicating with volunteer attorneys, and providing updates, as needed
* Monitor the clinic email inbox for general and volunteer attorney communications
* Support, alongside staff from external partner organizations, the coordination of updates to the clinic guidebook and the associated county quick-sheets, as necessary
* Provide notary services - if licensed as a notary public - to clients within residential county, as able
