Essential Duties and Responsibilities:
* PCC's Austin Recovery Connections (ARC) program supports patients with substance use disorders, including opiate use disorder and alcohol use disorder. Patients may participate in medication assisted recovery (MAR), behavioral health counseling, and behavioral health group visits. The Recovery Nurse Care Manager (RN) supports this clinic by increasing patients' self-management skills, reducing patient barriers to participation in CDC programs, providing administrative support, as well as supporting patient referrals to higher levels of care.
* The Recovery Nurse Care Manager (RN) shows dedication to treating substance use disorder in an interdisciplinary, outpatient setting. Provides therapeutic support to patients in clinic, in groups, and over the phone. The Physician - HIV Specialist is responsible for delivering high-quality, patient-centered primary care to individuals, including those living with or at risk of HIV. Position Summary
Senior Security Officers will be armed in accordance with Hospital Policy, State, and Federal statutes. Security Officers are responsible for ensuring the safety and security of patients, staff members, volunteers, visitors, students, equipment, supplies, and all assets necessary to provide comprehensive health care at COH, Chicago.
Key responsibilities include:
Responding to reported or observed acts of physical violence within the hospital.
Proactively addressing any overt active threats on the premises.

Department: Behavioral Health Manager Title: Behavioral Health Manager FLSA Status: Exempt Job Summary: The Behavioral Health Consultant provides behavioral health care services to patients in all life cycles. The Behavioral Health Consultant also participates with a multidisciplinary team to ensure holistic psychosocial and medical needs are met.
Essential Duties and Responsibilities:
* Maintain patient care hours per week at designated site as determined by the Behavioral Health Manager, Chief Population Health Officer, and the Chief Medical Officer. Follow PCCs policies for scheduling, access, chart documentation and availability for communication regarding clinical matters during regular business hours.
* Provide consultation and academic support to residents and attending physicians in the areas of behavioral medicine.
* Participate fully in relevant quality assurance and performance improvement measures.
* Provide mental health and substance abuse consultation regarding assessment and mental/behavioral health and substance abuse treatment options to established patients.
* Assess patients and/or families bio-psycho-social situations that result in diagnostic conclusions that include development issues, family dynamics and stressors, and a DSM V diagnosis.
* Document data, assessment, treatment plan and expected outcome in medical record.
* Identify and follow-up on all referrals you have made to assure continuity of care and patient/family needs are met.
* Perform any crisis intervention, individual counseling, family counseling, advocacy that is needed for the patient.

Requirements
MD or DO from accredited institution, licensed physician in Illinois.
Federal DEA license, Illinois Physician's license.
Board Eligible or Certified in Family Medicine.
Interest in and comfort with working with diverse patient population.
Bilingual preferred- Spanish.
