The Community Liaison works with Physicians, Discharge Planners, and professionals who handle the coordination and treatment of those seeking Homecare and Hospice services. Qualifications and Experience

• The Community Liaison is asked to keep accurate and relevant records, including call logs, Referrer Profile Sheets, and Visit notes.
• The Branch Director or Marketing Coordinator with the Community Liaison's input will set monthly individual and team goals and standards. The Community Liaison will participate and direct their efforts to achieving those goals.
• Maintain a continually evolving working knowledge of the territory, the demographic and referral source stats. This will include average number of referrals per year and type of referrals, ie home care, hospice, diabetic, continued independence. The census if hospital, SNF, ALF, or apartment complex, as well as general “who's who” with regards to administration, social worker, admission coordinator, service coordinator and clinical teams.
• A positive and forward-thinking attitude as well as being a team player is essential.
• Active participation in all sales calls and trainings along with an innovative approach to sales calls and group/individual sales blitzes is required.
• Adhere to all deadlines when pertaining to submission of action plans, monthly petty cash accounting, call logs and any other required documentation.
• Maintain professionalism, diplomacy, sensitivity, and tact to portray the company in a positive manner.
• Other duties as required.

The Community Liaison works with Physicians, Discharge Planners, and professionals who handle the coordination and treatment of those seeking Homecare and Hospice services. In addition, the Community Liaison assists in managing relationships with patients, caregivers, and families. The Community Liaison will educate themselves in many aspects of Home Health and Hospice including Medicare guidelines, Hospice/Homecare eligibility qualifications, basic medical terminology, and medical roles.
Qualifications and Experience
A minimum of one year experience in sales, within the Health Care industry or closely related field. A medical background or knowledge of Hospice is preferred but not required.
Knowledge of principles and methods for presenting, promoting, and selling products or services. This includes marketing strategy and tactics, sales techniques, and sales control systems.
An established presence in the community or territory assigned. The ideal candidate with have an existing network and relationship with potential referral sources.
A valid drivers license and current Auto Insurance
Independent transportation
Pass all criminal background checks
Essential Duties and Responsibilities:
Normal hours of work will be from 8:30 until 5 however special events and activities may occur outside of these hours on occasion. Every attempt will be made to make these hours up throughout the week.
The Community Liaison will see a predetermined average number of prospects per week, as determined by Branch Director or Marketing Coordinator and adjusted according to territory needs.
The Community Liaison is asked to keep accurate and relevant records, including call logs, Referrer Profile Sheets, and Visit notes.
The Branch Director or Marketing Coordinator with the Community Liaisons input will set monthly individual and team goals and standards. The Community Liaison will participate and direct their efforts to achieving those goals.
Maintain a continually evolving working knowledge of the territory, the demographic and referral source stats. This will include average number of referrals per year and type of referrals, ie home care, hospice, diabetic, continued independence. The census if hospital, SNF, ALF, or apartment complex, as well as general whos who with regards to administration, social worker, admission coordinator, service coordinator and clinical teams.
A positive and forward-thinking attitude as well as being a team player is essential.
Active participation in all sales calls and trainings along with an innovative approach to sales calls and group/individual sales blitzes is required.
Adhere to all deadlines when pertaining to submission of action plans, monthly petty cash accounting, call logs and any other required documentation.
Maintain professionalism, diplomacy, sensitivity, and tact to portray the company in a positive manner.

Job SummaryHome health Speech-Language Pathologists are central to the care team and maximize a patient's ability to safely remain in the least restrictive environment as their place of residence. Clinicians working in home health are on the front lines of health care and often practice at the top of their license. Responsibilities
Perform assessments of patient physical, functional, cognitive, and communication levels within the scope of Speech-Language Therapy
Develop a plan of care with patient goals
Provide evidence-based direct intervention with patients in their place of residence
Communicate effectively and respectfully with a diverse patient population and co-workers
Educate patients and family members about home programs, diet modifications, equipment, and available resources
Submit required documentation in a timely manner
Qualifications
Active State Speech-Language Pathologist License
Current CPR with BLS card
Valid driver's license, at least state minimum auto insurance, and an operational vehicle
Possess or obtain a good understanding of Federal, State, and local laws and regulatory guidelines governing home health operations
Great organization skills and an ability to work with minimal supervision
Capacity to appropriately supervise others
Ability to remain calm in emergency or crisis situations

Job SummaryWe are seeking a Registered Nurse to join our team. In this role, you will primarily be creating and managing a plan of care for individual patients. You are a Registered Nurse (RN) who operates with great attention to detail and a big heart. You take pride in creating personalized care plans for patients and are able to observe and communicate effectively in your patient's best interest. To succeed, you should have demonstrated knowledge and skill in current nursing practices as well as a strong penchant for solving problems.
This position is for Part Time or PRN shifts.
Responsibilities
Coordinate with other colleagues to assess, plan, implement, or evaluate patient care plans
Administer medications and monitor patients for side effects or reactions
Document any changes in patient conditions and report on changes
Maintain accurate records of patient care and discuss observations with the supervisor
Qualifications
Currently licensed Registered Nurse (RN) in state of employment
Previous experience as a Registered Nurse is preferred
Excellent interpersonal and client care skills
Valid driver's license
First aid/CPR certification required

Nursing with Harmony Home Health
As a Pediatric Home Health Nurse, you will be responsible for providing specialized nursing care to pediatric patients in the comfort of their homes. Your primary focus will be on promoting and maintaining the health and well-being of our patients who require medical attention due to chronic illnesses or disabilities. You will work closely with the patient's family, caregivers, and other healthcare professionals to ensure a patient-centered, compassionate and high quality approach to care.
Private duty nursing allows the nurse to focus care on one patient throughout the shift. Join our amazing team for a rewarding career in which you only have one patient. An RN or LPN working as a private duty nurse carries out orders including medication administrations, IV fluids, specialized feedings, and respiratory cares including trach and ventilator and oxygen management, assistance with ADLs, bathing, and feeding. Make a difference in a child's life by helping ensure they can stay home with their family.
Essential Duties and Responsibilities: · Work under the supervision of a Pediatric RN Case Manager.· Provide direct patient care as defined by the state Nurse Practice Act and provide services in accordance with agency policies. · Carry out the plan of care.· Recognize and use opportunities to teach/counsel health concepts to the patient and family regarding medication, treatment regimens and self-care techniques/activities. · Complete and submit necessary documentation in accordance with Harmony Policy and applicable regulations.· Evaluate outcomes of care and inform the physician and other staff of changes in the client's needs.· Maintain HIPAA.· Demonstrate effective time management and organizational skills. · Communicates the mission, ethics, and goals of Harmony Home Health and Hospice.
Qualifications and Experience:
· Graduate of an accredited school of nursing is a must. · Current Utah RN or LPN license.· Must be able to work with minimal supervision.· Must have and maintain a reliable automobile.· A criminal background check and drug screen must be performed upon hire.· Current TB test, CPR card, driver's license, and automobile insurance.

Job SummaryWe are looking for a Home Health Aide to join our team! You will directly work with patients, following a one-on-one care plan in patient homes. You operate with a big heart, not only providing personalized and attentive care but by building genuine relationships with those you serve. You are detail-oriented, highly organized, and committed to creating a healthy environment that meets patient needs. Strong verbal and written communication skills are needed to succeed in this role. Prior Home Health or Hospice experience is a plus.
Responsibilities
Assist patients with daily activities such as moving in and out of beds, baths, wheelchairs, or automobiles
Care for patients by changing bed linens, doing laundry, cleaning the home, or assisting with personal care
Maintain records of patient care, condition, progress, or problems to report and discuss observations with the supervisor
Build relationships with patients by conversing or reading aloud to help keep them mentally healthy and alert
Give medications and immunizations
Engage patients in exercises or other activities
Qualifications
Graduated from an accredited Home Health Aid program
High School Diploma or GED
One-year prior professional experience
Driver's license required
CPR certification required
