Provides triage, antepartum, intrapartum, and immediate postpartum care according to established protocols. Maintains active hospital privileges as requested.
Performs history and physical exams.

Supports Family Physicians and Certified Nurse Midwife members of assigned Delivery Teams.

Ensure the Outlook calendar is updated with daily tasks and/or travel in real time.
Maintain an active caseload of 20-25 with a maximum of 15 individual participants with demonstrated higher level of acuity or risk related to trauma, suicide risk, mental health, etc. and as assigned by Program Coordinator.
Attend any scheduled Clinical Team meetings within the assigned area for purposes of ongoing program development related to clinical efficacy, case review/staffing, and professional development
Facilitate groups for discretionary clients within designated LANs
Ensure all required crisis intervention and/or intake documentation is complete and accurate.
Responsible for maintaining active CCBYS client files with documents not exceeding more than 2 weeks past due
Responsible for updating and maintaining their designated LAN client, MRAI/referral, outreach, and quarterly success stories spreadsheet.
Monitor and supervise clients to ensure their safety and well-being during programming hours and transport.
Other Duties:
Assist the Community Liaison with the upkeep and development of community relationships
Assist the Community Liaison with planning, budgeting, and executing monthly client baskets and events, as well as delivering to clients or other LANs when necessary or assigned by the Program Coordinator.

Represents EFHC externally as assigned.

Qualifications
Skills and Knowledge

Licensed physician in Illinois.

SUMMARY
The purpose of the Community Support Specialist II position is to act as a stabilizer and provide assessment, counseling and service linkage to youth at risk for running away, being beyond the control of their parents and/or from becoming involved in the juvenile justice system. ESSENTIAL DUTIES AND RESPONSIBILITIES
Daily Job Duties:
Respond to all emails, texts, and phone calls from Program Coordinator within a 15-minute time frame.
Ensure the Outlook calendar updated with daily tasks and/or travel in real time.
Maintain an active caseload of 20-25 with a maximum of 15 individual participants with demonstrated higher level of acuity or risk related to trauma, suicide risk, mental health, etc. and as assigned by Program Coordinator.
Attend any scheduled Clinical Team meetings within assigned area for purposes of ongoing program development related to clinical efficacy, case review/staffing, and professional development
Facilitate groups for discretionary clients within designated LANs
Ensure all required crisis intervention and/or intake documentation is complete and accurate.
Responsible for maintaining active CCBYS client files with documents not exceeding more than 2 weeks past due
Responsible for updating and maintaining their designated LAN client, MRAI/referral, outreach, and quarterly success stories spreadsheet.
Monitor and supervise clients to ensure their safety and well-being during programming hours and transport.
Other Duties:
Assist the Community Liaison with the upkeep and development of community relationships
Assist the Community Liaison with planning, budgeting, and executing monthly client baskets and events, as well as, delivering to clients or other LAN's; when necessary or assigned by Program Coordinator.
Assist the Community Liaison in planning and executing CCBYS all staff events and assigned lunch and learn presentations; when necessary or assigned by Program Coordinator.
Assist the Community Liaison with delivering files to the correct LAN or Program Coordinator for monthly division audits and level audits; when necessary or assigned by Program Coordinator.
Assist the Community Liaison with in coordinating and attend annual community outreach meetings; when necessary or assigned by Program Coordinator.
Obtain Letters of Support (LOS) and Memorandums of Understanding (MOU) from community partners annually.
Participate in individual, group supervision, team meetings, program staff meetings, and trainings as scheduled to address client issues and revised documentation, placement options, crisis schedules, etc.
Provide required documentation (i.e., mileage, time sheets, CQIs, surveys, etc.) as indicated by agency protocols.
Complete all necessary paperwork and documentation, i.e., timesheets, vouchers, progress notes, incident reports, etc., and submit in a timely manner.

Key Responsibilities
Provide weekly in-person contact with assigned families to assess needs and support goals.

Maintain consistent communication with families and obtain all necessary consent/release forms.
Provide transportation to essential appointments (e.g., medical visits, WIC, community services).
Offer parent coaching, household budgeting support, and homemaker-type assistance (e.g., cleaning, daily household routines).
Monitor and support healthy parent-child interactions.
Respond to family crises as needed, including after-hours situations.

SUMMARY
Our client is seeking an On-Call Mobile Crisis Responder to provide immediate crisis intervention services to youth under 21 who are experiencing a mental health emergency that may pose a danger to themselves or others. This position is critical in stabilizing crisis situations through rapid in-person response and short-term service linkage.
RESPONSIBILITIES
Serve as an on-call responder for Designated Service Area (DSA) #25.
Be immediately available during scheduled on-call shifts to respond to crisis referrals for youth under the SASS program, as well as Medicaid adults, MCO members, and Pathways clients.
Accept all crisis referrals from the CARES line within 30 minutes-no-decline policy applies.
Respond in-person to all Mobile Crisis Response (MCR) requests within 90 minutes of receiving the referral.
Complete crisis screening and assessment using the Illinois Medicaid Crisis Assessment Tool (IM-CAT).
Provide immediate short-term follow-up, transition planning, and linkage to necessary services.
Complete MCR intake documentation at the point of crisis and ensure all records are accurate and timely.
Facilitate safe outcomes such as family reunification, temporary placement, or psychiatric hospitalization.
Coordinate referrals and follow-ups based on client and family needs.
Remain reachable via mobile phone during on-call hours; maintain a home phone line as a backup contact method.
Submit all required administrative documentation (e.g., mileage logs, timesheets) per agency protocols.
Attend supervision, team meetings, and ongoing training as required.
Adhere to mandated reporting requirements and agency policies.
Provide on-call crisis support as directed by the supervisor.
Perform additional duties as assigned.
