Essential Duties and Responsibilities:
Represent the CNO, and the hospital in a professional and congenial manner in person, in telephone conversations and in written correspondence.
Composes letters and statements independently requiring application of hospital policies, rules and regulations and monitors follow-up for all distributions that require responses.
Primary contact for department managers, staff and outside parties as it relates to the COO and CMO and hospital.
Manages projects for the Executive Team and Leadership Team.
Screens telephone calls and visitors, ascertaining who can be redirected to subordinate managers or other offices.
Maintains files of administrative and hospital matters that are extremely confidential in nature. Such as, hospital contracts, regulatory compliance, physician complaints, joint commission, IDPH, etc.
Prepares documentation for the CNO for all hospital related events, where error could have significant effects on cost, such as, typing, proofing and assembling reports or proposals.
Takes and reviews minutes during highly confidential meetings that would have negative impact on the hospital if shared beyond its intended audience.
Makes phone calls on behalf of the CNO when instructed.
Helps resolve patient and employee disputes when necessary by contacting the appropriate hospital personnel.
Serves as a resource for the hospital.
Makes travel arrangements for hospital related functions.
Assists CNO with acknowledging gifts and contributions to the hospital and in performance of employee relation functions. Such as, sending flowers in case of illness or death, sending out cards, etc.

This position is a joint position between Humboldt Park Health and The West Side Health Equity Collaborative (WSHEC) an initiative on Chicago's west side that uses a comprehensive network of community-based partners and medical providers to screen community members with chronic illness for social determinants of health and provide a suite of comprehensive services to resolve identified issues. Based in the Emergency Department, the HPH/WSHEC Behavioral Health Department (BH) Navigator will screen HPH patients in the BH. The BH Navigator will serve as a bridge between the patient, the medical system, and community-based organizations by building trusting relationship with community members served by the program. The BH Navigator will also help in identifying barriers to accessing quality care, work with individuals to overcome these barriers, provide relevant referrals
Essential Duties and Responsibilities:
Patient Identification
Assist with identification of patients with SUD or co-occurring mental health disorders in the emergency department (ED) and, where feasible, within inpatient units by monitoring patient tracking systems to screen for eligible patients and checking in with clinicians and nursing staff to receive referrals of eligible patients.
Establish a positive relationship with patients struggling with drug use or co-occurring mental health disorders. If required by the hospital, to allow for billing, this may include initial patient assessments and brief interventions using standardized tools.
Make navigator contact information widely available to people who use drugs, patients with co-occurring mental health disorders, and clinicians; respond to calls or texts directly from patients and providers.
Advocate for a culture of low threshold access to MAT for patients with opioid use disorder (OUD) that includes signage or materials inviting patients to seek help for substance use in prominent areas of the ED and hospital.
Patient Engagement in Treatment
Facilitate initiation of MAT with hospital clinicians.
Use motivational interviewing techniques to communicate with patients in a respectful, culturally appropriate, non-judgmental manner.
Maintain up-to-date information about the effects of various substances, withdrawal symptoms, and treatment options to effectively educate and counsel patients.
Promote harm-reduction strategies based on patients' goals, preferences, and life circumstances.
Engage patients with co-occurring mental health disorders and help them access treatment.
Follow-Up Care Navigation
Help patients overcome barriers to filling prescriptions for MAT (e.g., insurance status, copay expense, cost differences between formulations, etc.)
Schedule appointments at MAT-capable clinics for ongoing treatment and address access barriers by assisting with transportation, retrieving medical records, providing cell phones, or other supports as determined by patient needs and community resources.
Establish a relationship with patients and communicate via telephone, text, and/or email to remind patients of appointments, help navigate obstacles to follow-up treatment, and provide encouragement.
Work with hospital staff to set up a robust system for ensuring patient referral and follow-up outside of the navigator's regular hours.
Develop expertise in insurance benefits and exclusions related to treatment.
Provide patients referrals to other services, such as mental health services, shelter, primary care, social services, and residential treatment facilities.
Assist out-of-county patients to access MAT and other services in their home county.

Obtains clinical, demographic and insurance information from the patient, guarantor, relative or police through an interview either over the phone or in person, or from documentation presented at the time of registration; correctly enters all information into the computer system.

Job Description

Contributes to the long-range positioning strategy of the organization.
Enhances and promotes client and patient satisfaction in all departmental areas.
Qualifications:
Minimum 5 - 7 years of experience
Behavioral health and leadership/management experience is required
Bachelor's degree in Nursing, Master's degree is highly desired

Job Description

Assists in data collection to identify the patient's needs, problems, concerns or human responses, i.e. Vital signs.
Assisting, under the direction of the GI Registered Nurse, in the implementation of the established plan of care.
Communicates appropriately and clearly to charge nurse, co-workers and physicians, identifies and addresses psychosocial needs of patients and family, and manages and operate equipment safely and correctly.
Consults other departments as needed and as appropriate to provide for an interdisciplinary approach to the patient's needs.
Assisting the physician and GI Registered Nurse after, during and after diagnostic and therapeutic procedures.
Providing and maintaining a safe environment for the patient and staff by complying with regulatory agency requirements and standards set forth by professional organizations and employers.
Complies with ethical, professional and legal standards inherent in patient care and professional conduct, e.g., patient's bill of rights.
Participates in quality management activities as directed, understands practice issues and continues to stay educated in the field of gastroenterology .
Collaborates with the gastroenterology team and other healthcare professionals to ensure quality and continuity of care.
Follows regulatory bodies and the SGNA on the proper method to clean, disinfect and store endoscopes.

Summary:
The Chief Hospitalist will provide inpatient clinical care to patients who are referred to the organization through assigned and unassigned admissions from medical staff members or other sources. The CH will be aware of and provide care to admitted patients on an ongoing basis and ensure that discharges are handled in a timely manner in accordance the policy guidelines and federal regulations. The CH will serve as a clinical team lead for all codes, rapid responses, and other emergent clinical scenarios while working to ensure optimal team performance and adherence to all relevant guidelines. The scope of care will include adult internal medicine, pediatric, critical care, and behavioral health.
Additional Duties:
Provide oversight to all admitted patients to ensure they are cared for on an ongoing basis and discharged in a timely manner in accordance to policy and regulatory guidelines
Actively participate in activities to improve quality and patient satisfaction such as medical staffing committees, performance improvement projects, peer review, QA meetings, and overall use and performance of guidelines / protocols
Regularly coordinate with both the clinical and administrative leadership teams to ensure high quality and effective patient care
Teach and mentor clinical staff on an ongoing basis to ensure they are educated on the best evidence based guidelines when providing care
Communicates regularly with referring physicians, specialists, administration, patients, and other hospital departments regarding the care and treatment provided to patients
