Transform Hospice Care.** We are seeking a dedicated Hospice Nurse Practitioner to perform Face-to-Face (F2F) encounters essential for hospice eligibility certification and recertification. In this role, you will gather comprehensive clinical information, perform detailed assessments, and collaborate closely with hospice physicians and interdisciplinary teams to ensure quality patient care and regulatory compliance.
**Key Responsibilities:**
+ Perform and complete physical assessments for hospice F2F encounters within CMS-established regulatory timeframes.
+ Document clinical findings accurately and attest F2F encounters in patient medical records.
+ Utilize Local Coverage Determination (LCD) criteria per hospice policy and regulatory requirements.
+ Communicate clinical findings, review treatment plans, lab results, diagnostics, and medications with certifying physicians.
+ Collaborate effectively with physicians, interdisciplinary group (IDG) members, patients, caregivers, and families.
+ Provide clinical recommendations to the IDG based on F2F assessment findings.
+ Conduct assessments in the patient's place of residence, including home, skilled nursing, or other settings.
+ Notify the hospice agency promptly if unable to complete F2F visits as scheduled.
+ May provide F2F coverage for multiple provider numbers with approval.
**About You**

+ Master's or higher degree in Nursing
+ Completion of an accredited graduate-level nurse practitioner education program
+ Current registered nurse license in state of practice

Company: Providence at Home with Compassus The Home Health Aide is a trained paraprofessional who performs basic nursing or therapy functions, such as personal care, prescribed therapeutic exercises and household services essential to providing health care at home following a specific written plan of care. The Registered Nurse Clinical Case Manager supervises the Home Health Aide and the Home Health Aide plan of care in the home. If there is not an RN on the case, then a Physical Therapist, Occupational Therapist, or Speech Language Pathologist supervises the HHA and the POC for that case.
Position Specific Responsibilities
* Provides competent personal care as outlined on the Home Health Aide Plan of Care established by the patient, family/caregiver and case manager.
* Care provided in a safe, comprehensive fashion, treating each patient with dignity and respect while maintaining confidentiality.
* Performs basic nursing care as directed and instructed by the supervising nurse including taking and recording vital signs and temperature, performing simple dressing changes, assisting with routine bowel care.
* Provides personal care, including, but not limited to, bath assist, oral and denture care, skin, hair and skin, shaving, and nail trimming.
* Helps patient to use the bathroom or in using bedpan or commode.
* Provides care of surgical or prosthetic appliances or devices as instructed, i.e., catheters, colostomies, leg braces, dentures, etc.
* Assists with prescribed exercises which patient and aide have been taught by the Physical or Occupational Therapist or Registered Nurse and as described in the Home Health Aide plan of care.
* Assists patient in bed with positioning, assists patients in and out of bed, and assists with transfers and ambulation.
* Assists patients as per agency procedure with medications that are ordinarily self-administered by the patient under the direction of the Registered Nurse.
* Performs light housekeeping, which facilitate the patient's self care at home and which are necessary to prevent or postpone institutionalization and are essential to the patient's health care recovery at home.
* Observes and reports changes in condition, body functions, and needs to the professional responsible for patients care.
* Completes appropriate records and documentation of the status and the care and/or services furnished.
* Works collaboratively with interdisciplinary team to develop, implement, and update plan of care/treatment.

How you make a difference
An Emergency Medical Technician (EMT) provides essential medical care to inmate patients within correctional facilities. This includes conducting initial medical examinations upon in-take and responding to day-to-day medical issues that may arise among the inmate population. The Hospice Admissions Coordinator processes patient referrals, coordinates the admission of patients, and enters patient admission data after all requirements of hospice have been met.
Position Specific Responsibilities
Attends and participates in staff meetings, educational programs and community events, as requested.
Communicates identified needs and potential solutions to supervisor.
Receives and processes referrals from physicians, patients, families, friends, and community.
Contacts physicians to secure an order for admission. If the Admissions Coordinator is not a Registered Nurse, s/he will have to ensure a nurse is present to take a verbal order or will have to ask for a “script” to be faxed.
Contacts insurance companies for pre-certification authorization.
Schedules appointments with patients and families for informed consents and the RN assessment visit.
Explains hospice services and benefits to patients, family members, and significant others, if needed.
Informs appropriate team members of time of appointments.
Completes admission process according to agency/corporate policy.
Ensures proper notification of admission to assigned interdisciplinary team (IDT) and ancillary providers.
Enters admission data into a third-party system.
Ensures patient chart, ensuring all documentation is signed and dated and turns chart over to Team Coordinator.
Markets referral sources.
Contacts long term care staff, hospital staff, and physicians to promote hospice services.
Provides back-up support for Hospice Care Consultant.
Documents marketing contacts, as directed.
Performs other duties as assigned.
Education and/or Experience
High school diploma or GED required.
Bachelor's degree preferred.
Two (2) to three (3) years of related experience and/or training strongly preferred. An equivalent combination of education and experience will be considered.
Experience communicating with physicians, nurses and other healthcare personnel highly preferred.
Experience in insurance and Medicare/Medicaid billing and reimbursement preferred.
Skills
Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.
Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. + Communicate clinical findings, review treatment plans, lab results, diagnostics, and medications with certifying physicians.
+ Collaborate effectively with physicians, interdisciplinary group (IDG) members, patients, caregivers, and families.
+ Provide clinical recommendations to the IDG based on F2F assessment findings.
+ Conduct assessments in the patient's place of residence, including home, skilled nursing, or other settings.
+ Notify the hospice agency promptly if unable to complete F2F visits as scheduled.
+ May provide F2F coverage for multiple provider numbers with approval.
About You
