In this role, you will ensure that intake and referral processes are completed efficiently and effectively to meet the needs of patients, families, referral sources, and the organization. You will play a vital part in building strong professional relationships, coordinating admissions, and supporting community awareness of hospice services. **This is a WEEKEND full-time position. In this role, you will ensure that intake and referral processes are completed efficiently and effectively to meet the needs of patients, families, referral sources, and the organization.
