**Key Responsibilities**
+ Supervise all volunteer activity within the designated service area.
+ Maintain compliance with the Medicare Hospice Conditions of Participation 5% volunteer requirement.
+ Assess patient and family needs for volunteer services and coordinate appropriate placements.
+ Recruit, train, and retain Administrative, Direct Care, Vigil, and Bereavement volunteers.

Reporting directly to the Executive Director or Administrator, you will play a vital role in maintaining a high-performing hospice program by managing patient care activities, supervising clinical staff, and ensuring compliance with all company policies and regulatory standards.
**As a Senior Hospice RN Patient Care Manager, you will:**
+ Lead and support hospice patient care operations to ensure quality and compliance
+ Supervise, teach, evaluate, and develop nursing and clinical associates
+ Manage clinical and administrative workflows, including referrals, DME pickups, Medicare eligibility, insurance verification, and patient benefit tracking
+ Coordinate and facilitate Interdisciplinary Group (IDG) meetings to review patient care plans and update clinical documentation
+ Oversee scheduling, visit assignments, and timely completion of patient visits
+ Respond compassionately and professionally to referrals and clinical inquiries
+ Serve as a liaison between clinical teams, leadership, and external providers
+ Ensure adherence to hospice industry regulations including Medicare, Medicaid, JCAHO, ACHC
**About You**
**Qualifications - What You'll Bring:**
+ Current Registered Nurse (RN) license in the state of employment
+ Graduate of an accredited nursing program (Associate degree accepted with relevant experience)
+ Minimum 3 years direct patient care experience
+ Knowledge of hospice principles, regulatory compliance, and managed care
+ Strong communication, collaboration, and organizational skills
+ Proficiency with Electronic Medical Records (EMR), Microsoft Office, and healthcare software
+ Valid driver's license and automobile insurance
+ Ability to provide skilled nursing care as needed, including on-call responsibilities
+ CPR certification

+ Bachelor's degree in Nursing
+ 2+ years hospice or home care experience
+ Leadership experience in clinical supervision or program management

Responsibilities
* Communicate significant findings, problems, changes in condition or environment to the clinical manager/attending physician and other members of the interdisciplinary team.
* Complete face to face encounters and assesses the following: disease related changes, decline in function, adherence and effectiveness of pain and symptom regimen, the patient and family coping mechanisms and quality of life.
* Order and review appropriate lab, x-rays, EKG's and other diagnostic procedures for palliation and treatment of the terminal disease as dictated by the patient's condition and documents the treatment plan accordingly and reviews results with collaborating physician.
* Make referrals to other disciplines as indicated by the patient's needs.
* Actively participate in interdisciplinary team conferences.
Qualifications
* Current and unencumbered license as a Nurse Practitioner in the state the employee is assigned to work by the Company.
* Minimum of one (1) year experience as a Nurse Practitioner or minimum of one (1) year experience as a hospice Registered Nurse in last three (3) years.
* Must have the authority to practice as a nurse practitioner in the state where services are provided.
* Must have the authority to prescribe as a nurse practitioner in the state where services are provided.
* Must have obtained Medicare billing privileges as a nurse practitioner.
* Must meet physician-nurse practitioner restrictive collaborative requirements specific to state assigned to work by the Company and supply a copy of all active collaborating physician agreements and indicate whether these will be transferable to your employment with Amedisys.
* Must have insurance required by state law for nurse practitioners to practice [and prescribe] in the state where services are provided or be eligible for such insurance requirements to be satisfied by group coverage with an employer.

What You'll Do as a Hospice Aide:

+ Observe and report changes in the patient's physical or mental condition

About You
Qualifications - What You'll Bring:
