We are seeking a highly skilled, experienced, accountable, and self-motivated home health Registered Nurse (RN) to join our agency and serve existing and returning clients throughout southeastern Ohio, primarily in Jackson, Vinton, Pike, and Gallia Counties. This position requires travel to several counties but offers a consistent schedule to fit your personal / family commitments (no overnight shifts or travel). With core skills in managed care, patient assessments and compliance, client care and treatment plan documentation, and discharge planning, you will oversee patient services and provide expert nursing care. You will submit updated documentation of the patient's condition and ongoing care needs timely and within compliance of all state, federal, and agency guidelines. You will ensure the successful communication with and between our physicians, our agency staff, service administrators, other caregivers and clinicians, and the client + his/her family.
