PrimeCare Medical is currently looking for a Licensed Professional Counselor (LPC) or Licensed Social Worker (LSW or LCSW) to work as a Behavioral Health Professional in the medical department at the Montgomery County Correctional Facility in Eagleville, PA.
This a 40-hour position working Monday - Friday, during normal business hours (8:00am-4:30pm).
*Please note; Saturday shift or evening weekday coverage may be required as needed with advanced notice given.*
Job Responsibilities:
Conduct clinical assessments based on referrals and self-referrals. Assessments include documentation of findings, including at a minimum a mental status examination and case conceptualization, diagnosis, and treatment plan. Makes referrals to psychiatry when appropriate.
Risk assessments are to be conducted utilizing structured interviews and risk assessment tools. Inmates at risk for suicide and / or engaging in self-harm are managed utilizing existing protocols according to current policy and procedures.
Serve as the PrimeCare Medical (PCM) mental health liaison for mental health services, outside the facility.
Facilitates admission and placement of inmates/patients to forensic hospitals when necessary.
Monitors all mental health services provided.
Monitors clinical services to ensure that standards of care are in accordance with NCCHC, and accepted psychiatric/ psychological standards.
Is a member of the Quality Assurance Committee.
Develops and conducts in-service education programs according to annual in-service calendar.
Collects data and prepares monthly reports, which reflect performance.

Position Details
Family Based Therapist (FBT) works as one half of a team which provides treatment services to families including therapy and counseling to child and family. FBT travels to client homes, other agencies, and community resources for the purpose of arranging and providing community and home-based services for children, adolescents, and families.

The Role:
The Brothers of Charity Services Ireland Limerick Region provide a wide range of services to people with an intellectual disability and their families in Limerick city and county. The services range from Progressing Disabilities Services for children and a variety of programmes for adults devised according to the level of support that individuals require and priorities that they identify. Services for adults include day, residential, respite, a range of multidisciplinary supports and supports to people who live independently.
Reporting/Responsible To: Director of Services Limerick Region
Qualifications:
Candidates must have a MSW or CQSW or a Letter of Accreditation issued by the National Social Work Qualifications Board (NSWQB). Candidates must be registered with CORU.
Experience:
Candidates must have at least 3 years work experience as a Senior Social Worker or Social Work Team Leader preferably in services for people with intellectual disabilities.
Demonstrated experience in managing a team is essential.
Skills:
Candidates must demonstrate clear leadership and management abilities, be able to communicate effectively and to both lead, manage and work as part of a team. Candidates should demonstrate organisational skills together with up-to-date knowledge of current thinking and practice in services for people with learning disabilities, be highly motivated, flexible and committed to working as part of a team. Candidates must demonstrate a knowledge and understanding of the regulatory environment as it relates to the provision of Services to people who have intellectual disability.
Working Hours: 35 hours per week Mon-Fri. Flexibility will be required and in order to meet service demands the Head of Social Work may be contacted outside of these hours in the event of an emergency.
Annual Leave: 30 days per annum

This post is permanent, full-time and pensionable.
Probation: A probationary period of nine months from the date of appointment applies to the post. The employment may be terminated at any time during the probationary period should the employer find that the appointee is unsuitable to continue employment. The probation period may be extended at the Employer's discretion.

Main duties and functions:
· To manage and lead the Limerick Region Social Work team in fulfilling its agreed functions and its ongoing development.
· To promote and support team development.
· To liaise with Heads of Social Work colleagues in other regions of the Brothers of Charity Services and co-operate with the Brothers of Charity Services National Office as required.
· To support the Designated Officer and the Senior Management team in developing a culture of safeguarding with the BOCSILR.
· To keep up to date with developments in respect to safeguarding matters, findings of regulatory inspections and to ensure consistent approaches to safeguarding across the organisation.
· To liaise with other senior personnel of other disciplines in ensuring the effectiveness of the multidisciplinary team.
· To provide a leadership role in the role out of ADMA within BOCSILR.
· To plan, organise, direct and co-ordinate the development, evaluation and management of the Social Work Service subject to the general direction of the Director of Services
· To maintain up-to-date records and reports in accordance with the principles of the social work profession and provide relevant information and reports when required.
· To manage the Social Work departmental budget
· To carry a social work caseload in the Adult Services as required.
· To organise the provision if required of social work services to any area of the Brothers of Charity Services Ireland Limerick Region
· To promote appropriate service innovations
· To ensure and plan for the provision of efficient and effective social work service to people supported and their families through the establishment of clear team goals within the social work team and the appropriate organisation of day-to-day operations carried out by social workers
· To co-ordinate social work team activities, organise and chair team meetings.
· To co-ordinate following through on matters arising and decisions taken at team meetings
· To promote adherence to the Brothers of Charity Services Ireland Policies and Procedures
· To document emerging trends and service needs and keep management informed of these trends.
· To co-ordinate ongoing development plans for the service and advocate for resources and identify unmet needs.
· To promote the Brothers of Charity Services Ireland commitment to community services and to support a network of community partnerships.
· To ensure the rights of people supported and their families are respected
· To provide supervision to the Social Work Team in the Brothers of Charity Services Ireland Limerick Region.
· Chair the Assisted Decision Making (Capacity) Act Regional Committee and issue a plan of work on an annual basis.
· Member of the Admissions Management Team.
· To support and play a leadership role in the co-ordination and development of the Human Rights Committee (HRC)
· Deputise for the Designated Officers as required.
· To attend regular supervision meetings with the Director of Services.
· To set clear goals for the team and review individual performance regularly, give constructive and specific performance feedback, and coach and develop individual staff.
· To ensure that issues that arise under the grievance and disciplinary procedure are dealt with appropriately, promptly and reported promptly to Human Resources and senior management.
· To participate as required in selection and recruitment of social work staff
· To initiate, participate and co-operate in relevant research.
· To facilitate and encourage opportunities on the data collection and training.
· To liaise with the HSE and other relevant statutory and voluntary organizations
· To contribute to the development of policies and procedures in line with good practice within the department.
· To be conscious of health and safety matters in the workplace and, in particular, to comply with employees' obligations as set out under the Safety, Health & Welfare at Work Act, 2005. To ensure, as far as possible, the safety of people supported and to report any potential hazards.
· To ensure that all accidents/incidents to people supported, staff and visitors are reported promptly to the person in charge/deputy.
· To operate within the policies and procedures of the Service at all times.
· To promote dignity at work and show respect to people supported, colleagues and stakeholders in the course of duty.
· To personally co-operate and engage with all mandatory training and ensure the participation of the social work staff in mandatory training.
· To perform such other duties assigned from time to time by the Director of Services. All duties must be carried out in person at all times.
· As the duties and responsibilities of any post in the Service are likely to change with the ongoing needs of the people supported and service development, staff are expected to have a high level of flexibility, a willingness and an ability to develop new approaches to their work as the service needs demand.
The role of the post holder will not be limited by reference to this Job Description. It would be expected that the role will evolve as professional, structural and people supported demands change and the post holder will be expected to demonstrate flexibility skills that will facilitate this.
