**Job Description:**
+ Provides grief counseling, disease adjustment support, crisis intervention, goals of care planning support, and de-escalation services for patients as appropriate.
+ Assesses patients' and families' wholistically for discharge planning needs in the inpatient, observation and/or emergency departments, including prior functioning, support systems, financial, and psychosocial in a timely fashion to avoid delays in discharge planning.
+ Reviews the medical record, including medications, history and physical, labs, and progress notes and incorporates the clinical, social, and financial factors into the transition of care plan.
+ Develops discharge plans with appropriate contingency plans throughout the hospital stay to ensure timely care coordination and progression of care, making arrangements for post-acute care services and facilities as well as community care for social needs.
+ Leverages technology and follows standard work and best practices to communicate with post-acute care services and facilities to ensure patient care information is communicated for continuity of care, medical records are complete, and discharge reconciliation is accurate.

Job Summary:
Northwestern University Mesulam Center for Cognitive Neurology and Alzheimer's Disease (Mesulam Center) is seeking an exceptional socialworker who demonstrates a pronounced interest in working with persons with neurocognitive disorders and their families in a diversity of roles. The SocialWorker will work as a team member to provide clinical care for persons living with dementia and their families and achieve the goals and objectives of the Outreach, Recruitment and Community Engagement (ORE) Core, the Miller Family Buddy Program, in addition to other psychosocial initiatives.
Specifically, the SocialWorker will provide clinical social work services for persons living with dementia and their caregiving families in the Neurobehavior and Memory Clinic of Northwestern Medical Group; contribute to the recruitment and retention of research participants that meet Center research priorities; manage and facilitate monthly care partner support groups; and participate in the coordination and planning of an annual scientific conference. In addition, they will participate in the preparation and review of annual progress reports and competitive renewal for the NIA and other funding agencies.
The Mesulam Center is a multi-departmental component of the Feinberg School of Medicine. We specialize in the clinical care and scientific study of neurological diseases that interfere with cognition and behavior. Areas of emphasis include age-related memory, language-based and behavioral dementias. We are focused on Alzheimer's disease, Primary Progressive Aphasia and Frontotemporal Dementia. Our three interacting missions are to investigate the causes and clinical characteristics of neurodegenerative diseases, to ensure that our patients and families are the beneficiaries of the latest research advances, and to prepare future scientists and clinicians in our field.
Specific Responsibilities:
* Provide clinical social work services to patients and families of the Northwestern Medicine Neurobehavior and Memory Clinic and center research participants. Complete psychosocial assessments and provide education, counseling and linkage to appropriate community services. Collaborate with multi-disciplinary team of behavioral neurologists, neuropsychologists, psychiatrists, and other clinicians through weekly clinical rounds and patient care discussions.

In addition, treatment efforts are coordinated with other institution and unit staff, parole agents, family members and/or community resources.
A SocialWorker Credential from the Wisconsin DSPS is required to be considered for interview.

Pay will be set in accordance with the Compensation Plan and Wisconsin Administrative Code in effect at the time of hire. A probationary period is required.
Candidates with no prior permanent employment with the State of Wisconsin in the past 5 years may be eligible to accept a sign-on bonus of $3,000. The sign-on bonus will be paid in two installments: $1500 on the first paycheck and $1500 with the paycheck for the pay period in which the employee attains permanent status (usually 1 year). (UW System employment isn't disqualifying.)
To receive the bonus, a candidate must sign an agreement indicating they will remain in that occupational area for a year. If the candidate leaves their occupational area prior to completing the one year, they may be required to reimburse the initial payment.

Part\-Time School SocialWorker - Illinois | January-June 2026 A local Illinois district has partnered with Clarifi Staffing Solutions to bring on a dedicated School SocialWorker for a part\-time assignment through the second half of the school year. This opportunity is well\-suited for a licensed professional who values meaningful student connections and prefers a reduced weekly schedule.
From January 5 to June 3, the selected candidate will be on campus two days each week, supporting students during the regular school day (8:30 a.m.-3:30 p.m.). The socialworker will play an important role in addressing social\-emotional needs, offering short\-term counseling, and working collaboratively with school staff and families to strengthen student engagement and overall well\-being.
Job Snapshot:
Provide direct social work services to students.
Participate in IEP and 504 meetings as appropriate.
Offer individual and small\-group social\-emotional support.
Assist with implementing behavioral interventions and strategies.
Help students develop coping skills and strengthen social\-emotional functioning.
Collaborate with teachers, administrators, and families to support student success.
Contribute to a positive, inclusive school environment where all students feel supported.
Requirements School\-based experience is highly desirable and will enhance a candidate's suitability for the position. Applicants should have strong communication skills and the ability to collaborate effectively with a multidisciplinary team. A Master's degree in Social Work (MSW) from an accredited institution is required, and candidates must hold a valid Illinois Professional Educator License (PEL) with the School SocialWorker endorsement.
