SUMMARY: The Qualified Mental Health Specialist (QMHS) is a community-based, mobile individual responsible for providing Community Psychiatric Supportive Treatment (CPST) and Therapeutic Behavioral Support (TBS) services that address the individualized mental health needs of clients. Coordinates the referral of patients and families to appropriate community, social service, home health service, extended care, and rehabilitation facilities, and organizations. Creates appropriate discharge plans and documents information in the medical record. Assists in identifying opportunities for improvement, participates in data collection and analysis, contributes to problem solving and identification of actions to be taken and effectiveness of those actions. This role is designed to provide direct care and other related technical and clinical support activities that align with Empowering to Elevate Mission, Vision & Values. Reports To: Clinical Director or Clinical Supervisor Essential Tasks, Duties, and Responsibilities: Responsible for the clinical management of his/her assigned clients. Assists in identification of treatment/contract goals, completing the initial psychosocial assessment. Responsible for maintaining contact with the assigned client's referring/outpatient therapist during the course of the client's stay and the family when/if appropriate. Completes and updates treatment plan for assigned clients. Meets with clients' frequencies according to their treatment plans, to help explore identified issues and behaviors in relationship to current symptoms. Leads and participates in group meetings, meals and other activities in the milieu. Provides comprehensive understanding of current behaviors and relational dynamics while using a variety of therapeutic modalities. Attends the weekly team meetings and share observations, comments and important information about clients with the treatment team and provides input into the development of the weekly schedules for all of his/her assigned clients.
