Job Summary: The Licensed Clinical SocialWorker (LCSW)/ Licensed Marriage and Family Therapist (LMFT) is an integral part of the clinical care team providing behavioral health services to the patients and families served by PCHC. Coordinates patient care services for patients across the continuum of care through collaboration with the patient.
Participates in our patient-centered medical home, collaborating with medical providers in developing a standard of care. The LCSW/LMFT provides baseline mental health assessments, counseling, crisis intervention and referrals for psychiatric evaluation and other community resources when needed.
Essential Duties:

* Interfaces with other care team members, and maintains open communication, collaboration and care coordination with Providers.
* Promotes the principles of a Patient Centered Medical Home.

Job Qualifications and Skills:

Requirements
Essential Duties and Responsibilities of the clinician include the following:
1. Maintain complete and current licensure (or pre-licensure associate/trainee status) with the State of California BBS and associate/trainee requirements.
2. Maintain personal malpractice insurance covering all services provided at LSSSC and provide proof of current insurance to HR at the time of hire and upon expiration.
3. Provide Individual Services, Therapy, Group Therapy, and classes as assigned according to Clinical Soundness and Best Practices guidelines.
4. Produce and maintain all clinical records as required by contract.
5. For DBH, this includes but is not limited to:
Thorough understanding and implementation of the guidelines outlined in the Standards and Practices Manual and Outpatient Charting Manual.
Accurate entry of services into the Clinical Record and the Electronic Health Record (EHR) System within given time frames.
Accurate reporting of billing based on the presence of documentation in the clinical record.
6. Strictly adhere to all codes of conduct and ethics as required by the BBS, LSSSC Employee Handbook, and contractual requirements.
7. Participate in the rotating On-Call Schedule for the program, which includes the Family Urgent Response System (FURS) for the Success First Program.
8. Work a flexible schedule, with the ability to offer In-Person services to clients during the evening, weekend, and holiday hours out in the field or client's residence.
9. Using the On-Call schedule for the program, be available to address the regular and emergency needs of the clients, including emergency response availability, call back staff, and offer support for in-person crisis response during and after regular working hours.
10. Attend all training (both internal and external) as required by the contractor, the Clinical Supervisor, or other LSSSC management.
11. Meet and maintain specific program target productivity goals as assigned. Manage workload to meet program target goals by the end of each month and the fiscal year.
12. Maintain client list every month, including but not limited to removal of clients not being seen, the addition of clients being seen, closing clients who are non-compliant with services in a timely fashion, and assurance that clients are seen within the Plan of Care (POC) and clinical guidelines.
13. Document in the EHR clinical follow-up for no-show clients with attempted phone contact the same day.
14. Participate in Chart Monitoring/Review as assigned.
15. Provide accurate statistical information on client care as requested within assigned deadlines.
16. Respond to County and LSSSC plan of improvement for corrective action within one week of receipt.
17. Engage in continuous and open communication with the Clinical Supervisor and Clinic Director regarding clients, problems with documentation, and any other issue which arises while providing services.

The SocialWorker will be responsible for working with the child, resource family, biological family,

ESSENTIAL JOB FUNCTIONS AND RESPONSIBILITIES INCLUDE THE FOLLOWING:
Be knowledgeable of the SocialWorker Manual, Program Statement, Title XXII, and Agency policies and guidelines.
2. Conduct an intake meeting with the client, placement worker, natural parent, and resource parent, if all
parties are available, when the child is placed with the Agency. Explain the program expectations and
complete necessary intake paperwork, including the Appraisal Needs and Services Plan, or 30 day initial
Treatment Team Review.
3. Following the Intake meeting, hold and create an initial treatment plan report within 30 days of
placement, in coordination with other treatment team members...
4. Conduct a Treatment Team Review (TTR) on a quarterly basis to review progress made towards
established goals following the TTR procedures. Prepare a quarterly report immediately following the
TTR, and submit to the supervisor within 10 working days.
5. Provide weekly consultation to the treatment resource parent, in the resource home. Consultation will
consist of discussion of child's progress towards goals as defined in the Treatment Team Review, as
well as teaching and discussion of therapeutic interventions, disciplinary techniques and resource parent
training. SocialWorker will provide and/or coordinate referrals for additional services as needed (i.e.
medical, psychiatric, etc.)
6. Provide weekly support and case management to enable child to accomplish treatment goals. If a child
has needs that require the services of an outside therapist for a particular issue, the SocialWorker will
work on other issues with the child in coordination with the outside therapist, for purposes of
consistency and support of written treatment goals.
7. Document child's weekly progress at home, school, and/or work, major incidents, visitation and other
significant, collateral contacts.
8. Obtain approval from placement worker prior to allowing any contact between the child and others.
Approve and facilitate all ongoing visitations with child.
9. Encourage resource parent to involve child in community activities and extra curricular activities.
10. Consult supervisor regarding incidents, significant issues, and possible terminations, and follow-up as
directed.
11. Meet with supervisor for a minimum of one hour weekly of individual supervision and case
consultation.
12. Ensure that client files in the facility and office are complete and up-to-date.
13. Utilize KaleidaCare web-based system to maintain client documentation, including weekly contact
notes, SIRs and personal documentation for the client.
14. Submit all reports and documentation to supervisor for final approval.
15. Complete monthly foster home inspection forms to ensure compliance with CCL regulations.
16. Facilitate resource parent and co-worker training, and support groups as directed by supervisor.
17. Complete initial permanency assessments and written reports for potential resource parents. Submit
resource parent annual assessments for recertification and as needed updates to this assessment as
required by Community Care Licensing.
18. Provide on-call crisis intervention. Perform the function of duty-worker at the office as requested.
19. Conduct monthly and quarterly foster home inspections and address compliance issues as needed.
20. When the Skills Trainer is working with a family and/or child on the SocialWorker's case load, the
SocialWorker will consult/supervise the Skills Trainer weekly regarding goals and/or progress. The
SocialWorker must first obtain approval from their supervisor to utilize the services of a Skills Trainer
prior to scheduling an initial appointment.
21. At all times project a favorable image of Walden Family Services.
22. Performs other duties as assigned or requested.

* Provides residents with information about available services, including services related to crisis intervention, childcare, medical and dental care. Actively assists residents with accessing needed services.
* Develops, presents, plans, and/or conducts activities and/or trainings to participants
* Work with the Program Manager/ Director and county staff to receive and review referrals, interview applicants, determine eligibility for the program, and determine needs of the potential participant.
* Participates in 24 hour on-call program/crisis intervention services for participants, on a rotating schedule with other designated team members as needed and required.
* Attends all participant, program, and county meetings, such as one on one supervision, staff meetings, etc., as requested and required.
