Position Specific Information This position is public facing and may be located between 3205 Freedom Drive, Charlotte, NC 28208 and between partner locations in which people seeking shelter may attend. Standard hours of Sunday - Thursday from 8:00 am to 5:00 pm. Subject to change, based on need. Mecklenburg County is the Lead Agency for Coordinated Entry (CE) for the Charlotte-Mecklenburg Continuum of Care. Coordinated Entry is the intake for the local homeless response system. It is part of the process to access homeless resources and shelter in Charlotte-Mecklenburg. This Social Worker I position conducts Coordinated Entry (both in person and telephonically) with individuals and families experiencing homelessness, or those at imminent risk of homelessness, to efficiently connect them to the best available (scarce) homeless shelter, temporary housing, or homeless prevention options in the community. It may require the provision of short-term Systems Navigation support to clients until which time they are connected to a homeless services case manager and/or their situation resolves. This position also engages clients within a strengths-based, solution focused framework to identify their service needs and goals, and to facilitate referrals to community resources so that clients have a plan of action toward enhanced self-determination. CE clientele is diverse and brings with them a wide range of experiences and concerns; and may be in different stages of mental health, substance use, and trauma recovery. In addition to clients, the Social Worker I requires the ability to effectively engage with community partners to provide on-site CE to clients wherever they may be (in places not meant for human habitation, hospitals, treatment centers, day centers, shelters, etc.) Position Summary Provides community-based services to clients, including evaluation, case management, counseling, and referrals. This position, under direct supervision, is responsible for determining the individual's specific problems and assisting in developing an action plan and/or mutual responsibility agreement. Essential Functions Assess needs of individuals and families Interview clients, obtains family, social, and medical information Determine clients' eligibility for financial assistance and if referrals are appropriate Provide counseling and services in both individual and group settings Discuss goals, expected outcomes and means with which to meet goals and expected outcomes Assist clients in the completion of required paperwork for items that may determine program eligibility, referrals, or provide job assistance, such as job applications Manage records and maintains history records and prepare reports of services provided Refer clients to community resources for services and coordinates benefits and referrals to other agencies Develop and evaluate service plans in consultation with clients, and perform follow-ups assessing the quantity and quality of services provided Help clients work with government agencies to apply for and receive benefits Transport clients to appointments such as medical, job-related, or others, if appropriate Manage records and documentation of services provided and participate in team case conferences Act as a liaison with other staff with referrals and transitioning of clients May be required to staff shelters in the event of a disaster Minimum Qualifications Experience: Bachelors of Social Work from an accredited school of social work and one year of social work experience; or a qualifying internship; or successfully completion of the NC Child Welfare Collaborative program; or a bachelor's degree in a related field with 15 hours of social work/counseling courses and two years of social work experience; or Master's degree from an accredited school of social work; or a master's degree in a related field with one year of social work experience. The duties listed below are not all that may be assigned but are considered essential for an employee to perform. * Works a night duty schedule including nights, weekends, and holidays. Provides for constant coverage of the Child Abuse Hotline by responding timely to callers who contact the Child Abuse Hotline. Responsibilities include gathering, exploring, documenting, interpreting, evaluating, and staffing with the superior's information from reporters of child protective services issues via telephone calls, office visits, mail, and e-mails. This worker is responsible for making a recommendation to superiors as to whether the information reported meets North Carolina statutory definitions and NCDHHS-Division of Social Services Policy guidance around abuse, neglect, and dependency of children. This worker must conduct responsibilities in a hasty and time-sensitive manner in order to facilitate the initiation of accepted Child Protective Services Reports within required timeframes. This worker is responsible for evaluating needs, referring to helping agencies, and evaluating appropriateness for DHHS interventions. * Assesses reports of abuse, neglect, or dependency of children using either a forensic investigative assessment approach or a family assessment approach, depending on the severity of the maltreatment reported, and in accordance with state law and divisional policy governing such. Preliminary reviews existing DSS documentation (if such exists) and staff response process with the Supervisor. Conducts a comprehensive initiation of Child Protective Services reports including interviews of all victim children, all caretakers, all persons living in the home, all appropriate collaterals (as indicated by both the reporter and the family being investigated), and all other persons, entities, agencies, professionals, relatives, etc. who might have knowledge about family relationships, behaviors, and incident (or incidences) that prompted the report to DSS. Works collaboratively with law enforcement and prosecutors on joint investigations of a forensic nature. Follows mandated timeframes and procedures for appropriate initiation and continuous process of assessment. Observes the home structure and environment, ensuring that persons, provisions, and location are able to meet the basic needs of each child involved in the report. Completes safety assessments and other tools as appropriate with the family to promote a transparent process. When necessary, explore alternative safety resource placement options with the family, in order to avoid removal of physical or legal custody. Engages the family in planning for safety and well-being, encouraging immediate action and front-loading services. Incites the professional opinions of necessary contributors (law enforcement, medical personnel, mental health professionals, etc.) to determine the required safety measures adequately * Completes referrals from daytime staff by responding to and completing requested activities for Child Protective Services and Foster Care cases as needed after hours. Responds to emergencies that arise outside of office hours. * Invokes court action when children cannot be safe in their homes, seeking direction through legal consultation, and completion/filing of all necessary court paperwork. Accompanies law enforcement in taking physical custody of child(ren). * Participates in forensic joint interviews at the local Child Advocacy Center on appropriate cases (with law enforcement and other appropriate professionals). * Participates weekly in unit/team staffing, presenting completed case assessments for unit/team decision-making. Solicits guidance, direction, and support from the supervisor individually at any point in case and/or requests assistance in completing tasks, if needed. * Documents all activities, contacts, dates, and times using a structured documentation tool. Requests necessary supporting documentation from external sources and places it in the case file. * Participates in local, professionally-enhancing, and state-mandated trainings. Participates in monthly MDT meetings to discuss specific cases in a multi-disciplinary setting.
