Ennoble Care is seeking a Full-time SocialWorker Care Manager (Hybrid) to service our patients within and around the Oklahoma City, OK area. This role is well-suited for early-career socialworkers looking to build experience in integrated behavioral health within a collaborative, team-based environment.
This position will be primarily remote with occasional travel to patients homes/facilities.
Responsibilities:
Support the mental and physical health care of patients on an assigned patient caseload.
Screen and assess patients for common mental health and substance abuse disorders. Facilitate patient engagement and follow-up care.
Provide patient education about common mental health and substance abuse disorders and the available treatment options.
Systematically track treatment response and monitor patients (in person or by telephone) for changes in clinical symptoms and treatment side effects or complications.
Support psychotropic medication management by consulting with the behavioral health consultant and then communicating with the primary care provider assigned.
Provide brief behavioral interventions using evidence-based techniques such as behavioral activation, problem-solving treatment, motivational interviewing, or other treatments as appropriate.
Provide or facilitate in-clinic or outside referrals to evidence-based psychosocial treatments (e.g. problem-solving treatment or behavioral activation) as clinically indicated.
Participate in regularly scheduled (usually weekly) caseload consultation with the psychiatric consultant and communicate resulting treatment recommendations to the patient's medical provider. Consultations will focus on patients new to the caseload and those who are not improving as expected under the current treatment plan. Case reviews may be conducted by telephone, video, or in person.
Track patient follow up and clinical outcomes using a registry. Document in-person and telephone encounters in the registry and use the system to identify and re-engage patients. Registry functions can be accomplished through an EHR build, on a spreadsheet used in conjunction with an EHR, or can be built into a stand-alone clinical management tracking system that may or may not be linked to an EHR.
Document patient progress and treatment recommendations in EHR and other required systems so as to be shared with medical providers, psychiatric consultant, and other treating providers.
Facilitate treatment plan changes for patients who are not improving as expected in consultation with the medical provider and the psychiatric consultant and who may need more intensive or more specialized mental health care.
Facilitate referrals for clinically indicated services outside of the organization (e.g., social services such as housing assistance, vocational rehabilitation, mental health specialty care, substance abuse treatment).
Develop and complete relapse prevention self-management plan with patients who have achieved their treatment goals and are soon to be discharged from the caseload.
Be comfortable utilizing Excel and Word and completing assessments and productivity trackers daily.

Provides or supervises plan of care interventions to include resource and referral help, crisis intervention, prevention, education, patient advocacy, bereavement, therapy (LMSW under supervision) and support as appropriate. Partners with the healthcare team and involves the patient and family in the development and implementation of plans.
Provides patients and family education regarding diseases, treatments, social or financial situations, emotional stress and/or other problems
Monitors care to evaluate progress towards goals and institutes appropriate interventions as needed
Conduct high risk screening, provides crisis counseling (LMSW under supervision), and provides education and resources to patients and families regarding abuse or neglect, physical abuse, and documents these sessions.
Acts as a liaison between patients/families and the clinical staff and other agencies
Advocate for patient concerns and escalate ethical dilemmas through the appropriate channels.
Maintains an open, approachable manner treating others fairly and respectfully preserving self-confidence and dignity as well as show regard for their opinion
Completes applicable reports bases on mandated reporting requirements.
LMSW or LCSW with 1 year experience can precepts newly hired SW Level I or BSW and MSW students.

Education Requirements: Master of Social Work from an accredited institution required
Experience Requirements: 0-3 years of work experience, health care experience preferred.
License/Certification/Registration Requirements: Current Licensed Master SocialWorker (LMSW) or under supervision for Licensed Clinical Social Work (LCSW) or LCSW from the Oklahoma State Board of Licensed SocialWorkers. Current Basic Life Support certification from the American Heart Association required

Psychosocial must become part of patient's chart within 24 hours of completion.
Regularly assess the patient and family psychosocial needs.
Regularly assess the pre-bereavement/anticipatory grief needs.
Implement the Plan of Care provisions to meet their needs, which include: social service goals for alleviating problems, supportive counseling, problem solving, community referrals pre-bereavement and bereavement care.
Regularly re-evaluate the patient's and family's psychosocial needs.
Participate in weekly interdisciplinary team meetings.
Assist with making funeral arrangements if requested.
Provide counseling during the process of the terminal illness and at the time of death if requested.
Required Skills
Qualifications:
Master's Degree from a school of social work accredited by the Council on Social Work Education
Minimum of two year social work experience in a healthcare setting or social services agency
Current SocialWorker license
Hospice experience a plus
Knowledge of and ability to obtain community resources (information and referral, state/federal financial programs and eligibility requirements, etc.).
Understand Hospice philosophy, principles of death/dying
