The Hospice Social Worker will provide direct social work services to all terminally ill patients and their families experiencing social, emotional, environmental and economic difficulties related to the impact of a terminal illness. Time-management skills are a must and the BHCM needs to be able to manage their day effectively through calendars and the EMR system. This role is PRIMARILY telephonic, however in person visits are done and the BHCM has to be comfortable completing them both ways. Qualifications: The Social Worker Care Manager will have a minimum of MSW Preferred Licensure or provisionally licensed Behavioral Health experience and knowledge Must be licensed driver with an insured automobile in good working order Ability to communicate effectively, both verbally and in writing. Time Management skills Prior behavioral health experience is helpful but not required. New MSW graduates are encouraged to apply.
