Working under the supervision of the PACE Center Manager, the PACE Social Worker is independent in the application of social work knowledge and skills. He/she is responsible for providing direct social work care management services to the participants of the North East Medical Services PACE program and works collaboratively with the PACE Interdisciplinary Team (IDT) to manage long-term care needs.
ESSENTIAL JOB FUNCTIONS:
Conducts psychosocial assessments to determine PACE Participants' care needs, preferences, and goals.
Participates as an active member of the IDT for Participant care planning.
Documents accurately and thoroughly in the Participants' electronic health record in accordance with industry and departmental standards including time requirements.
Communicates effectively and openly with other Interdisciplinary Team members on participant status and needs.
Maintains current and thorough written case management records, including periodic reassessments of program Participants.
Provides supportive counseling to Participants and/or their family members, as indicated.
May develop and lead group counseling, support groups, and/or other relevant activities.
Refers Participants and families to appropriate community agencies or facilities, acts as a liaison with such organizations, and as an advocate for participants.
Consults with and advises staff members regarding the social, emotional, and cultural factors associated with health and medical care, and as to the availability of social services in the community.
Works with the IDT to manage smooth care transitions between settings (hospitals, skilled nursing facilities, home, etc.)
Participates in program and policy development related to social work.
Understands and demonstrates respect for Participants' rights and utilizes established mechanisms for management of ethical issues in Participant care.
Demonstrates active participation in Quality Improvement activities, staff training, and meetings.
Performs home visits, as necessary.
Works closely with the Enrollment Team to support enrollment of prospective Participants in the PACE program.
Provides care to Participants in-person, via telehealth, and/or via telephone, as indicated.
Utilizes Epic appropriately and efficiently and serve as content and workflow expert for resolving issues and proposing solutions in Epic.
Performs other job duties as required by manager/supervisor.
May provide after-hours on-call coverage to support PACE participants when the PACE Center is closed as needed for support.
QUALIFICATIONS:
Required:
Master's degree in Social Work from an accredited college or university required.
Current BLS and First Aid required
Preferred:
Current CA licensure as a Licensed Clinical Social Worker (LCSW) preferred
Minimum 2 years of experience in a health-related area preferred
Minimum one year of experience working with a frail and/or older adult population preferred
Valid California Driver's license with good driving history within the last 5 years.
LANGUAGE:
Must be able to fluently speak, read and write English.
Fluency in Chinese (Cantonese and/or Mandarin) is required.

Under general direction, provides skilled social work services to cases requiring maximum service, and involving a variety of complex social problems and treatment plans. Social Worker III positions in the Social Services Agency, Department of Family and Children's Services work in a variety of assignments connected to child safety, well-being, and permanency. The Social Worker III duties may include: receiving and making reports of alleged child abuse and/or neglect; assessing and investigating allegations of abuse and/or neglect; developing appropriate interventions, providing referrals for services, and monitoring services for children and parents; removing children from unsafe environments; providing case management services; preparing documentation, case plans, safety plans, Court Reports, and other legal documents; providing testimony in Court; assessing and evaluating potential caregivers; placing children into appropriate homes; conducting home visits with children, caregivers, and parents; and assessing and addressing issues specific to the children and families circumstances such as substance abuse, intimate partner violence, mental health issues, sexual abuse, parenting, developmental issues, immigration, and psycho-social-emotional-behavioral issues.

In addition to sign-on bonus, this classification is scheduled to receive salary range increases on the following dates:
Effective June 22, 2026 - 4.00% increase
Additional benefits include:
* Paid differentials (e.g. bilingual, assignment) may be given to a hired applicant.
* DFCS Social Worker III assigned to Emergency Response receive a 9% differential above the base salary plan. All other DFCS Social Worker III receive a 6% differential above the base salary plan
Possession of a Masters's degree in Social Work may be required for specific programs.

Possession of a Master's degree in Social Work preferred or possession of a Master's Degree in Behavioral Science, Counseling, Education Counseling, Gerontology, Marriage and Family Therapy, Nursing, Psychology, Social Psychology, Vocational Rehabilitation or equivalent titles to the above degree from an accredited college.
AND
Three (3) years of social work experience of which two (2) years are equivalent to a Social Worker II in Santa Clara County.
OR
Possession of a Master's Degree in Social Work, as a Title IVE student
AND
Two (2) years of experience equivalent to the classification of a Social Worker II in Santa Clara County.
Master's degree in Social Work may be required by statute for certain positions.
Possession of a valid California driver's license prior to appointment and the ability to qualify for and maintain a County Driver's permit.

Under general direction, provides skilled social work services to cases requiring maximum service, and involving a variety of complex social problems and treatment plans. This list will be used to fill any current or future vacant positions Adult Protective Services (APS) within the Social Services Agency, Department of Aging & Adult Services (DAAS). Social Worker III's in these departments provide services to elder, disabled, and/or dependent adults.
In the IHSS program, Social Worker III's may be assigned to case carrying assignments, evaluating and assessing yearly service needs based on Medi-Cal, Medicare, and SSI eligibility. Candidates must be able to conduct field visits to complete initial and yearly reevaluations of clients' service needs.
In Adult Protective Services (APS) program, Social Worker III's may be assigned to receive, assess, and document reports of Elder or Dependent Adult abuse and neglect, investigate allegations, and document case-related activities. The ability to apply critical thinking skills and to develop professional collaborative partnerships with multi-disciplinary teams, county and community programs and law enforcement agencies are highly desired. A Master's Degree in Social Work is preferred for Social Worker III positions in APS.
