Such contact may include, but not limited to, primary care physicians, insurance companies and other service providers involved in the care of clients. * Conducts risk assessment and formulates Safety Plans. * Provides appropriate case management, including work with Erie County Department of Social Services (ECDSS), schools, probation, Primary Care Providers (PCP), mental health court, criminal justice system and any other applicable external agencies. * Responds and directs care appropriately for clients in crisis. * Utilizes a harm reduction model in treatment. * Performs (or coordinates) toxicology screenings as clinically appropriate. * Provides Social Skills Training, Substance Abuse Treatment, Behavioral Activation, Coping Skills Training, and Psycho-education. * Rotating on-call responsibilities with other team members. * Meets agency Key Performance Indicators. * Maintains client confidentiality at all times following all agency and HIPPA policies. * Completes all trainings required by the agency. * Performs all other duties as assigned. QUALIFICATIONS * Minimum Master's degree in Social Work, Mental Health Counseling, Marriage & Family Therapy, or CASAC-T/CASAC-P required. * Professional licensure: LMSW, LMFT, MHC-P, CASAC, CRC, LMHC, LCSW preferred. * 2 years of experience working with children & families is preferred * Must have knowledge of a variety of intervention techniques, including motivational enhancement; common mental disabilities, chemical dependency interventions and resources. * Excellent communication skills. * Must be familiar with community resources to be able to appropriately link clients. * Minimum proficiency with Windows based software applications is required. * Must have experience with Electronic Medical Records databases and basic computer skills. * Must appreciate cultural diversity and be aware of cross cultural counseling issues. Provide individual, family, and relationship-based counseling, supportive counseling, and solution-focused interventions. Teach coping and emotional regulation skills to improve functioning in daily life. Provide post-crisis management and stabilization support in the home and community. Collaborate with community partners, providers, and informal supports as part of a multidisciplinary team. Maintain knowledge of community resources for linkage and referral. Document all services in the agency's electronic case recording system, ensuring accuracy and compliance with Medicaid billing standards. Maintain confidentiality of all youth and family information. Attend required agency and OMH trainings and participate in regular supervision. Perform all other duties as assigned. Knowledge, Skills, and Abilities Ability to work effectively with youth, families, staff, and community partners from diverse backgrounds. Strong understanding of behavioral, emotional, and mental health challenges in children and adolescents. Strong collaboration and communication skills. Valid NYS driver's license, adequate auto insurance, and reliable transportation. Ability to pass an annual physical for Category 1 job classification. Qualifications Bachelor's degree in a human services field plus two years of related experience; OR Registered Professional Nurse plus one year of experience; OR Master's degree in a human services field plus one year of related experience.
