Horizon Health is seeking a certified therapist for our BRAND NEW opioid treatment center in Petersburg, VA The Opioid Treatment Program (OTP) Therapist/ Counselor is responsible for providing counseling, educational, and referral services to enrolled patients and their families as defined by OTP protocols. Counseling services include individual, group, and family counseling. The OTP Therapist/ Counselor is responsible for providing a range of cognitive, behavioral, and other substance use focused counseling, reflecting a variety of medically necessary evidence-based, individualized, person-centered care. The OTP Therapist completes individual psychosocial assessments and assigns treatment objectives and goals accordingly. The OTP Therapist works closely with other members of the interdisciplinary team to provide comprehensive care to enrolled patients.
Responsibilities:
Acts as primary counselor to address substance use disorders.
Develops individualized, PCP and its ongoing revisions in coordination with the Client and ensures its implementation.
Provides ongoing assessment and reassessment of the Client based on their PCP and goals.
Provides individual and group counseling based on the Client's individualized, PCP.
Provides crisis interventions, when clinically appropriate.
Provides substance use, health and community services education.
Provides coordination and consultation with medical, clinical, familial, and ancillary relevant parties with Client consent.
Ensures linkage to the most clinically appropriate and effective services along with arranging psychological and psychiatric evaluations.
Informs the Client about benefits, community resources, and services.
Advocates for and assists the Client in accessing benefits and services.
Monitors and documents the status of the Client's progress and the effectiveness of the strategies and interventions outlined in the PCP.
Other duties as assigned.

The preferred candidates would provide crisis stabilization services to adults, adolescents and children who are at imminent risk of hospitalization or an out of home placement. The counselors will work with identified individuals who are experiencing acute and severe mentalhealth conditions in the community setting.
Mobile Crisis Response at CSS is implemented utilizing a Team Approach, consisting of a team of two (2) Professionals who are a part of a integral Clinical team consisting of a Clinical Supervisor dedicated to their team, as well as Clinical Manager, and Clinical Director.
Supervision for Mobile Crisis Response Teams at CSS are done in "Real Time", and during the implementation of services weekly. This can be up to 6 Supervisions weekly between you and your QMHP teammate.
Clinical MCR teams at CSS cannot consists of teams where both individuals are QMHP's.
In addition to direct client care, Mobile Crisis Counselors are responsible for coordinating services according to clients treatment plans, facilitating, engaging families in crisis intervention and stabilization services to provide complete wrap-around services, and facilitating admissions and discharge preparation from the program. Experience providing Emergency Services, MCR, or Community Crisis Stabilization services and/or knowledge of community mentalhealth services and resources are preferred for this position.
Coordinate the development and timely submission of Individualized Service Plans and all other required program documentation
Maintain and ensure compliance with DBHDS and Medicaid standard
Ensure all Serious Incident Reports are completed within 24-hours
Meet with clinical supervisor weekly to staff and assign/reassign cases
Perform client file audits as needed to ensure compliance with all Medicaid and DBHDS standards
Ensure safety planning is completed on day 1 of service initiation due to the acute nature of symptoms and short admission time period
Ensure all Human Rights reportable incidents are investigated and reported according to agency policies and all necessary steps are taken to protect clients'
Adhere to all agencys policies/procedures and complete other tasks assigned by Crisis Supervisor and Program Director for successful operation of the program
**Must have the Mobile Crisis Responder Training

About the Opportunity
The Group Home Day Counselor is responsible for the treatment of up to 8 youth in a teaching-family home under the direction of the Group Home Manager. This position provides teaching, guidance and supervision

· Maintain weekly program activities calendar, ensuring alignment with the house budget.

· Conduct vehicle checklist and document accordingly.
· Ensure food supplies are sufficient to meet the needs of the posted menu (coordinate with Staff Supervisor regarding shopping/budget). Food bank pick-up.
· Ensure essential emergency supplies are available, such as first aid kits, flashlights, batteries, snow shovels, ice melt (seasonal)
· Ensure general office supplies are sufficient for operation of the home.
· Take minutes during team meetings
· Ensure vehicles have adequate gas for planned outings.

Services include assessment, brief counseling and consultation, action planning, referral to resources (assuring linkage as appropriate), and follow-up as indicated.
* CYB counselors will also have a focus on supporting the staff and personnel of DoDEA , community schools, summer camps (if available/assigned) as well as providing parent training and guidance.
* Provides training and health and wellness presentations, participates in health fairs and other base/installation activities focused on children and youth.
* Enters counselor activity data daily through smart phone, or web application, to assure that reporting is accurate from assigned installation while maintaining confidentiality and anonymity of service/family member.
* Establishes and maintains working relationships with community resources focused on children and youth and provides appropriate linkages. Consistently visible within the school which allows MFLC services to be accessible when working with children, the counselor must abide by line of sight protocol (another adult with responsibility for the child must be able to see the counselor and child they are working with at all times).
* Partners with POC to provide CYB services in a manner that addresses the needs of the installation/facility. Develops an effective and professional working relationship with the installation/facility POC.
* Manages duty to warn and restricted reporting situations according to DoD protocol and staffs the cases with Regional Supervisor/Regional Director.
* Communicates with Regional Supervisor and participates in regular individual and group supervision, sharing information regarding trends and issues on the installations and in facilities to which they are assigned. Responds to critical incidents and special requests as directed by the POC and approved by the Office of Secretary of Defense (OSD) program manager. The CYB will flex schedule to respond to urgent requests as needed or at the request of the Regional Supervisor/Regional Director.
* Participates in regular in-services/training, Quality Improvement committees, initiatives, studies, pilot programs other contract activities as assigned/appropriate as directed by the customer and/or Magellan. This includes participation in pilot program certification and training processes, completion of activity documentation, integration of pilot activities in non-medical counseling work, and the utilization of applicable technology to complete required activities.

The counselors will work with identified individuals who are experiencing acute and severe mentalhealth conditions in the community setting.
Mobile Crisis Response at CSS is implemented utilizing a Team Approach, consisting of a team of two (2) Professionals who are a part of a integral Clinical team consisting of a Clinical Supervisor dedicated to their team, as well as Clinical Manager, and Clinical Director.
Supervision for Mobile Crisis Response Teams at CSS are done in "Real Time", and during the implementation of services weekly. This can be up to 6 Supervisions weekly between you and your QMHP teammate.
Clinical MCR teams at CSS cannot consists of teams where both individuals are QMHP-Trainees.
In addition to direct client care, Mobile Crisis Counselors are responsible for coordinating services according to clients treatment plans, facilitating, engaging families in crisis intervention and stabilization services to provide complete wrap-around services, and facilitating admissions and discharge preparation from the program. Experience providing Emergency Services, MCR, or Community Crisis Stabilization services and/or knowledge of community mentalhealth services and resources are preferred for this position.
Coordinate the development and timely submission of Individualized Service Plans and all other required program documentation
Maintain and ensure compliance with DBHDS and Medicaid standard
Ensure all Serious Incident Reports are completed within 24-hours
Meet with clinical supervisor weekly to staff and assign/reassign cases
Perform client file audits as needed to ensure compliance with all Medicaid and DBHDS standards
Ensure safety planning is completed on day 1 of service initiation due to the acute nature of symptoms and short admission time period
Ensure all Human Rights reportable incidents are investigated and reported according to agency policies and all necessary steps are taken to protect clients'
Adhere to all agencys policies/procedures and complete other tasks assigned by Crisis Supervisor and Program Director for successful operation of the program
**Must obtain QMHP registration from the Virginia Board of Counseling
**Must have reliable transportation
**Must possess a valid driver's license
**Must be able to obtain an official copy of your college transcript
**Must possess an ACTIVE valid car insurance coverage
This position is a Full-Time position with a starting pay of $30.00 per hour for individuals currently holding a QMHP-Trainee and $27 per hour for PRS, and CSAC-A.
For individuals with a QMHP-A, or C, and a CSAC with the starting pay of $40.00 per hour.

We are also offering all New Hires for Mobile Crisis Response, a Sign-On Bonus of $500 after a positive 90 Day Evaluation, and eligibility for $500 Performance Bonuses Bi-Annually.
The Mobile Crisis program is available 24/7 as a part of the MCR contract with C.S.S. , and will be required to sign an employment disclosure and employer binding agreement upon hire.

EMS of Virginia is seeking a skilled assessor who will complete assessments for our community-based programs, including Intensive In-home, MentalHealth Skill Building, and Crisis Services. As well, the individual will provide Intensive In home Counseling to children and families. The successful individual will be licensed in the state of Virginia as an LPC or LCSW, or be eligible for supervision toward licensure. EMS will provide the licensure supervision to the right candidate, as an added benefit of the position.
Duties include:
Meet with clients in person or virtually to conduct comprehensive mentalhealth assessment
Complete assessment documentation in electronic health record (Lauris)
Gather information for submission of request of authorization to Medicaid MCOs
Electronically submit authorizations requests to MCOs
Communicate with assessment and supervisory team about the process
Conduct individual and family counseling in the home and via telehealth
Complete treatment plans
Complete progress notes
Conduct care coordination with collaterals
Successful candidate will be available 20-40 hours per week. Scheduling will be flexible based on client need and request for services.
