Your Job as a Substance Use Counselor:
The Substance Use Counselor is instrumental in our patient's treatment and recovery from opioid use disorder. Substance Use Counselors provide high quality, compassionate care through various mediums which include both individual and group counseling sessions.
Job Responsibilities:
Provide high quality, compassionate guidance in both individual and group counseling sessions.
Plan, oversee, facilitate and document patient's recovery.
Co-facilitate assigned group or family sessions as needed.
Ensure all documentation regarding patient care, treatment, and incidents is completed timely and in a clear, concise manner.
Prepare individual treatment plans for each assigned patient.
Initial assessments as well as follow up assessments.
Evaluate patient needs and determine if referrals to other programs or facilities are needed.
May plan for aftercare for assigned patients.
Provide crisis intervention to patients, as needed.
Provide case management duties for patients, ensuring individualized quality care as needed.
Act as a liaison between referral sources and patients, as needed.

Required Education, Skills, and Qualifications:
Bachelor's or Master's degree in social or health services field is required; degree must be from an accredited college or university.
Minimum of 1 year of professional counseling experience is required.
Previous experience in addiction recovery or behavioral health settings, such as outpatient, residential, or correctional facilities, is highly preferred.
* Familiarity with Medication-Assisted Treatment (MAT), including methadone, buprenorphine, and naltrexone.
* Experience conducting individual and group counseling sessions focused on substance use recovery.
* Knowledge of evidence-based practices, such as Cognitive Behavioral Therapy (CBT), Motivational Interviewing (MI), and relapse prevention techniques.
* Documentation experience, including maintaining clinical notes, treatment plans, and progress updates in accordance with state and federal guidelines.
* Familiarity with state regulatory standards (e.g., 42 CFR Part 2, HIPAA, Joint Commission standards).
* Experience collaborating in a multidisciplinary team with medical staff, case managers, and peer support specialists.

POSITIONS SUPERVISED: NA
POSITION OVERVIEW: The Residential Behavioral Health Specialist (BHS) is responsible for maintaining a safe, sanitary, supportive, and stable environment in residential facilities. BHS will be trained to use a Housing First model when working with residents to complete goal orientated tasks, building independent living skills and promoting mentalhealth recovery. BHS will apply a harm reduction and trauma informed approach to recovery orientated tasks to support mentalhealth and/or sobriety. BHS must assign and supervise tasks carried out by residents, offer teaching opportunities and support to residents, be able to provide daily support of medication management skills, and provide transportation as needed. BHS will be expected to coordinate with Team Leads, Housing Case Managers, internal agency, and other stakeholders to provide collaborative care to residents. Completion of state required, annual internal, and program specific training will be necessary prior to service delivery. BHS will be responsible for daily record keeping in compliance with contract expectations and foodbank guidelines.
ESSENTIAL POSITION RESPONSIBILITIES:
Provide services to maintain required productivity / billing standard of 85% of all hours worked.
Complete electronic documentation in clients' electronic medical record in a manner that individualizes each note, reflects appropriate interventions and progress towards goals, and meets the standards for medical necessity.
Maintain accurate and timely documentation of service provision, signing all progress notes within 24 hours of service delivery, or as per contract guidelines.
Maintain a safe, clean, and sanitary environment within the Residential Care facility and submit timely facility requests. Night support staff are expected to complete deep cleaning and ongoing cleaning of facilities, as outlined in daily checklists.
Completing and documenting safety drills as assigned, maintaining policies and procedures compliant with Health & Safety expectations and contract guidelines.
Complete nightly room checks per program guidelines to ensure the safety and well-being of all residents.
Provide motivation, support, and instruction for residents through designated service provisions to promote development of responsible living patterns, achieve a more adaptive level of psychosocial functioning, self-sufficiency in health and personal hygiene, improved interpersonal and community relationships, facilitate movement towards obtaining an affordable, stable, and permanent living environment, and employment.
Provide support for an alcohol / drug abuse-free lifestyle environment conducive to social interaction and the fullest development of the resident's rehabilitative potential.
Assess, problem-solve, and effectively handle situations requiring crisis intervention including documenting and incidents reporting to the resident's Housing Case Manager (HCM) and supervisor per department, agency policy and contract expectations.
Work collaboratively with supervisors, HCM's, residents, team, external stakeholders, and community providers to facilitate contract expectations and resident's individualized plan of care. Provide daily correspondence to team.
Provide coverage of facilities as requested in the event of emergency, holidays, vacations, or illnesses.
Supervise and monitor medication intake, ensure on-going changes to medications are communicated with the team to ensure an up-to-date Medication Summary Record (MSR) as per Residential Care Policy.
Requirements
POSITION REQUIREMENTS: BHS is expected to have a high school diploma plus one-year post-high school education or equivalent experience in the human services field. Preferred areas of experience include working with SPMI population and/or dually diagnosed population and working in residential and/or shelter-based settings. Must demonstrate effective communication and documentation skills as well as the ability to develop and maintain rapport with residents, constituents, and staff. Staff must possess basic computer and typing skills. Must complete all required training within designated timeframe. A valid Kansas driver's license and access to personal vehicle required. Valid annual TB test required for some programs.
OTHER POSITION REQUIREMENTS:

Requirements
Bachelor's Degree (B.S./B.A.) in early childhood education, social work, or related degree. If education does not include at least 9 hours of Early Childhood coursework, must commit to obtaining that additional education. An Associate's Degree will be considered with five years of relevant experience and an educational plan to attain a Bachelor's Degree in a relevant field.
Requires 3 years experience in either Early Childhood Education or in the field of childhood MentalHealth and Disabilities working with children with IEPs/IFSPs, or Behavioral Plans.

You'll care for members in an outpatient clinical setting, primarily via telehealth, but also in the home and community as needed. You will be part of a multidisciplinary team made up of Psychiatric Nurse Practitioners and non-clinical Community Guides to collaboratively provide a holistic approach to member health, and support you to work at the top of your license. Together, this team is responsible for providing and coordinating member-centered quality care for a panel of members with complex health and social needs.
Reema is expanding our service delivery into behavioral healthcare, so we are looking for someone who is comfortable helping create the details of our care model within the ambiguity of a fast-paced startup. For the right person, this position has the potential to grow into a leadership role.
Primary Responsibilities:
Assess and diagnose behavioral health conditions, both chronic and acute.
Formulate treatment plans for members in consultation with other members of the collaborative health team as the member's condition warrants.
Ensure a behavioral health perspective is incorporated into comprehensive care planning for complex patients.
Complete initial intakes and assess patient risks and needs, delivering therapeutic interventions and therapy, and referring and navigating to community resources for access to the broader behavioral health continuum of care.
Collaborate with primary care providers, psychiatrists, social workers, or other professionals to discuss treatment plans and progress.
Provide psychoeducation to members, emphasizing mentalhealth promotion and disease prevention
Support referral for members who need additional specialty behavioral health care (e.g. to an in-patient facility) and advise on treatment plans until members are engaged in appropriate care.
Establish and foster trusting relationships with patients and ensure care is aligned with the patient's goals and values.
Participate in multi-disciplinary case conferences to ensure holistic and appropriate care for patients.
Provide formal and informal coaching and education to other clinical and non-clinical team members about supporting patients with behavioral health issues.
Support post-discharge planning to prevent readmissions.
Timely and appropriate documentation in our EMR to manage member care and coordination with the multidisciplinary team.
Contribute to the evolving design and implementation of clinical pathways and protocols for therapeutic treatment of behavioral health conditions in Reema's care model to ensure we are providing the best patient care possible.

Experience and Skills:
3+ years of outpatient experience working with adults enrolled in Medicaid, Medicare, or are dual-eligible.
3+ years of experience in assessing, planning, and managing behavioral patient care as acquired through direct clinical experience, including direct therapy and care/case management work
3+ years of experience working with individuals who have significant mental and chemical health needs and believe that recovery is possible.
1+ years of experience practicing telemedicine
Experience with underserved populations facing socioeconomic barriers to health care
Excellent working knowledge of trauma-informed and evidence-based psychotherapy treatment for mental and chemical health conditions
Awareness and comfort in participating in an integrated behavioral health model where you will work alongside Community Guide team members
Comfort with electronic medical record documentation and excited about how technology can support your work and drive ongoing improvement towards new and better care
Ability to make quick and accurate clinical assessments of mental and behavioral health conditions
Ability to exercise balanced judgment in evaluating situations and making decisions, and to handle difficult or confrontational situations in a calm, consistent, and equitable manner
