ROLE OVERVIEW and PURPOSE
The County Medical Director will strategically design and enhance patient care delivery within our healthcare system by implementing innovative approaches to care team formation and patient allocation. Reporting directly to the Associate Chief Medical Officer (ACMO), this position will oversee multiple clinics across the county, ensuring optimal utilization of clinical resources and improving patient outcomes. The County Medical Director role represents a critical opportunity to optimize patient care delivery, improve clinical outcomes, and enhance operational efficiency within our healthcare system. By leveraging interdisciplinary care teams and innovative patient allocation strategies, we can ensure that patients receive the right care from the right providers at the right time, ultimately advancing our mission of providing high-quality, patient-centered care.
RESPONSIBILITIES
Leadership and Vision:
Provides strategic leadership to drive growth and development in areas of underserved medical needs within a designated county
Develops and forecasts long-term plans (three to five year planning) to enhance healthcare delivery and meet the evolving needs of the community
Team Management:
Manages a medical team of Clinician Leaders, including Senior Regional Medical Leads, and Associate Regional Medical Leads
Establishes and manages interdisciplinary care teams composed of Medical Doctors and Advanced Practice providers, as well a per diem Provider pool to optimize patient care across clinics
Fosters a culture of collaboration, accountability, and continuous improvement within the medical team
Operational Optimization:
Collaborates with the operations team to establish constructive workflows that optimize clinician efficiency and enhance patient care outcomes
Designs and implements strategies to streamline processes, reduce inefficiencies, and enhance overall operational performance
Care Team Development:
Collaborates with clinical leadership to create multidisciplinary care teams, consisting of medical doctors and advanced practice providers
Participates in examining patient complexity to appropriately allocate patients to Medical Doctors or Advanced Practice Providers
Establishes systems for accurately assessing patient panels and allocating patients to appropriate care teams based on clinical complexity
Provides guidance on patient allocation protocols to ensure that complex cases are managed by physicians, while routine visits are handled by advanced practice providers
Conducts training sessions for clinicians on team-based care models, patient allocation strategies, and best practices in patient management
Utilization Management Oversight:
Oversees the clinical component of utilization management, ensuring appropriate and judicious uses of medical services are consistently rendered
Collaborates with utilization management teams to develop and implement strategies for optimizing resource utilization, identifying/referring to appropriate inhouse services, minimizing unnecessary costs, and promoting evidence-based care practices
Workflow Management:
Evaluates and measures workflows to effectively manage care teams and facilitate cross-coverage to ensure continuity of care
Develops protocols and guidelines to standardize clinical practices and enhance coordination among team members
Direct Care:
Administers or prescribes treatments
Discusses potential side effects to medications and/or immunizations with patients
Promotes health by advising patients about diet, hygiene, and methods for disease prevention
Refers patients to medical specialists for consultant services for their well-being, as needed
Follows up with patients regarding progress in high risk or emergency cases
Conducts physical examinations to provide information needed for admission to schools, consideration for jobs, or eligibility for insurance coverage
EDUCATION/EXPERIENCE
Medical Degree (MD or DO) from an accredited institution required
Valid CA Medical and DEA License required
Board certification in a medical specialty required
Valid BLS certification from approved American Health Association provider required upon hire
Seven years' clinical experience in primary care or relevant specialty
Experience in higher-level leadership roles within healthcare organization

The PACE Bus Driver is primarily responsible for providing safe transportation to participants in a timely and competent manner and communicating important information relevant to the overall care plan to the multidisciplinary team.
Responsibilities
* Operate a 12-passenger bus safely in accordance with traffic laws and Neighborhood's policies.
* Transport participants to and from the day center, medical facilities, and other scheduled outings.
* Assist participants in boarding and exiting the vehicle safely, including those using mobility aids such as walkers or wheelchairs.
* Conduct pre-trip and post-trip vehicle inspections and maintain accurate records.
* Maintain cleanliness and basic upkeep of the vehicle.

* Maintains standard records, (e.g., logs of passengers and miles driven, service records, gasoline purchases)
* Promptly reports to appropriate staff regarding participant-related concerns noted in home or on the bus; writes progress notes for documentation
* Delivers medications and other supplies to participants
* Follows all safety guidelines and standards; attends safety in-services and offers guidance or assistance to staff working in an unsafe manner
* Reports accidents and incidents to supervisor immediately
* Follows proper infection control procedures and maintain supply of emergency clean up kits
Qualifications
Education/Experience
* High school diploma/GED preferred
* Valid CA Class A, B, or C commercial driver's license (CDL) with passenger endorsement required
* Two years medical, ADA, pupil, and/or elderly transportation experience preferred
* Clean driving record (free from major infractions including, but not limited to, DUI, negligent, reckless, or excessive minor infractions and convictions) required
* Successful completion of defensive driving and passenger assistance training courses within three months of hire
* Valid BLS certification in accordance with the American Heart Association Guidelines required upon hire

The Echocardiography Technologist will perform all types of echocardiography testing. Neighborhood Healthcare PACE is a managed medical plan built around surrounding participants with a team of physicians, nurses, social workers, therapists and care coordinators to help them maintain good health and a good quality of life. Our goal is to keep our seniors happy and healthy at home surrounded by their family and community.
As a private, non-profit 501(C) (3) community health organization, we serve over 500k medical, dental, and behavioral health visits from more than 100,000 people annually. With two PACE centers located in Riverside County, our PACE program is positioned to serve over 650 senior participants.
The Physical Therapist Assistant works to support the mission and vision of Neighborhood Healthcare by supporting participants rehabilitative physical therapy (PT) treatment. This role will assist physical therapist with participant evaluations and treatment plans for program participants. In addition, this role will assist participants to remain independent and live safely in the community.
This role/site is eligible for a $2,500 signing bonus!
Schedule: M-F 7:30/8:00am-4:00/4:30pm. This position is fully onsite.
Responsibilities
Assists physical therapist implement PT programs for participants accessing day services, including therapeutic exercises, neuromuscular reeducation, rehabilitation services, functional activities, gait trainings, orthotics trainings, prosthesis, and trainings
Makes recommendations to the physical therapist to modify the program based on the patient population, as needed
Performs assessments of patients to determine abilities
Assists physical therapist implement individualized plans of care to decrease or eliminate disabilities, increase or maintain capabilities for independence, and develops functions to a maximum level
Ensures individual plans of care are followed accurately
Provides a variety of physical therapies aligned to participants plans of care, including functional ability tests, treatment to relieve pain, and develop or restore function
Assists participants to achieve and maintain maximum performance using physical means, such as exercise, heat, sound, water, light, or electricity
Guides patients and caregivers in the use of adaptive devices and therapeutic, creative, and self-care activities for improving function
Evaluates the effectiveness of treatments and makes recommendations to modify goals, as needed
Records and signs treatments in the patient's health record
Ensures services provided are in the clinic are conducive to restoring dignity and self-esteem to all patients
Assists physical therapists to ensure that required equipment and supplies for the physical therapy program are available and/or ordered, as needed
Keeps abreast of all regulations and standards to ensure compliance with orders or directives issued by duly constituted government and regulatory agencies (PACE, CMS, DHCS)
Participates in training programs for staff and caregivers
Qualifications
Education/Experience
Associate degree in physical therapy assisting required; an emphasis on geriatric populations is preferred
California Physical Therapy Assistant license in good standing is required
One year of physical therapy assistant experience working with the geriatric population preferred
Basic Life Support (BLS) through the American Heart Association certification required upon hire
Bilingual (English/Spanish) highly preferred

The Social Worker will manage a caseload of participants by providing a psychosocial perspective to the interdisciplinary evaluation, assessment, plan of care, ongoing services, and disenrollment processes of the PACE program at Neighborhood Healthcare. This role will collaborate with the interdisciplinary team to optimize the health status and quality of life of Neighborhood PACE participants. Additionally, this role will liaise between participants, members of participants' support network, and the interdisciplinary team.

Responsibilities
Works with the PACE Interdisciplinary Team (IDT) to conduct initial, semi-annual, unscheduled, and annual assessments
Provides participant health status updates to appropriate staff in daily meetings
Obtains extensive psychosocial history from participants and/or family members upon admission to program and intermittently at pre-enrollment
Coordinates, schedules, and facilitates family conferences in conjunction with the PACE IDT to address levels of care, medication/treatment non-compliance, out-of-home placement, complex diagnoses, behavioral concerns and contracts, conditions of involuntary disenrollment, and alternative program options
Develops and facilitates various group counseling topics designed to assist at-risk populations over the age of 55 with chronic health conditions
Provides individual caregiver and family support counseling, as needed
Coordinates with participants, family/caregivers, and primary care providers to complete advance life planning documents, assist with end-of-life planning, and provide educational resources
Conducts independent risk and safety assessments at skilled nursing facilities, residential care facilities for the elderly, assisted living facilities with/without memory care units, hospitals, and in-home, as needed
Screens and develops share of cost for out-of-home placements based on financial documentation from participants and family members
Creates and executes supportive housing forms with care facilities and family members,
Conducts intermittent and annual income reviews to update supportive housing forms and share of costs
Schedules, coordinates, and accompanies participants and family members with transportation and admission to higher level of care to promote safety and continuity of care in cases of high acuity
Implements hospice and palliative care services by providing referral documentation, coordination, education, and support to participants and families
Communicates with hospitals/skilled nursing facilities and utilizes external medical records systems to coordinate tailored discharge planning for participants
Assists with ongoing financial eligibility for participants, including Medi-cal recertifications, as needed
Liaises effective connections and communications with other organizations in the eldercare field
Provides support and education to staff members in areas of social, emotional, and cultural factors in the participant population, including how these factors relate to health, medical care, and the availability of social services in the community
