Responsibilities TECHNICAL TASKS * Perform bedside neurology/stroke care patient/family evaluation. * Assist with diagnosis, treatment, and symptom management of acute and chronic health conditions. * Order, perform, and interpret treatments within scope of professional practice. * Consult program medical director as needed; comply with applicable laws and regulations with respect to Collaborative agreements. * Initiate reimbursement for services rendered. * Assist in development of clinical practice guidelines/standards/protocols in support of quality palliative care. * Provide training and continuing education for staff on palliative care. * Meet monthly to provide reports and outcomes in relation to Stroke Care team goals and objectives. * The leadership role of the Practitioner includes patient and family expectations; group dynamics to affect positive change; and provides a strong image as a role model. The practitioner will also supervise all RNs who are assisting with the program. 2. INTERPERSONAL RELATIONS * Communicates and collaborates with physicians, nurses and other multidisciplinary team members involved in the care of patients. * Acts as patient advocate constantly monitoring, tracking, assessing and evaluating quality of care * Promotes communication with referring physicians and the entire team regarding plan of care * Promotes professional development of the team. 3.
