Identify diagnoses to be used in care management and active medical management. Provide care coordination to alleviate identified needs related to behavioral, physical, and social issues. Focus on addressing issues that will decrease medical costs and increase the quality of life. Develop positive relationships with clients, caregivers, providers, and other team members. Managing physical health problems through the provision of education. Communicate gaps in care to the member's primary care physician if needed. Home safety evaluations. Under the supervision of a physician, the nurse practitioner visits patients, completes physical assessments, performs face to face assessments and evaluates hospice eligibility as assigned in consultation with the hospice interdisciplinary team. The nurse practitioner builds from the resources of the community to plan and direct services to meet the need of individuals and families within their homes and communities. DUTIES & RESPONSIBILITIES Patient Care Provides medical care, palliates and manages terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician.
