**You will:**
+ Provide Primary and Family evidenced-based care inclusive of accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning for our patients.
+ Provide administrative duties to include but not limited to opening/closing the clinic, inventory, patient follow up, verifying insurance and collecting payment, maintaining a clean environment.
+ Be responsible for handling patient appointments and performing routing examinations to ensure positive patient health outcomes
+ Perform duties including speaking with patients about healthy lifestyle choices, completing diagnostic tests to help diagnose and treat patients for injuries or illnesses and coordinating with other healthcare professionals to maintain a comprehensive plan of care
+ Encourage patients to become active participants in their own healthcare and well-being through services such as health screenings, behavioral health and wellness, chronic condition management
**Required Qualifications:**
+ **A minimum of one year of experience as a Nurse Practitioner is required. Experience in Family medicine or primary care practice preferred.**
+ Active Family Nurse practitioner license in good standing
+ Provide holistic, evidenced-based care inclusive of accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning for our patients. This includes education and treatment for pregnancy prevention, sexually transmitted infection (STI) Prevention and safer sex practices.
+ Order and administer immunizations and injections (to include but not limited to COVID19, influenza, TB, Depo) and provides evidence-based patient/parent vaccine counseling and education.
+ Minimum of two years of medically relevant experience or equivalent
+ Complete the DOT certification exam within 30 days of employment
+ Depending on the market, the ability to be proficient in both speaking and writing in additional languages not limited to but including Spanish
+ Basic Life Support (BLS) certification, including but not limited to performing CPR and operating an Automated External Defibrillator (AED).
+ Pass a FIT test and wear the provided respirator mask when protocol requires.
**Education:**
+ Master's Degree level Family Nurse Practitioner program with current National Board Certification and State of Employment license to practice in the Advanced Practice Nurse role
+ Or in select states, Master of Science in Physician Assistant Studies (or other health-related master's degree or equivalent experience together with Bachelor of Science in Physician Assistant Studies) with current National Board Certification and State of Employment license to practice in the role.

**Pay Range**
The typical pay range for this role is:
$46.03 - $99.14
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.

About the Role
During a visit that can last up to one hour, Matrix providers review and observe a member's current health, medical history, medication adherence, social environment and other risks. This provides unmatched insight into a member's overall health and well-being that can be difficult to capture during routine office visits. The Matrix Comprehensive Health Assessment (CHA) helps to improve quality of care and allows us to potentially close multiple care gaps with a single visit. Our providers:
* Conduct adult / geriatric assessments to include medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and clear documentation according to Matrix guidelines and protocols
* Administer and order point of care and lab-based testing as needed
* Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate

Job Description

The Advanced Practice Provider will perform face-to-face, in-clinic assessments/evaluations with Veterans to complete the required Disability Benefits Questionnaire (DBQ) for submission to VA for eligibility determination. DBQs include questions that require a direct clinical exam of the Veteran (e.g., testing range of motion of joints, documentation of crepitus, etc.).
In addition to VA-related assessments, this role includes providing occupational health services for various government departments and agencies. These services may include:

Job Duties
Evaluate and manage chronic pain conditions as part of comprehensive care plans, integrating pharmacologic and non-pharmacologic therapies.
Collaborate with pain management specialists and complementary providers (chiropractors, acupuncturists, massage therapists) to develop individualized multimodal pain treatment strategies.
Perform interventional pain procedures as trained and credentialed, including joint injections, trigger point injections, and other minimally invasive therapies.
Educate patients on safe pain management practices including opioid stewardship, alternative therapies, and self-management techniques.
Monitor treatment efficacy and adjust care plans to optimize function, quality of life, and long-term health outcomes.
Deliver care using a holistic, interdisciplinary approach in collaboration with medical doctors, chiropractors, acupuncturists, behavioral health providers, dietitians, and other health partners within our patented care model.
Participate in a multidisciplinary team to develop, execute, monitor, and adjust comprehensive and coordinated care plans that include shared goals with measurable outcomes.
Assess and document patient health through interviews, examinations, observations, and review of medical histories.
Diagnose health conditions, order and interpret lab work and diagnostic studies following established protocols.
Document all patient encounters and sign medical charts within 72 hours as per industry standards and guidelines.
Develop treatment plan as part of patient's overall care plan that addresses dysfunction and disease while minimizing risk of further injury.
Prescribe and dispense medication when granted authority in accordance with State licensure and staff privileges.
Employ evidence-based approaches to shared decision-making that respect the patient's preferences and social norms while targeting optimal treatment outcomes.
Qualifications
Experience in pain management or interest in developing expertise in this area preferred.
Comfort in managing patients with chronic pain using a multimodal, evidence-based approach.
Current Board Certification as a Nurse Practitioner or Physician Assistant required.
Current DEA registration (Drug Enforcement Administration) or eligibility/ability to obtain DEA registration required.
Current Physician Assistant or Nurse Practitioner licensure in the state of California.
1+ years of relevant work experience as Physician Assistant/ Nurse Practitioner preferred but not required.
Bilingual (English/Spanish) is a plus but not required.
