Principal Accountabilities

Transitions between inpatient/outpatient.
Directs and evaluates throughput; delegates or provides backup assistance including triage.
Ability to float, round to multiple facilities and/or take call (first call).
External Ventricular Drain (EVD) Insertion (Mischer Neurosciences only).
Provides accurate billing, coding and documentation.
Completes medical records and submit charges in a timely manner.
Responsible for overall unit performance in collaboration with physicians or other providers.
First Assist physicians in performing surgical procedures by providing support as needed.
Obtains medical histories; performs physical examinations/ reviews/orders/interprets diagnostic tests and radiographic results; formulates and manages patient treatment plan and outcomes; provides interventions and performs procedures within legal cope and authority that the APRN or PA is approved, delegated to and credentialed to perform including but not limited to incision and drainage, pelvic exams, lesion removal/destruction, suturing, and casting/splinting.
Collaborates in the development and evaluation of patient care. Updates clinical guidelines/protocols/pathways; works autonomously and accepts accountability for outcomes; signs orders in accordance with institution and state board requirements; collaborates in research and teaching projects and applies research to practice.
Uses interpersonal and verbal communication skills to educate patient/family on care resources; participates in process improvement and multidisciplinary clinical outcome improvement activities utilizing advanced nursing expertise.
Ensures safe care to patients, staff and visitors; adheres to all Memorial Hermann policies, procedures, and standards within budgetary specifications including time management, supply management, productivity and quality of service.

The After-Hours NP will manage acute and chronic primary care needs during
evenings and weekends, helping expand patient access while maintaining continuity of
care. This position blends urgent care-style visits with traditional family practice,
supported by clear protocols, strong MA/front-desk coverage, and on-call physician
support as needed.
SCHEDULE
Wednesday - Friday: 3:00 p.m. - 9:00 p.m.
Saturday & Sunday: 7:00 a.m. - 7:00 p.m.

KEY RESPONSIBILITIES
1. Direct Patient Care
Evaluate and treat patients across the lifespan for:
o Acute illnesses (URI, flu, COVID, UTI, GI complaints, minor infections)
o Minor injuries and musculoskeletal complaints
o Chronic condition follow-ups as appropriate
o Same-day urgent visits
Perform comprehensive histories and focused physical exams.
Diagnose, treat, and manage conditions within family practice scope.
Provide evidence-based, patient-centered care.
2. Preventive & Primary Care Support
Address preventive care needs when appropriate during visits.
Review medication lists and manage chronic medications safely.
Order and interpret labs, imaging, and diagnostic tests.
Provide patient education, counseling, and clear follow-up instructions.
3. Care Coordination & Continuity
Document visits clearly to support continuity with daytime providers.
Coordinate referrals, follow-ups, and next-day care as needed.
Communicate urgent findings to on-call physicians or leadership.
Ensure smooth handoffs to weekday primary care teams.
4. Documentation, Coding & Compliance
Maintain timely, accurate documentation in the EHR (eCW experience preferred).
Ensure documentation supports appropriate coding and medical necessity.
Adhere to state, federal, and payer regulations.
Follow all Wellspire clinical protocols and SOPs.
5. Patient Experience & Professionalism
Deliver compassionate, respectful, and efficient care.
Manage high-volume periods calmly and professionally.
Communicate clearly with patients and families.
Uphold Wellspire's standards for service, safety, and ethics.
QUALIFICATIONS
Required
Current, unrestricted Texas NP license
Family NursePractitioner (FNP) certification
Active DEA license or eligibility to obtain
Minimum 3+ years of experience in Family Practice, Urgent Care, or Primary Care
Strong clinical assessment and decision-making skills
Comfortable managing patients independently during after-hours shifts
Preferred
Experience in urgent care, after-hours clinics, or extended-hours settings
Comfort with high-acuity primary care complaints
