Job Summary\- Responsible for setting up and administering resident medications as ordered by physicians and per professional standards of medication administration. Providing direct care to residents, communicate residents' needs effectively, attend\/participate in scheduled in service\/training programs, assist with programming activities, adhere to safety and infection control policies and procedures, promote positive public relations for facility and perform other tasks as required by Director of Nursing or designee.
Qualifications
Valid QMA certificate from an accredited program. At least one year experience in long term care, assisted living preferred.

Review medication sheets for completeness and accuracy.
Report all discrepancies noted concerning physician s orders, medication count or any other unusual situations and complaints regarding resident services and care to your immediate supervisor.
Chart and perform documentation duties are required in accordance with community policy and procedures.
Assist in admit, transfer and discharge of residents as requested.
Review charts for specific treatment, medication, diet orders, etc. as needed.
Notify nurse in charge of changes in resident condition, (including medication and treatment refusal).
Make sure work areas (medication and treatment cart, medication room, nurse s station, etc.) are maintained in a clean, sanitary manner.
Always show courtesy, respect and positive attitude to residents, family s visitors and associates.
Report accidents and incidents observed on the shift they occurred. Report suspected abuse immediately.
Document important events in the 24-hour report.
Assist with activities of daily living.
Encourage the Social development of each resident.
Assist with light housekeeping including emptying the trash and changing linens.
Escort residents to and from dining areas for meals; assist with tasks related to dining as needed.
Perform any duties delegated by supervisor in order to maintain the self-respect, personal dignity and physical safety of each resident while adhering to the standards of Residents Rights and all applicable state, Federal and local licensing requirements and regulations.

About the role:
The PRN hospice aide is a paraprofessional member of the interdisciplinary group who works under the supervision of a registered nurse and performs various services for a patient as necessary to meet the patient's personal needs and to promote comfort. The PRN hospice aide will be assigned in a manner that promotes quality, continuity, and safety of a patient's care.
Job Responsibilities:
1. Performing personal care needs after given a patient's assignment and attending to his/her requests promptly. If unable to perform a certain task, report to the Case Manager immediately.
2. Providing patient and family with positive communication techniques, within the realm of ethical and respectful care, including confidentiality.
3. Meeting the safety needs of the patient and using equipment safely and properly (foot stools, side rails, O2, etc.)
4. Giving personal care including baths, back rubs, oral hygiene, shampoos, and changing bed linen as often as assigned.
5. Assisting in dressing and undressing patients as assigned.
6. Planning and preparing nutritious meals, including shopping, as assigned.
7. Assisting in feeding the patient as assigned.
8. Taking and recording oral, rectal, and auxiliary temperatures, pulse, respiration, and blood pressure when ordered with appropriate completed/demonstrated skills competency.
9. Providing proper care and observation of the patient's skin to prevent the breakdown of tissue over bony prominence.
10. Assessing and reporting on the patient's condition and significant changes to the Case Manager. Also aware of the caregiver or other individuals living with the patient and interpersonal issues.
11. Assist in ambulation and exercise as instructed by the hospice nurse or therapist.
12. Provide a normal range of motion and transfers/positioning as directed.
13. Assistance with self-administered medications as allowed by state regulations, such as “reminders.”
14. Offering and assisting with bedpans and urinals. Assisting as assigned with light laundry needs.
15. Performing range of motion and other simple procedures as an extensional therapy service as ordered with appropriate completed/demonstrated skills competency.
16. Providing respite for patient's/family/caregiver when on-site, as appropriate.
17. Keeping the patient's living area clean and orderly, as assigned.
18. Adhering to the organization's documentation and care procedures and standards of personal and professional conduct.
19. Participates in quality assessment performance improvement teams and activities.
20. Respectful of patient and family/caregiver environment and patient's personal needs.

Job Qualifications:

Position Summary:
The Hospice Aide reports to the Clinical Manager and is expected to function as a member of the health services team. Provides care, treatment, and services in accordance with standards of practice, practice guidelines, law, and regulations.
Appropriate and safe techniques in performing personal hygiene and grooming tasks. Gives the patient personal care, including assistance in the activities of daily living including hair shampooing in bed and recognizing and reporting changes in skin condition.
Recognize and report changes in patient skin condition (including pressure ulcers) to supervising professional (Staff Registered Nurse, Clinical Manager, and other appropriate skilled professional).
Performs or assists with treatment procedures including prevention of decubiti, warm and cool applications, simple unsterile dressing changes, and reinforcing other dressings.
Takes and records temperature, pulse, respiration, blood pressure, and weight according to Hospice Aide Care Plan.
Prepares and maintains clinical documentation according to Agency policies and acceptable nursing standards.
Reports cases of possible abuse, neglect, fraud, non-compliance, and exploitation to the Compliance Officer, Administrator, or Designee immediately.

Contacts the Clinical Manager to inform him/her of alternate schedule for patient visits during vacation, sick days, or personal days off. Reports any schedule change or inability to complete assignment as scheduled.
Accepts changes/additions to schedule during staffing difficulties when necessary. Accepts assignment in time of emergency, regardless of any location and/or any day of the week be it weekend/holidays.

Required Experience for Medication Aide:
QMA Licenses Required
High School Diploma or GED required.
Must have aptitude for dementia-related situations.
Must be at least 18 years of age.
Must have a TB clearance.
Responsibilities for Medication Aide:
Prepare and administer medication and treatments as ordered by physician in accordance with policy and procedures.
Verify identity of resident before administering medication or treatment.
Follow hand-washing policy for medication passing, universal precautions and infection control.
Order mediations, supplies, and equipment as necessary in accordance with established policy.
Remove expired or discontinued medications for cart and resident apartments as required in accordance with established procedure.
Ensure medication records are accurate for your shift and keys are always accounted for.
Review medication sheets for completeness and accuracy.
Report all discrepancies noted concerning physician's orders, medication count or any other unusual situations and complaints regarding resident services and care to your immediate supervisor.
Chart and perform documentation duties are required in accordance with community policy and procedures.
Assist in admit, transfer and discharge of residents as requested.
Review charts for specific treatment, medication, diet orders, etc. as needed.
Notify nurse in charge of changes in resident condition, (including medication and treatment refusal).
Make sure work areas (medication and treatment cart, medication room, nurse's station, etc.) are maintained in a clean, sanitary manner.
Always show courtesy, respect and positive attitude to residents, family's visitors and associates.
Report accidents and incidents observed on the shift they occurred. Report suspected abuse immediately.
Document important events in the 24-hour report.
Assist with activities of daily living.
Encourage the Social development of each resident.
Assist with light housekeeping including emptying the trash and changing linens.
Escort residents to and from dining areas for meals; assist with tasks related to dining as needed.
Perform any duties delegated by supervisor in order to maintain the self-respect, personal dignity and physical safety of each resident while adhering to the standards of Residents Rights and all applicable state, Federal and local licensing requirements and regulations.
